Employee states its a progressive pain, pain progressed over a few days and reported on 5-26-2026, no known injury. While lifting and packaging stone employee experiences pain in right forearm extending down to EE's ring finger. Seen Dr. no tests performed, told to use ibuprofen and ice, but placed on restricted duty. Follow up with Dr. in one week.

Showing all 6While lifting up a stone tread off of a stack of treads to see the bottom side, the tread slipped and fell onto the employees left middle finger pinching it between two treads. Employee suffered a smashed open fracture/crushing injury to the finger with nail damage.
While moving a large stone on a conveyor belt EE smashed right index finger between the stone and the conveyor frame. Laceration on top and bottom of finger with swelling. Urgent Care cleaned and inspected wound for any broken bones & none were found. Returned to work with no restrictions. EE was completely discharged from their care after a 2 day follow up.
EE was in Building #18 operating Saw #4. While loading a larger stone on the feed conveyer they tried pushing the stone up against the stone in front of it on the conveyor belt. EE then heard a pop in left chest and felt some pain. The Doctor says they fractured a rib on the left side.
EE woke up 11/19/20 with pain in neck & worked all day. EE reported issue to mgt. 11/23/20. EE did not recall any specific incident causing pain. EE was pushing/pulling slabs on gravity roller conveyor leading to hyd. splitter. EE indicated EE did nothing at home to cause injury. Dr. report 11/23/20 indicates sprain of ligaments & strain of other muscles @ left shoulder upper arm.
While packaging stone EE attempted to hand a co-worker a rock which slipped out of EE's hand and landed on EE's left foot directly behind steel toe. X-ray of Left foot revealed a fracture at base of Left 5th toe. EE did not seek medical attention until pain and swelling developed on morning of 11/4/20.
