Injured was helping 2 other miners re-rail a 555 scoop at the 230 foot mark of the slope track. Two airbags were placed to lift the scoop, blocking was being installed to support the scoop to deflate the bags when a crib block shot out from underneath an airbag and struck the injured employees hard hat, resulting in a small contusion to EEs forehead and a sprain/strain to EEs neck.

Employee was installing roof bolts on the right side of the continuous miner. EE had completed drilling a hole and was pushing a roof bolt up in the hole when EE felt a "pop" in EE's right shoulder. Injured employee was helping hang the continuous miner cable. Lifted it to EE's shoulder and then up into a hanger. Felt pain in EE's shoulder and later after work, EE noticed a small knot and swelling in EE's shoulder. Reported the incident on 3-9-26 and sought treatment that day and began missing work. No rules or regulations were violated and EE Was wearing required PPE.

Employee was operating a roof bolter installing bolts on the 8W CM Section. EE stuck EE's left hand down on the bolter head to check suction and at that time, a drill steel fell out of the hole EE was drilling and smashed EE's left little finger against the drill head. Employee suffered 2 small fractures to the end of EE's little finger. Was wearing metacarpal gloves.

Injured loading top belt structure into a belt tub in the Sugar Run Supply yard with the rest of belt crew. When EE placed the structure down the other employee pushed it over before injured's gloved finger was out of way and pinched it between structure and tub resulting in a chipped bone. No problems with equipment or compliance issues, training or PPE.

Injured was assisting pulling longwall hoses at the Longwall belt drive using a sheave wheel connected to a roof hanger. A nylon rope was run through the sheave and connected to the hoses. As the rope was being pulled, the hanger broke and the sheave struck employee's right knee. Employee was behind guarding but moved away from it during the process.

Initially treated for appendicitis, but no evidence of this was found at follow up appointment, so employee was diagnosed with an unspecified abdominal muscle strain. No problems with equipment or compliance issues, training or PPE.

The EE reported a back strain while lifting and moving a vulcanizing press from a belt line UG to a track on a flat car on a mine site. At a later time the EE received treatment at a medical facility and an over counter prescription resulting in a recordable incident due to the original injury.

Injured picked up a lump of coal off the mine floor near 150 shield (6W) to throw into pan, but EE was closer to the pan than EE thought and struck EE's hand on the edge of the pan, pinching right ring finger between pan and coal, resulting in a fracture and open wound. No problems with equipment or compliance issues, training or PPE.
