Employee was replacing the cutting edge of a loader when the carriage bolt dropped down from its mounting slot, causing the bolt to spin and cut through EE's glove, causing a quarter inch deep laceration, about 2 inches long, in the palm of EE's left hand

Maintenance replacing the trunnion/bearing rollers on the rotary screen. While attempting to remove the strap holding the roller/bearing, the weight shifted and caused the roller and bearing to shift and struck the Techs right hand causing a laceration to EE's (R) index finger, requiring 7 stitches.

Employee was relocating a waterline pipe in the shop to be fabricated and pinched EE's pinky finger between the pipe and floor when setting the pipe down. Employee was taken to the urgent care to find a fractured fingered.

The employee was assisting in loading an X-ray machine into the back seat of a pickup truck. The weight of the machine shifted in EE's hands unexpectedly, resulting in EE's hand being pinched between the corner of the machine and the center B pillar/door jam of the vehicle, resulting in a laceration to the right ring finger.

Two maintenance employees were adding a return roller and brackets to BC-124. While putting the roller into the brackets one side went in with ease. That employee then went to help EE's partner by lifting the belt. The struggling employee hit the roller with EE's had causing it to fall out of the bracket and land on the side of EE's boot fracturing EE's pinky toe

While lifting the lump basket from the loading opening of a bulk pneumatic trailer FLW experienced a "pop" in the center of the back. Continued working until about 10:00 AM until they determined they would require medical attention. Diagnosed with a "lumbar back strain".

Maintenance Mechanic working on a bearing to a fan completed the repairs and was re-installing a pulley guard, the guard got stuck and as the Mechanic pulley on the guard to free it to complete the installation EE's (R) ring finger and pinky finger got pinched between the guard and the fan. Causing a fracture to the tip of EE's ring finger.

Employee was attaching a trailer mounted pump to a track hoe bucket in order to move the pump into position. The track hoe bucket unexpectantly lowered, catching the employee's hand between the bucket and pump frame.

Fluid bed sand dryer was plugged with wet sand. Operator was checking the progress of unplugging efforts by opening an access door to the discharge chute. Operator did not stand aside when opening the door and was struck by hot sand. Injuries sustained were first and second degree burns to face, head, neck and upper arms.

Two employees were moving a tote of coag using a pallet jack. One employee was pulling on the pallet jack handle while the other was pushing the tote. The employee pushing on the tote felt a pop in their left calf muscle.

Employee grabbed a 5-gallon water jug and took it to the filter belt building. As the employee was attempting to place it in the hoist basket the employee had to move a used belt to make room for the water jug. As the night went on the employee experienced some discomfort in their right leg groin area. Employee sought medical treatment on 10/16/2023.

Employee restacking pallet of 50 lb paper bags to different size. Near the end of the pallet, lifted bag about head height, when EE felt pain in the left shoulder. EE finished the pallet and reported the injury. Advised to perform duties as tolerated by local facility, was able to perform all shipping duties. Taken to ortho on 9/12, modified at that point.

Employee was grabbing samples at BC5340 and went to open lid but it was locked. As the employee tried to pull the lid open and they felt a pain in their right hand up to their right shoulder. Did not become a lost time reportable until 01/17/2024.

In attempting to remove 150lb guard for install, injured EE hand was in line of fire when other EE lowered guard thinking they were out of the way. The worker was replacing a bad bearing on SR-02A. The worker had to perform lifting and hammering during the removal of the old bearing and installing the new. The front line maintenance worker's left elbow is sore. The new bearing weighs 87.5lb.

While working in the Air Separator (GSAS-01) at the Grinding Mill, a maintenance EE's hands slipped of a Whizzer bar EE was removing. There was a sharp piece of metal protruding which lacerated the EE's right thumb between the 2 knuckles. EE received 4 stitches to their thumb.

Employee was on ladder of railcar, reaching for a portable electric hoist (~20-25 lbs), grabbed the hoist and rotated EE's left arm (with the hoist in hand) to hand off to another operator, when EE felt a pop and pain in EE's upper left shoulder. Employee went to the emergency room that day and was placed on light duty, to include no lifting, pushing or pulling.

EE was placing a fan blade on the maintenance shop table when it slipped and dropped causing a laceration to EE's index finger on EE's left hand. After removing the fan blade from the motor shaft, EE removed EE's gloves to take a break but then decided to sit the fan blade on the table to get it off the floor.

The lead mechanic/electrician reached out to pull electrical wire to place on a pole and heard right shoulder pop. First visit to a medical doctor was Monday 06/07/2021. During this exam the mechanic was placed on restricted duties.

A Relief Operator while closing a valve in the crushing area felt a pop in left elbow. Employee immediately reported the occurrence to supervisor. As the shift progressed the pain, motion, and hand grip strength worsened causing the employee to request a medical evaluation.

The employee was pushing with right foot (up to 20 lbs. of force) on a metal truck inspection ramp connected to two cables with counterweights and heard a pop and felt pain in right knee. After months of therapy and an MRI, it was determined the employee had a torn meniscus. The employee worked up to scheduled surgery on 09/23/2021.

While assisting with a pushout in our Rail Yard, the employee opened the knuckle to the railcar they were connecting to, the front of the knuckle fell off and struck EE's left foot. The knuckle was missing the connecting pin. EE reported the incident to EE's supervisor who took EE to the ER, they did an x-ray and it showed EE fractured EE's fourth toe.

Employee was hanging 50lb empty bags on loading spout and felt a pop/pull in EE's right forearm. Massaged it for a while and continued working thru EE's shift. Came back the next day worked half a shift doing same job then changed jobs for 2nd half. Not sure of damage, was told to follow up with an orthopedic doctor.

Maintenance was unloading an empty oxygen bottle from the maintenance truck to replace it with a full one. As they attempted to lift the bottle, they felt a sharp sensation in lower back and numbness in right leg and toes. Employee was taken to a local medical facility and evaluated. Employee was returned to full duty and upon a follow up appt, changed to modified duty.

Miner was moving a sand tilt hopper using a forklift. Once the hopper sand contents were discharged, the hopper did not return to its upright position. Miner then parked and dismounted forklift and pushed hopper back in an upright position, straining EE's shoulder.
