Pump Checker was hooking up the 8"" x 20' pipe to the pump; they had to drag the hose about an inch to get it lined up with the connection when they felt something pull in left lower abdomen.
Maintenance employee hurt right forearm while chipping ice off BC2120 head wheel. Employee was using an electric demo chisel in the blistering cold weather around 11:30pm Sunday Jan. 25th. Both their forearm and elbow hurts with numbness in right hand.

Millwright was working out of a Manlift. They knelt down on left knee and put all of their weight on a bolt that was laying on the floor of the basket. This caused pain in their knee. This happened on 10/14/25, they did not report it to employer until 11/5/25. Once they sought medical they were sent to Physical Therapy on 12/17/25 which at that time made it a reportable incident.
Maintenance was changing a vertical driscoll pipe. The rigging let go and a miner was struck by the pipe from behind. Bump on the forehead, sore neck, shoulders and upper back. EE did not seek medical until 6/8/25, that is when EE was put on restrictions.
Sandbagger was assisting with loading bags, and a bag fell off a pallet on the conveyor rollers. Employee made sure the conveyor was shut off and then stepped on the roller to adjust the bag. As the employee was lifting the bag back onto the pallet, they felt discomfort in their right groin.

Employee was putting a return roller into place on conveyor, and it kept falling out of place. Employee kept catching it with their left hand over their head and then felt pain in their left shoulder.

Employee grabbed a 5-gallon water jug and took it to the filter belt building. As the employee was attempting to place it in the hoist basket the employee had to move a used belt to make room for the water jug. As the night went on the employee experienced some discomfort in their right leg groin area. Employee sought medical treatment on 10/16/2023.
Employee experienced chest pain and was taken to the ER were it was determined EE had a heart attack.
Pit Millwright fixing a leak on the 16' sand line that feeds the South Plant. EE was installing a hugger clamp using an impact wrench with an extension to tighten down the bolts. The extension broke and the impact wrench hit the palm of the employee's hand cutting finger.
Employee was grabbing samples at BC5340 and went to open lid but it was locked. As the employee tried to pull the lid open and they felt a pain in their right hand up to their right shoulder. Did not become a lost time reportable until 01/17/2024.

Employees hearing test on 11/17/22 resulted in a reportable hearing loss. Received results 12/21/22. Clinic was not taking appointments until after the first of the year. EE's last day on site before retiring was 12/30/2022. We scheduled EE to be retested twice and EE did not show up for the appointments. Now that EE is retired, we will not be able to get EE retested.
Removing a gearbox assembly when they felt a pop in left shoulder. After PT and cortisone shots, medical decided they needed surgery.
Loadout Operator slipped while on top of a railcar. Operator had on fall protection but did hit wrist on the grating. Resulting in a fractured wrist and a torn bicep.
Changing a conveyor belt, winch slipped and cut mechanic's right forearm.
Employee was outside on a very windy day and got debris in right eye which they could not get rinsed out with saline. This caused a scratch across the top of pupil.
Slipped on ice and landed on left side hurting EE's shoulder, elbow and wrist. After further evaluation and an MRI the employee is scheduled for surgery on EE's shoulder.
A miner's audiogram indicated that they had received a greater than 25 decibel standard threshold shift hearing loss.

Getting out of an Excavator the miners foot slipped due to a missing pad on the track. Employee used right hand to grab the cab and keep from falling, in doing so they hurt their right shoulder.
Miner was loading drisco pipe in the back of a pickup truck. The pipe slid and the miner reacted to grab it. In doing so EE twisted right knee.
Been having tremors and numbness in EE's wrists for several years. Finally decided to have them looked at, which resulted in having Carpal Tunnel surgery on EE's right wrist on 11/22/2022

Changing a bearing using a come along. The bearing jerked, which in turn jerked the arm of the employee. Causing pain to EE's right shoulder.
Replacing the dog bones on VSC 5040. 2 person job. One was holding the bolt with a crescent wrench while the other was using a ratchet to tighten the bolt down. The wrench caught and pinched the Millwrights finger between the wrench handle and the framework, cutting EE's right index finger. Took employee to Urgent Care Clinic where EE received 10 stitches.
While stepping out of the cab of a Tele Handler an employee rolled their ankle, dislocating the ankle.
Employee was changing a bucket tooth. The employee struck the tooth with a hammer and a piece of the tooth come off and struck them in the lower chest.
While assisting with a pushout in our Rail Yard, the employee opened the knuckle to the railcar they were connecting to, the front of the knuckle fell off and struck EE's left foot. The knuckle was missing the connecting pin. EE reported the incident to EE's supervisor who took EE to the ER, they did an x-ray and it showed EE fractured EE's fourth toe.

Employee stepped out of man basket at it's lowest position after completing a decel screen cleaning task. The employee's left knee ""popped"" as they stepped out of the basket. After a period time during shift the employee felt pain and discomfort. The employee was taken to the local hospital, given ten days off and prescribed a knee brace.
Employee was patching a leaking hose from an extendable ladder which had slipped out from under the employee on a grated steel floor. Employee fell at a height of four feet and struck their head on the flooring. The employee was taken to the hospital ER and received six staples to a laceration on the back of head.

Employee injured hand closing a pump house door. Employee immediately returned to work after seeking medical exam. Upon an Orthopedic follow up employee was given a lifting restriction and provided a hand brace by doctor.
Employee was injured in a fall on a refueling platform on 10/10/2019. A recent medical exam determined the employee will need shoulder surgery and be off work until completely healed. Official verification that the surgery needed to mend the employee's injury was verified on 06/15/2020.
Employee was manually removing a steel grate placed over a roadway drainage trough and strained lower back muscles. Employee was taken to the local hospital, released and was given a follow up appointment for 10/03/19.
Employee was removing a grating that had fallen in a drainage trough. After task completion, employee felt discomfort in abdominal region. Discomfort was reported on 10/2/19. Discomfort became worse on 10/30/19. Employee was taken to the hospital on 10/30/19 and diagnosed with a potential hernia.
The employee was lifting a rail car lid when employee felt a strain in employee's back. Two days later employee sought medical treatment including prescription medication
The employee was lifting a car lid when EE felt a slight pull in EE's back. EE filled out initial incident paperwork, but did not seek treatment until 6/20. This incident turned into lost time on 6/21.

Employee went to the Doctor on November 30 and was put on restricted duty on December 3. Employee was walking downhill when EE slipped, picked up speed and stopped hyperextending EE's right knee. Employee did not go to the doctor immediately.
Employee reported incident to supervisor on 11/20. Employee tripped and hit right forearm against air compressor resulting in a torn bicep. Became lost time 11/21, employee worked until 11/21.
This injury was a first aid until employee went to the doctor on June 5, 2018. The employee was exiting the cab of the dozer when they twisted their arm and lost footing. This resulted in a strain to the right bicep. The employee missed two days of work.
Employee has a personal medical condition which caused EE to feel light headed and fall while walking up BC2024 catwalk. Employee fell and struck the back of head on the catwalk resulting in stitches to close a cut on the back of head.

Employee was climbing down ladder in dryer room by BC-06 when the upright hand hold broke and ee fell about 6 feet. EE left wrist was sprained.
Pipe was plugged, employee took bolts out of flange and moved one pipe up so employee could look into the pipe to see where the blockage was. As employee held the flashlight in the pipe, the pipe settled pinching and breaking employee's pinkie finger, left hand. The finger is broken.
Employee was walking from Wedron #3 product elevators towards the maintenance vehicle when employee slipped in area covered with water and algae. Employee fell striking the back of employee's head resulting in a cut requiring 5 stitches to close.
Employee was picking up or removing 8 inch Tee from shipping container when the Tee shifted cutting the employee's left thumb requiring stitches to close the cut.
A bearing was being tapped into place on a head pulley shaft using a hammer and a chisel when a small portion of the chisel cutting edge chipped off striking an employee 6 feet away in the right chest. The piece was removed requiring two stitches to close the wound.
Employee was removing the dust collector assess door when it fell striking ring finger cutting it which required stitches.
Dyno Nobel crew was feeding loading hose through outbound hose sheave assembly. EE was standing on truck using 3 points of contact. EE's right hand was on hose boom carousel. Hose boom moved pinching EE's middle finger right hand between hose boom guide wheel and hose boom carousel. EE received 7 stitches.
This incident was initially reported as first aid. It became reportable on 3/20/2017 when physical therapy was prescribed. Employee was walking to retrieve a sample when they stepped over a pile of sand. Employee's foot sank in the sand causing them to slip and twist their back, this event aggravated a pre-existing injury.

Employee was reinserting fuses when a short occurred resulting in second degree burns to right arm.
Employee was performing ground testing at Wedron 2 when EE called supervisor over to enquire the status of a piece of equipment. EE shifted towards the supervisor and slipped falling against the handrail bruising rib cage. When employee had a follow-up Doctor appointment on October 25, 2016 the Doctor reported a fractured rib making this a reportable incident.
Walking down stairs and neck started hurting. Went to doctor on 1/26/16 and was told to ice neck. EE decided to take the rest of the day off using a vacation day so ee could ice neck.
While walking to Bobcat ATV the employee slipped on a small patch of ice and fell to the ground. The resulting injury was a minor sprain to the associate's left wrist. Light restrictions were prescribed as a result.

While stepping off of a ladder the employees knee gave out and she fell from the second rung. Her knee gave out due to a previous injury that was not work related. She received minor restrictions.

While cutting up cardboard boxes to place in the recycling bin the associate cut his left hand with a utility knife. The resulting injury required three stitches to close the wound.
Employee was cleaning out dewatering building using a 2"" diameter water hose which involved lifting and maneuvering the hose. The employee stated that he bent over and twisted to move the hose when he felt a strain in his back. Not reported to plant management until 12-29-14. Unreported Muscle relaxer injection received by doctor (Notified Plant Mg. 1-05-15)
Employee reaggravated a pre-existing injury by working on a Stand Monitor water cannon. Employee experienced pain in his hand/arm due to the vibration in the water cannon handle.
Employee suffered burn on his wrist just above his glove while he was switching out screens.
The employee is claiming back pain from opening rail car lid hatches and recently had surgery. The incident was never initially reported to plant personnel as being work related and was being paid for by the employee's personal insurance. The incident is currently under investigation.
The associate was attempting to dislodge icicles outside the Cone Building when he fell on the ice resulting in a concussion and bruised back. As a result he was prescribed several different pain relievers by the attending physician at the local emergency room.

While cutting banding off of a pallet of bags with a utility knife the employee's hand slipped causing a cut requiring three stitches on his left hand fore finger.
According to associate, he injured his foot over a year ago and recently learned that it was fractured. The associate claims he injured his foot by kicking railroad car lids to open and close them.
While installing screens on a Rotex, the associate dropped a bolt between the Rotex and the floor. The associate attempted to retrieve the bolt by moving the Rotex manually by hand while quickly grabbing the bolt. The associate's hand was caught between the Rotex and floor as a result. The resulting injury was a swollen hand and restricted duty.

While changing a gate in the Wet Surge Tunnel the associate hit his tooth on a metal drip pan that slid out from the damaged gate. The resulting injury is a broken tooth.
The associate was using a pipe wrench to assemble a 2.5"" water line for the plant water truck. When he applied pressure to the wrench it slipped off the threads causing his fingers to strike the back of the truck while still holding the wrench resulting in a fracture to his finger and lacerations.

Worker reported pain in groin area after opening stuck rail car door. He reported incident to management via phone 8 hours after shift ended rather than while on job. He saw doctor on 10th & 11th. Doctor on 11th put him on several work restrictions until later consultation with surgeon for possible hernia. Management investigating incident.

While assisting someone remove a guard, the guard slipped from the worker's hand. A rough inside edge of guard struck his lower left forearm & caused a laceration. The plant manager took the worker to ER for evaluation. The laceration was cleaned & sutured closed rather than using the medical glue.
Two employees were clearing dry vegetation on a steep slope. One employee was cutting brush at the top of the slope with a saw attachment on a weed eater while the other was dragging the cut brush to a staging area. The saw blade kicked through a section of brush striking the second employee in the upper left front side torso resulting in a laceration.
Worker was changing bearing when guard that was leaning on handrail was blown by wind onto worker's back. He reported the incident & was fine (little pain) until 5/5/10 when great pain was reported. He was sent for medical evaluation. Doctor wanted 2-weeks light duty for worker. Worker has reported history of back issues/pain from recreational activities outside of work.
Miner was walking in from parking lot and twisted his ankle when wearing athletic shoes and not boots. He was taken to the clinic and the doctor indicated an ankle sprain. For 1-week, doctor did not want him to climb and take breaks if job had lots of standing/walking. He did not have any jobs that week that would require him to climb or with excessive standing/walking.

An associate lifted 2 3.5 gal sand buckets. 5 minutes later he felt slight groin pain and even more pain in evening at home. He called the health insurance nurse hotline who recommended a medical visit. He visited a clinic, the doctor diagnosed a strained groin & prescribed muscel relaxant and pain relief medication.
Worker had pain in the lower left back (left side) when lifting bag of sand. He did not report until next day (9/18) but indicated it was not bad. He said pain was much worse 11 days (9/29) after first report & was taken to clinic & released to full duty. 2 days later (10/1), he reported more pain & returned to clinic and was told to not return to work until later.
Associate was changing screens. The assoicate used a grinder to cut off a bolt and a piece of steel landed in the eye. The associate was wearing safety glasses.
The injured employee was draining the sand and water lines in the pit preparing for a pit shutdown. After he opened the drain valve he slipped and fell and injured his wrist.

Th associate was standing on a 2' sand bluff when the sand bluff gave way resulting in a strain to the right knee. The associate reported the incident on November 1, 2008 as a First Aid Incident with no medical required. On February 26, 2009 the associate first notified Fairmount Minerals that the case needed to be re-opened as a medical incident requiring surgery.
The injured employee was cleaning his work area around the forced air packer and strained his back while moving a 100 lb bag of sand. The incident was first reported on 10/16 and at that time the incident was not a recordable injury. The employee left work on 12/09/08 and we were informed on 12/12/08 that the employee was to be off work.
The injured employee lifted a motor off of the work bench, tripped and fell backwards, resulting in the injury.
Working on a dryer, he stepped outside to get additional tools and was stung on top of right ear by bee.
An 800 amp fused disconnect was turned off and locked out with only power to the line side. The door was opened to prep tools for a job. Employee set the tools on the top of the box when one of the tools rolled off the box. As he tried to grab the tool it fell into the top side of the switch and shorted out. All arch shields were in place.
The injured employee was removing a guard from a face pump. The employee was working with another EE to lift the guard over the pump/motor sheaves.
The employee was removing snow with the front end loader. The corner of the bucket caught a pot hole and jerked the cab of the loader to the right. Operator hit his head on the side window of the cab and required 8 stitches to close the cut. Operator was wearing his seatbelt at the time of the incident.

INDIVIDUAL WAS PLACING ROLL OF STRETCH WRAP ON MACHINE WHEN ROLL SLIPPED PINNING FINGER BETWEEN PLASTIC ROLL AND METAL SPINDLE. FINGER WAS CUT AT NAIL REQUIRING TWO STITCHES.
A PRESSURE REGULATOR VALVE FAILED, ALLOWING 35 PSI NATURAL GAS TO GET TO THE BURNER, RESULTING IN AN EXPLOSION WHEN THE DRYER WAS SHUT DOWN.

The injured ee was opening a rail car door. He pulled on a large steel bar he was using & felt a pain in the groin area. He went home. Went to ER & discovered a heart flutter. Then they discovered 2 pre-existing hernias. They also noticed a twisted testicle. Surgery was performed to correct everything. Notified on 11/6/06 of surgery on the testicle & is charged as a LTA.
The individual was standing at a monitor (water canon)-the pumps were boosting the water pressure on a high pressure pump & the monitor brok off at base. Piece of iron striking leg & foot. Threw the individual backwards about 8' and result was broken tibia in left leg, bruised right foot & leg.
The injured was splicing a conveyor belt. He was using angle to secure the belt for splicing. He turned to reach for a wrench on catwalk and hit his head on angle, cutting a slice in his forehead 5 cm long. It required 7 stitches.

The injured ee was working on a cooler. He was taking off the guide rings for the timing belt sheave. He put a screw driver in behind the ring. The screw driver sliped and cut finger on the guide ring. He recieved stitches to his left hand-ring finger.
EE WAS WORKING ON A CONVEYOR IN THE QUARRY. HE WAS PULLING HIMSELF UP THE CONVEYOR WHEN HE FELT A PULL IN LOWER BACK. COMPANY WAS NOTIFIED ON JULY 8 THAT HE RECEIVED MEDICAL TREATMENT- CHIROPRACTOR CARE.
Raising steel plates using hoist. Deck strap on plates broke causing plates to upend. Edge of plate caught employees arm causing laceration.

THIS ACCIDENT OCCURRED ON 4-15-03. WAS NOTIFIED ON 4-21-03 THAT THIS ACCIDENT OCCURRED. THE INDIVIDUAL BELIEVES IT HAPPENED WHILE BRUSHING/SCRUBING TESTING SCREENS VIGOROUSLY. SHE EXPERIENCED SEVERE CRAMPING WHILE PERFORMING THIS TASK. SHE DID SOMETHING TO HER MUSCLE OR NERVE IN HER LEFT UPPER BACK. SHE IS ON RESTRICTED DUTY.
IT STARTED SNOWING EARLY THIS DAY & CONTINUED UP UNTIL THE ACCIDENT. SNOW REMOVAL HAD BEEN UNDERWAY ON WALKWAYS. THE INJURED WAS WALKING ON A ROADWAY WHEN HE SLIPPED & FELL ON HIS BACK & HIT H IS HEAD ON THE GROUND. HE MISSED WORK FOR 2 DAYS & RETURNED TO REGULAR DUTY.

THE INJURED WAS PICKING UP A TRAY OF SAND SAMPLES. THE TRAY WEIGHED APPROX. 28-30 LBX. THE TRAY OF SAMPLES WAS ON A COUNTER TOP 34 1/2"" FROM THE FLOOR. WHEN SHE PICKED UP THE TRAY SHE FELT HER BACK GO WEAKER AND HAD A BACK ACHE-SHE MISSED STWO DAYS OF WORK AND IS CURRENTLY ON RESTRICTED DUTY.
THE INJURED WAS LOADING RAILCARS. HE WAS LIFTING ALID (TRYING TO CLOSE IT) & PULLED SOMETHING IN HIS RIGHT LEG. THE INDIVIDUAL KNEELS DOWN ON ONE KNEE & LIFTS THE LID. THE INDIVIDUAL HAS A TOR N HAMSTRING & WILL BE ON RESTRICTE DUTY. THIS IS NOT A LOST TIME ACCIDENT.

IT WAS AT THE END OF A PRODUCTION DAY. THE INJURED WENT TO THE DEISTER SCREEN AND WASHED THE SCREEN DECK CLEAN. HE THEN STEPPED DOWN AND CAUGHT HIS RIGHT REAR END ON A PIECE OF STEEL. HE RECEI VED A CUT THAT REQUIRED 2 STICHES.

PUSHING A MARTIN MOTOR. ACCIDENT REPORT FILED ON 7/25/01. REFUSED MEDICAL TREATMENT & REFUSED TO SEE DOCTOR. ON 12/11/01 SAW DOCTOR WHO PUT HIM OFF WORK. THIS WAS FIRST NOTICE TO DEDRON OF ANY PROBLEM. EMPLOYEE CLAIMS ACCIDENT FROM 7/19/01 WAS THE CAUSE.
THE INJURED WAS ON A BAGGING PLACING BAGS ON SPOUTS. HE STARTED A CONVEYOR CHAIN AND IT BECAME STUCK. HE SHUT THE POWER OFF AND USED A CROW BAR TO BACK THE CONVEYOR UP THE BAR SLIPPED. HIS HAN D FELL INTO A SHARP PBJECT AND CUT INTO THE TOP SIDE BEHIND THE LITTLE FINGER. HE RECEIVED 3 STITCHES AND WAS ON RESTRICTED DUTY.
EE WAS REMOVING ONE OF THE CUTTING EDGES FROM THE BUCKET OF THE VOLVO L330 LOADER. AS THE EE CUT THE NUTON THE NEXT TO LAST BOLT, THE PLATER (CUTTING EDGE) FELL ON HIS FOOT. HE RECEIVED A FRAC TURE & IS OFF WORK.
THE INJURED WAS POUNDING ON A RAIL CAR DOOR WITHA STEEL BAR TRYING TO CLOSE IT. HE WAS ALSO SHOVELING SAND. HE COMPLAINED OF BACK PROBLEMS. 2/6 HE CAME TO WORK WITH RESTRICTED DUTY SLIP.

ON 9/10/98 HE CLAIMED CARPAL TUNNEL. ON 12/14/00 THE INDIVIDUAL WAS OFF WORK AND HAD SURGERY TO HIS WRIST. HE RETURNED TO WORK ON RESTRICTED DUTY 12/28/00. HE WAS REMOVED FROM RESTRICTED DUTY ON 1/12/01. THIS INDIVIDUAL USES ALL TYPES OF TOOLS AND EQUIPMENT ON MAINTENANCE.
THE INDIVIDUAL INJURED HIMSELF WHILE PULLING ANDCARRYING A WATER HOSE UP SOME STAIRS. AT FIRST HE DIDN'T THINK MUCH ABOUT THE INJURY. IN DEC. HIS ACHILLES HEEL ON THE LEFT FOOT STARTED HURTING FROM THE ACCIDENT (HE SAID). FROM 12/19 TO 1/12 HE WAS OFF WORK. HE CAME BACK TO WORK & IS ON PHYSICAL THERAPY IN JANUARY BUT NOT ON RESTRICTED DUTY.
INSTALLING A BEARING ON A SNUBBER PULLEY. THE BOLT USED FOR RIGGING WAS NOT WELDED PROPERLY. THE BOLT THAT BROKE OFF & HIT THE INJURED IN THE LIP & THE SNUBBER PULLEY LANDED ON HIS HAND. THE I NJURED RECEIVED STITCHES IN HIS LIP.
THIS EE HAD SURGERY FOR CARPAL TUNNEL. HE CAME BACK TO WORK ON 7-17 ON RESTRICTED DUTY. THIS INDIVIDUAL LOADS SAND INTO RR CARS & SEMIS. HE DOES THIS IN FRONT OF A CONTROL PANEL. HE USES SCREE N SIEVES & PUSHES BUTTONS. HE DOES PAPER WORK & USES A COMPUTER.
