EE was walking low to avoid the pipes in the belt line. EE hit a fire valve and it pushed EE's hat into EE's nose. EE went to the doctor on EE's own with out informing GMM. after getting the loss run report today I realized that EE had fractured EE's nose.

Showing all 8While trying to position a plate for a door, the plate slid off the ledge and came down on left foot and bruised EE's foot.
EE was lifting a top cluster off of the rails and the bottom slid off one of the rails and came down and smashed EE's pinky toe on EE's right foot.
The roof bolter drill steel was stuck in the roof. EE was pulling on the steel with EE's hands and when the steel came apart the momentum of the steel caused it to come back and hit self in the mouth. EE's mouth was sore and EE went to the dentist on 11/1 and EE went to dentist again on 11/16. EE quit and we just found out they removed 2 teeth.
On 10/14/2022 EE was pulling cable off of the reel of the roof bolter and felt a sharp pain and a burning in their groin at the end of the shift. EE had a lump in their groin. EE continued to work until 10/22/2022 then EE decided needed to go see a doctor.
Employee was dragging HV cable backwards and tripped over HV nip, HV cable fell on their knee. Employee was originally released with no restrictions. they went to the doctor again and was put on restricted duty on 9/22/22.
EE was taking down a Kennedy stopping, when the panel slid down it cut through EE's glove and cut EE's pinky finger and had to get 5 stitches.
EE was working on a pump in the intake roadway, bent over to pick up a slick line. EE felt a pain in lower back.
