Drowning

Mantrip was passing the loadout and they observed a wall of water and debris coming towards them. They began backing up and hit the berm. EE was the first to exit the mantrip. They were on high ground with water flowing on both sides of them when EE stepped into the stream on the east side of them when EE was swept away. EE's body was found 6 miles downstream.

Employee was cutting a cable with a razor knife and cut 3 of their fingers.

Over-exertion in pulling or pushing objects

EE was shoveling belts on the surface. EE's shovel hit a chunk of ice. At that time EE heard a snap and EE's shoulder gave out.

EE was walking in mud about 15"" deep and lost EE's balance. EE's left foot was stuck in the mud and twisted EE's knee while trying to catch self from falling. EE continued to work and on 3-12-25 it was getting worse and we took EE to get it checked.

Struck against stationary object

The continuous miner was parked next to a rib and EE was checking something on the machine while the belts were down. The rib made a little noise and came down. EE didn't have time to move and the rib pushed EE's knee into the miner bruising and cutting the knee.

Contact with cold

Employee was shoveling belts on the surface in the cold weather. 2 fingers on their right hand started hurting. it was possibly a slight condition of frost bite.

EE was walking low to avoid the pipes in the belt line. EE hit a fire valve and it pushed EE's hat into EE's nose. EE went to the doctor on EE's own with out informing GMM. after getting the loss run report today I realized that EE had fractured EE's nose.

EE was driving in the 4L intake, drove off of a lip and the inner liner of the cab broke loose and landed on EE's head jamming neck and back

While roof bolting the operator was installing the second steel to continue drilling the hole and they pinched a finger between the steels.

Loading stock on the bolter. Slipped on a piece of brattice. Slipped backwards and felt a pinch in lower back.

EE was behind the line curtain, putting chunks of coal on the brattice. A rib rolled off and struck EE on the left side, fracturing some ribs and spraining EE's right ankle.

While trying to position a plate for a door, the plate slid off the ledge and came down on left foot and bruised EE's foot.

Roof bolting, putting pusher and starter steels together, didn't get connected. put pressure on drill steels, let pressure off and steels came apart and one of the drill steels sprung back and hit EE's hand, cutting it open.

Struck by falling object

EE was lifting a top cluster off of the rails and the bottom slid off one of the rails and came down and smashed EE's pinky toe on EE's right foot.

The roof bolter drill steel was stuck in the roof. EE was pulling on the steel with EE's hands and when the steel came apart the momentum of the steel caused it to come back and hit self in the mouth. EE's mouth was sore and EE went to the dentist on 11/1 and EE went to dentist again on 11/16. EE quit and we just found out they removed 2 teeth.

On 10/14/2022 EE was pulling cable off of the reel of the roof bolter and felt a sharp pain and a burning in their groin at the end of the shift. EE had a lump in their groin. EE continued to work until 10/22/2022 then EE decided needed to go see a doctor.

Employee was dragging HV cable backwards and tripped over HV nip, HV cable fell on their knee. Employee was originally released with no restrictions. they went to the doctor again and was put on restricted duty on 9/22/22.

tightening bolts on the continuous miner with an allen wrench. The wrench slipped striking and cutting finger.

slipped in a mudhole, tried to twist and jump out, EE's foot was stuck and EE twisted to jump out and EE's knee popped. EE worked at EE's normal duty's and went to doctor again on 7/18. Doctor ordered an MRI. EE is off work until results come back.

Accident type, without injuries

The end of the shift in the 1L retreat section the mining crew just finished a right hand cut in #6 entry between 4 and 5 crosscuts. When attempting to advance the MRS's the MRS on the left was stuck. The CM was attached to the MRS with a wire rope. When attempting to pull the MRS the roof caved trapping the 3 machines. MSHA was notified and a K order was issued.

EE was taking down a Kennedy stopping, when the panel slid down it cut through EE's glove and cut EE's pinky finger and had to get 5 stitches.

Employee was pre-shifting 1N belt and caught hard hat on the mesh and twisted left knee. Employee heard and felt it pop.

EE got out of the scoop and couldn't put any weight on knee, EE doesn't know what EE did to hurt the knee. It didn't pop or twist, just hurt. EE said EE did have problems with the knee in the past. EE worked 2 more days then went to the doctor the next day. There was some fluid build up in the knee.

EE was working on a pump in the intake roadway, bent over to pick up a slick line. EE felt a pain in lower back.

Crew was driving up the mine road when they were met by a wall of water and debris which overtook the mantrip, rolling it 4 times before coming to rest in a creek bed at which time EE kicked out the the back window for the crew to escape.

EE was driving down the mine road when a wall of water and debris overtook EE's truck. EE was able to exit the vehicle and was taken downstream about 1/4 mile where EE was found.

Accident type, without injuries

A severe rainstorm resulted in a flash flood out of the canyon to the northeast of the portal, which resulted in water in the intake entry. The return entry was not affected by the water. There was no one in the mine at the time of the incident. *The miners traveling the road were involved in the flood.

Struck against a moving object

The mantrip was in a flash flood, water and debris. The mantrip rolled several times. As a result EE injured left shoulder pulling a muscle behind the shoulder blade. EE continued working until EE went and seen a doctor on 8/10/2021.

EE was setting a roof bolt and the chuck adapter snapped sending wrench into EE's right wrist.

Had chest pains the night before at home(4/4/21). An hour after shift started on 4/5 started having chest pains again. Taken to hospital by ambulance. Sent home on 4/6/2021. Worked full shifts on 4/7, 4/8. Did not work on 4/9. Went to hospital on 4/9 where the determined employee was having a heart attack. Life flighted to Hospital where they performed surgery.

Moving MRS back and the plow flipped over a chunk of coal onto EE's right foot.

lifting a bolter drill box out of the jeep with the 4"" x 1/2"" bolts threaded into the sides of the drill box as handles, and while moving toward the shelf it slipped out of EE's left hand and sprained EE's right wrist.

Employee was on an A-frame ladder changing a fuel filter on a loader. As employee dismounted the ladder the employee missed the last rung on the ladder and rolled foot. Fractured a bone in right foot.

Making a splice on shuttle car cable, employee was pulling the outer jacket off the cable, felt a pop in right shoulder. Went to the doctor on 5/19/2020. The doctor ordered an MRI. Off work waiting for results of MRI.

Employee was puling miner cable back, when EE slipped and fell back straining the right shoulder. Accident was on 2-5-2020, EE continued working with no restriction until 2-24-2020.

Stacking continuous miner cable. When EE bent over and picked up cable EE twisted EE's back.

Pulling belt on tail piece. Tripped and fell, hitting chin on tail.

Removing a mettle stopping. When the panel collapsed the edge of the panel cut employee's right pinky finger.

Fall to the walkway or working surface

Was removing a rock from the top of the belt, using a pry bar. The pry bar slipped and ee fell, landing on right arm.

Struck by flying object

Miner operator was cutting coal in the 1st North section, in the No. 1 entry. A small piece of coal blew off the rib and struck EE on the right ear, causing a small laceration that required 2 stiches to close it.

Unloading miner cable off head of miner, swung cable to the rib and felt pain in right shoulder. Continued working at regular duties until 11/13/2017 when ee was examined by a physician.

DRILLING A HOLE FOR A ROOF BOLT. THE DRILL STEEL CAME OUT OF THE CHUCK AND HIT RIGHT HAND CAUSING A LACERATION.

Was cutting down a travel pad, reaching above head. Felt a sharp pain in right shoulder. Incident occurred on 8/9/2016, but did not seek medical attention until 8/17/2016 and was put on restricted duty.

Unloading top decking for Kennedy over cast from supply trailer, one end dropped off trailer, the other end tilted upwards and cut EE's forehead.

Employee went through a man door on a metal stopping, when he let go of the door his hand hit the latch on the door and cut his hand. Required 6 stitches.

Upon entering the secondary escapeway (return entry) in the 6th right section, smoke and carbon monoxide were encountered. It was determined that a thermal event had occurred in the #2 panel gob area.

He was changing battery on a tractor and slipped off and bumped his tailbone.

Car was leaving the miner, car driver thought he was past both the miner op. and his helper but the tailgate of the car caught his left leg and smashed it against the rib.
