Employee was relocating a hose into position in the Mill area, and reaggravated an existing back injury. The employee further aggravated their back when attempting to loosen a gearbox nut using a crescent wrench.

A loadout scale house operator smashed and caught left-hand ring finger as they were entering control room office. EE was taken to ER and followed up with an occupational doctor where they sustained a minor fracture at the tip of left-hand ring finger. The EE was not prescribed medication, nor was EE put on any restrictions and returned to work the same day.

The employee was removing a piece of concrete block that entered the feed hopper and got stuck under the angle iron on the conveyor guard. They tried pulling the block under the guard and heard a pop in right biceps with a sharp pain up into their neck.

EMP was re-applying Machine guard after a repair. While grasping the handle of the guard to reposition the guard to attach the bolts back on, the guard slipped from the bottom, causing the handle to come down on the employee's left middle finger & and landed on a supporting beam, trapping, pinching, & cutting the Left Middle Finger. Laceration.

Employee was at a dust drain , rinsing off with water, excess material at a tail pulley. Employee pulled hose (1 1/2 in dia. hose 30 to 40 ft. long) with only Left hand/arm and felt a pull in their left shoulder.

The EE reported on 9/22 that they were removing a tire from a light duty pick up and it fell backward against the hub after they took it off the studs and pinched their two middle fingers on their left hand between the metal wheel and hub. They did not want to seek medical attention until Monday, 9/25.

Employee was cleaning out a screen inlet chute from clogged material that resulted in a laceration to the right ring finger. When the EE removed their hand, EE made contact with the top of their right ring finger and a sharp metal edge of the chute inner lining. The EE received six stitches and was allowed to return to work with restrictions.

The EE reported lower back strain from pushing plates at the filter press on 6/8/23 but did not want to seek medical attention. They reported on 6/14/2023 that they did want to go to the doctor since their lower back was still sore. The doctor gave them prescription medication and PT.

The EE was washing mud from the filters at the filter press with another EE and got mud in their eyes. They used the eyewash but rubbed their left eye with their hand to try to get the mud out and it scratched their cornea.

Employee was installing Doctor Blades on the belt press. While pushing in the lever and pin, employee felt a pinch in left shoulder/back area. Employee is scheduled to be on light duty for 2 weeks. No lost time.

EE reported on 9/16/22 but did not want medical treatment. On 10/4/22 EE stated EE did want to see Dr. for strain to Rt. shoulder from pulling silo slide gate pipe handle on bin number three. Slide gate was stuck closed, and the EE overexerted to open the gate. On 10/7/2022, Dr. did not prescribe medication and no restrictions but referred EE to PT.

The employee reported on 8/5/22 that they were working at the palletizer with 100 lb bags and six bags stacked on the belt. They said they pulled a bag toward them across the air table more with right arm than left and strained upper right arm. Employee was diagnosed with triceps strain which is a pushing muscle.

Bagging Operator was helping a forklift operator align the lifting eyes on a super sack tote in the storage warehouse. The Employee's right ring finger was caught in between the forks and the building c-purlin resulting in a laceration requiring stitches.
