Employee was holding the liner for the hopper in place and slipped, causing their left palm to make contact with the drill bit that was coming through the hopper and liner, creating bolt holes.

The employee was removing a piece of concrete block that entered the feed hopper and got stuck under the angle iron on the conveyor guard. They tried pulling the block under the guard and heard a pop in right biceps with a sharp pain up into their neck.

The employee sought medical treatment for feeling ill and was misdiagnosed, received treatment, and released from care. The employee returned to work for two days and felt unwell again, returning to the ER on 3/12/25. Further testing confirmed a different illness on 3/16/25.

Employee was stacking empty paper bags on a flat working surface when EE rolled EE's right ankle as EE was making a turn to walk.

Employee was using a grinder to do a repair at our Wash Plant. While grinding a object got in EE's left eye. EE treated self using eye wash. The eye continued to bother EE the next day so EE reported the incident and was seen at a MedCenter clinic. EE was given eye drops as a precaution against infection. EE returned to full duties with no restrictions.

Employee drove to baghouse and stepped on a pallet as EE was exiting the vehicle and rolled their left ankle.

EMP was re-applying Machine guard after a repair. While grasping the handle of the guard to reposition the guard to attach the bolts back on, the guard slipped from the bottom, causing the handle to come down on the employee's left middle finger & and landed on a supporting beam, trapping, pinching, & cutting the Left Middle Finger. Laceration.

Employee was at a dust drain , rinsing off with water, excess material at a tail pulley. Employee pulled hose (1 1/2 in dia. hose 30 to 40 ft. long) with only Left hand/arm and felt a pull in their left shoulder.

Slipped on wet stairs while grabbing the handrailing with rt. hand. Injury to Rt. shoulder. Employee on Light duty. To be revaluated on 1/23/2023

Employee was installing Doctor Blades on the belt press. While pushing in the lever and pin, employee felt a pinch in left shoulder/back area. Employee is scheduled to be on light duty for 2 weeks. No lost time.

Bagging Plant employee overextended their back on Thursday December 1, 2022 while picking up a 100 lb. bag off the reject belt and placing it on a pallet. EE is scheduled to return to work on Monday December 12, 2022 on light duty for 1 week.

Employee was assisting maintenance personal put away tools when the employee stepped over a gutter and onto a curb. The employee placed the ball of their right foot on the curb and took another step with their other leg. The right heel dropped as they place weight on the leg causing a strained calf muscle.

To the best of our knowledge and information an employee who tested positive for COVID-19 may have been in contact with one employee, (vaccinated), for 15 minutes within 6 feet on 2-1-22. EE felt symptoms on 2-5-22 after shift. EE tested positive that evening and is quarantined at home for ten days.

Employee was removing a screen from the 3 division shaker with the help of another individual when employee felt pain in the middle of back. Employee is currently out of work until released by the physician.

Employee was preparing to clean up work area with water hose when EE slipped on muddy water and fell. EE tried catching self by putting down right hand. In doing so, EE sprained right hand and wrist. EE has been placed on restriction/light duty until further notice.

Employee was shoveling sand into a wheelbarrow when employee felt pain in lower back on the right side. Placed on restriction, no lost time.

Employee received contusion on right thigh when struck with reject belt while clearing backed up bags on palletizer machine.

Laceration to right Pinkie finger while trying to slide ball deck of Tycan screen into place. The deck fell on EE's finger after being pushed over a small lip.

Employee was grinding 2" pipe when a metal flake entered left eye. EE immediately notified supervisor and flushed it with eye wash. EE was taken in for medical attention. The piece of metal had to be removed. EE was wearing safety glasses and a straight clear face shield. We will be changing over to a V90 shield with built in goggles to prevent this from re-occurring.

Employee was using a cordless impact wrench with left hand. EE was tightening bolts on a motor when EE started to feel some pain in the left hand.

Employee was walking back to work station from lunch. EE was struck on hard hat by a rock while walking under the 3 division jaw belt. EE felt pain in neck and upper back. EE was taken to med center for evaluation.

On 10/18/2018 the employee was grinding angle iron when EE felt something in EE's right eye. EE continued working and it wasn't until 10/23/2018 when discomfort lead EE to seek medical attention. EE had a small flake of metal in EE's right eye that had to be removed. EE was wearing safety glasses at the time of the incident.

Employee had just parked the CAT 980C loader. As ee was exiting the loader, ee accidentally stepped on a wheel chock that ee had not noticed when parking. The chock was located at the bottom of the loader's step ladder. Stepping on the chock caused leg to twist, resulting in injuring left leg.

Employee was operating a CAT 972M Rubber Tire Loader when EE felt pain in back. EE then reported this to immediate supervisor, who took EE to the Med Center to be evaluated. There was not a specific activity that triggered the back pain. EE was released back to work with restrictions. The employee has been a Loader Operator at the facility for 28 years.

Employee was conducting scheduled cleaning of Plant. EE was using a general purpose hammer to clear dried material off the conveyor frame. As EE hit the frame, the hammer ricocheted off striking EE's opposite (left) pinky finger. EE received a laceration and was taken to the clinic. EE did not receive sutures however, EE was prescribed an antibiotics.

Employee was stripping wood frame around A/C unit and bending nails over as they were pulled. During clean up, EE stepped on a nail that was apparently missed. The nail had penetrated the work boot and punctured the bottom of foot. Foot was cleaned, received a tetanus shot, and was given precautionary prescription. No lost time.

Employee was grinding in the Welders Yard when ee got debris in left eye. EE immediately went over and flushed it out at the eye wash station. EE was wearing Safety Glasses and Face Shield at the time. EE started to feel some irritation approximately 8 working days later when ee notified employer on 5/16/2017. No work missed. First Aid with Prescription eye drops.

Employee was exiting vehicle when a large gust of wind slammed the door shut. Employee tried to catch the door when two fingers on right hand were pinched in the door as it shut.

While conducting an inspection/cleaning of the loader bucket, the operator had not completely lowered the bucket to the ground. The hydraulics slowly lowered while parked, causing the bucket to pinch the operators toes on left foot between the bucket and the ground.

While dismounting a skid steer, the miner caught his foot on a step and fell approximately two feet to the ground and injured his right knee. First aid was administered and an x-ray showed no fractures. There was no lost time and injured returned to work on restricted duty the same day. Doctor gave prescription for ibuprofen. Knee showed minor bruising and swelling.

While walking on a wet floor at the Slurry Plant, the miner slipped and fell, injury his right knee that had been previously injured at home. The miner finished his shift. The right knee swelled overnight and the miner was seen by a physician the following day, May 27, 2015. The miner was asked to follow-up with the doctor on June 1, with another follow-up scheduled for June 8.

Suffered minor arc flash burns to both eyes while welding inside a metal bucket attached to a front end loader. The injured said he should not have raised his welding helmet while another welder was working in the same area of a shiny reflective surface. The burns were not realized until the following morning. The other welder did not suffer any arc flash burns to his eyes.

Sprained left shoulder while lifting a gas cylinder of oxygen up into a storage box on a mechanic's service truck. The sprain was minor pain at first and the employee did not ask for first aid treatment until 04/01/2013.

Employee injured his left leg calf while walking up an inclined catwalk adjacent to the dry sand belt at the Rock Plant. As he was walking, he felt a "pop" in his left leg. He reported pain 10 minutes later and was taken to Simi Health Center Urgent Care and received first aid treatment.

The employee fainted and fell to the ground while walking from his lunch break area to his work area. He landed on his face and sustained abrasions to his face, knees, and elbows. His tongue was also lacerated and required 4 stitches. 9-1-1 was called and he was treated and released at the ER.

A 65-year old man was found dead in his automobile in a remote area of the mine site. It appears the deceased had strayed onto the mine site after he left his job on an adjacent property.

Heavy lifting may have caused a hernia while performing maintenance tasks at the Rock Plant. The employee did not report the injury to his supervisor until 8/20/12, at which time he was taken immediately to Simi Health Center, where he is currently being referred to a surgeon for treatment. Until then, the employee is on restricted duty with no lifting over 15 lb.

While working in the main mechanic shop, the employee tripped over an air hose as he walked around a service truck. The employee fell to the concrete floor and struck his chin on the pavement.

While grinding metal, the operator noticed some irritation in his right eye around 4PM. He went home end of shift at 5PM and applied eye drops. Did not help. Employee went to ER early morning of 3/8/11.

The operator was cleaning the area around the short head cone crusher at the rock plant when a piece of channel iron weighing about 20 lbs slid down the handrail along the catwalk and landed on his left foot. Two days later (Dec.8), he reported swelling around the ankle and was taken to an urgent care facility for medical treatment.

At the Rock Plant, a 3" diameter shaft approximately 24" in length was being used to pound on the hopper wall. The operator placed the shaft up on the tail pulley guard. He turned to talk to another operator when the bar fell on his right foot.

While permorming maintenance work on belt press #2, the employee reached out to place a come-along around a pulley and strained his back.

The employee drove the loader into the shop for maintenance and to take lunch break. As the employee was stepping down from the cab of the loader, he lost his grip and fell to the ground injuring his arm.

They were lifting the bed of an R85 dump truck with the skip loader when it slipped and the bed of the truck came down full force. Employee was sitting in the seat in the cab of the truck. The truck jumped and the bed hit bottom throwing him up and all over.

When employee was going down a ladder into the pit behind a wall at the scalper, he slipped & fell hitting his left knee & forearm.

Employee was grinding and a piece of metal flew into his right eye.

Employee was changing the oil filter on one of the machines and dropped the filter on his small right finger.

Employee removing chains from the back of service truck, He stepped down from the back of truck stepped on a rock and twisted ankel.

The operator climbed up a horizontal conveyor belt to clean it off after it plugged. Due to rainy condition the frame was slick causing him to slip.

DRIVING THE WATER TRUCK, CLIMBING IN AND OUT OF THE TRUCK PUTTING PRESSURE ON HIS TOE, AT THE WATER TANK FACILITY HE WALKED UP THE BANK. HE REACTIVATED AN ULCER ON THE LARGE TOE.
