Employee stated that he was moving a 55 gal drum of oil, the drum slipped from metal surface, smashing his hand betwene the drum and a metal pole.
Employee was carrying a cabinet fan upstairs into a shovel on his shoulder when he felt a pain in his lower back. Diagnosed with a lower back strain.
Employee removed hardhat to get between the frame and engine so he could install starter. He cut his head on a hose clamp which required 5 staples to close.
Employee was carrying an outrigger pad around to set up on the boom truck. While carrying the pad, he stepped in a hole in the ground and felt a sharp pain in the groin area.
AS HE WAS HELPING LIFT A RAM JACK FOR A SCOOP INTO THE SCOOP BUCKET, HE GOT HIS RIGHT LITTLE FINGER CAUGHT BETWEEN THE JACK AND SCOOP BUCKET. IT RESULTED INTO A FRACTURE AND LACERATION TO THE END OF THE FINGER
EMPLOYEE WAS LIFTING BOX FROM FLOOR OF MAGAZINE TO LOAD ONTO TRUCK. HE FELT A POPPING IN HIS BACK BELOW SHOULDER BLADES. SAW ER PHYSICIAN & WAS GIVEN LIGHT DUTY (LIFT NO MORE THAN 10 LBS) UNTIL 9-6-05
He was putting a pin in a rub rail guard on a continuous miner and got his thumb caught between the rail and the frame of the miner.
HE BENT OVER TO PICK UP A ROOF PLATE AND FELT PAIN IN HIS BACK. HE CONTINUED TO WORK & STARTED MISSING WORK ON 01/13/06.
Employee pinched his finger between a sledgehammer handle and a metal bulk prill bin, resulting in a fracture to his left ring finger.
Employee was cleaning weld shop and wearing safety glasses and hardhat. At break, he noticed something in his eye. After trying to remove it himself, he was taken to emergency where they removed a metal particle.
At the time of the incident Employee was repairing the final drive on a CAT 793 off-highway truck. When he bent over to pick up an impact wrench he felt a pop in his back and could not stand up straight.
He felt pain in his shoulder while lifting a 4" water line.
Employee was trying to pick up on a steel beam to work it into position for installation, when he felt a sharp pain in his groin area. Not reported to supervisor until 7/15/05.
Employee was removing a hoist pump from a haul truck. 2 of three bolts had been removed. When the third bolt was loosened the pump rotated downward suddenly and smashed the employee's left middle finger. The finger was broken and lacerated, requiring surgery to repair.
The employee smashed his right thumb while installing a pin on a dragline bucket dumprope. After getting the pin partially through the bores the pin stopped so the employee began to wiggle the pin. The pin freed up and as it dropped it pinched the thumb between the pin keeper and the rope becket end.
EMPLOYEE STATED THAT HE CUT HIS RIGHT HAND WHILE MOVING OIL BARRELS.
At the time of the incident employee was attaching a hoist cylinder to the blade of a CAT D-11 dozer. As the cylinder was being lowered into place, employees' left hand (left ring finger), which he was using to guide the cylinder, became pinched between the cylinder rod end and the hoist cylinder mount.
He was cleaning the scrubber as part of normal maintenance. He placed his hand on the frame of the miner to pull slack to the water hose and the scrubber lid fell on his hand.
EE WAS WAITING FOR FLOOR SWEEPER TO PICK UP PILE OF DIRT EE HAD SWEPT. EE WAS STANDING APPROX 10 FT AWAY WHEN SOMETHING STRUCK EE IN THE EYE
Employee was performing normal work duties when he twisted in an upright position and felt severe pain in his back. He was seen at the hospital and diagnosed with injured discs in the lumbar spine. Prior to the event he had been under doctor's care and receiving treatment for pre-existing back problems.
Employee was lifting an altenator to install in a track drill when he felt pain in his lower back and groin. He was diagnosed with a severe lumbar strain and disc injury.
EE WAS LOSSENING BOLTS ON A BELT FEEDER SPEED REDUCER. AS THE NUT LOSSENED, EE STRAINED RIGHT SHOULDER. EE CONTINUED TO WORK UNTIL EE HAD SURGERY ON 4-26-05 TO REPAIR A TEAR TO RIGHT ROTATOR CUFF
EE DISMOUNTED TRUCK LADDER TO LOWER PLATFORM (LOCATED ON BUMPER). HE UNHOOKED THE BOTTOM "SWING DOWN" STEP. AS EE PUSHED ON THE PLATFORM STEP WITH HIS LEFT HAND, THE STEP LIFT CHAIN PINCHED HIS RIGHT MIDDLE FINGER.
Lifted mop bucket to empty instead of using the lever to dump the water, resulting in back pain.
Employee was removing a crib block from underneath a D10 dozer track after the track had been bolted together. Two of the track pads came together, severing the tip of the employee's right little finger just below the fingernail.

Employee was aligning shims in the crowd take up assembly on top of the shovel bucket. When the shims were aligned, the shim retaining guard fell into place and pinched the employee's left middle finger between the guard and the frame. The finger was lacerated and required sutures.
EE REPORTED BACK PAIN ON 01/04/05 AND CONTINUED TO WORK UNTIL 02/14/05. EE SAID WAS BENT OVER WHEN EE EXPERIENCED PAIN IN BACK. EE WENT TO DR ON 02/14/05. DR TOOK EE OFF WORK PENDING THE RESULTS OF AN MRI. RESULTS WERE RELEASED TODAY SHOWING NO HERNIATION WITH DEGENERATIVE DISK DISEASE (MODERATE)
EE WAS PULLING FUEL HOSE TO TRUCK TANK, FELT PAIN IN LOWER BACK. DID NOT REPORT UNTIL 11/08/2004, TREATED BY DR, GIVEN RX AND ONE WEEK RESTRICTED DUTY.
WORKING ON ENGINE REPLACING LEAKING GASKETS WHICH REQUIRED REMOVING AFTER COOLERS. INJURY OCCURRED WHILE BENDING OVER PICKING UP OIL ABSORBANT PADS ON THE GROUND TO PROPERLY DISPOSE OF & HIS BACK "POPPED" AND FOLLOWED BY LOWER BACK PAIN.
EE WAS ASSISTING 2 CO-WORKERS IN MOVING A CABLE OUT OF THE ROADWAY BY HAND. AS THEY LIFTED THE CABLE, HE FELT PAIN IN HIS BACK. HE BEGAN MISSING WORK 9/17/04
Employee bent over to pick up a piece of nylon rope and experienced pain in his lower back. He was diagnosed with a lumbar strain.
Employee was using air to remove a brake hub from a shovel propel transmission. When the air was applied to the brake cylinder air fitting, it pivoted down and pinched his left forefinger between the brake cylinder and the propel case. On July 26, 2004 the employee went to see a doctor for pain and was diagnosed with a fracture in the end of the forefinger.
EMPLOYEE STATED THAT HE PICKED UP HARVARD TYPE OIL FILTER & BAIL CAME OUT, CUTTING TWO FINGERS ON RIGHT HAND.
HE WAS PULLING MINER CABLE OUT OF THE WAY IN THE ROADWAY ON THE 003-0 SECTION. HIS BACK STARTED HURTING HIM. HE CONTINUED TO WORK UNTI 7/22/04. HE WENT TO THE DOCTOR THEN. HE SAW A SPECIALIST ON 8/2/04 AND MRI WAS SCHEDULED.
Employee attempted to push a cut tree using one hand to fall in direction. Employee strained his back.
Employee was in preparation dept. crushing coal samples. When he lifted the lid on crusher to clean, wt. approx. 70 lbs., it fell back down and hit hand
HE STATED THAT HE WAS IN THE PROCESS OF MOVING A PINCH ROLLER FOR A BELT STORAGE UNIT USING A DIESEL LO-TRAC. AS HE WAS LOOSENING THE C.M. PULLERS THAT HAD THE ROLLER SECURED, THE WEIGHT SHIFTED AND CAUGHT IN HIS LEFT MIDDLE FINGER BETWEEN THE CHAIN & FORK OF THE LO-TRAC, MASHING IT. THIS RESULTED INTO A FRACTURE TO END OF HIS FINGER & STITCHES.
Employee was struck by a flying piece of metal from a dozer track which was being unloaded. He suffered a laceration to his stomach and arm.
Injured employee was standing behind dozer track as it was being aligned to be installed. The end of the track was elevated. The end of the track fell striking employee on the left leg.
EMPLOYEE REACHED INTO BOTTOM OF AIR CONDITIONING BOX AND CUT HIS FINGER WHEN HE INADVERTENTLY TOUCHED "SQUIRREL CAGE" GEARING.
Employee was transporting bed wear strips with a fork lift. Two wear strips fell off. He placed one strip back on the fork lift. He picked the end of second strip up and twisted his back as he was trying to get it onto the fork lift.
Employee was assisting another employee dispose of reject coal. He lifted a barrell of reject coal and felt a sharp pain in the groin area
Two employees were loading a 4' by 8' sheet of stainless steel into the back of a pickup. While pushing the sheet into the bed of the pickup, one employee cut the palm of his hand. He was not wearing any gloves. The cut required 9 sutures to close.
WHILE CONDUCTING HIS PRE-SHIFT INSPECTION OF HIS HAUL TRUCK, EE'S RIGHT INDEX FINGER WAS CAUGHT BETWEEN ACCESS LADDER AND FRAME. THE FINGER WAS LACERATED AND REQUIRED SUTURES TO CLOSE.
EMPLOYEE WAS ADJUSTING A CHOKER ON X-BRACING & FELT PAIN IN THE LEFT SHOULDER, WHEN HE REACHED UP TO ADJUST IT.
EMPLOYEE WAS ENTERING THE CAB OF A CATERPILLAR 988F ENDLOADER. AS HE SAT HIS LUNCH BOX ON THE CAB FLOOR, HIS HARD HAT FELL OFF. HE ATTEMPTED TO CATCH THE HARD HAT WHILE BENDING OVER. HIS MOUTH CONTACTED THE SUPPORT GUARD SURROUNDING THE JOYSTICK CONTROL. THE CONTACT DAMAGED A FRONT TOOTH.
EMPLOYEE WAS STRUCK ON THE LEFT INDEX FINGER BY BOLT/NUT FROM CUTTING EDGE ON 575 KOMATSU DOZER. *NOTE: EMPLOYEE DID NOT VISIT DOCTOR UNTIL 1-16-04. MANAGEMENT WAS NOT NOTIFIED OF THE DIAGNOSI S UNTIL 1-17-04. MEDICAL RECORDS WERE NOT OBTAINED UNTIL 1-21-04.
