High winds blew the doors on the back of the Drag Line house open. The employee and two others decided to close the doors. They successfully closed the right door and were attempting to close the left door when a gust of wind hit the door and flung it open blowing all three of them back with the door. The employee was pinched between the door and the guarding around the Gen-set.

Showing 50 of 117While positioning a barrel of solvent in the lube room of the 102. The employee's finger got caught between the barrel and the air compressor bank pinching the Left hand pinky finger crushing it between the barrel and the compressor bank.
Employee noticed a hose blew apart down in Ramp 58. EE went down to the pump, shut the pump off and started to disassemble. The hose was not repairable, so EE went to get a different hose to replace it. When starting to pull the hose apart EE felt a poke through the glove. After removing the glove, the employee found a laceration on their left middle finger.
Employee was performing an oil change on 1774 after removing the first oil filter. Employee began to remove the second filter the employee felt 3 pops in their right arm between the bicep and forearm. Employee stated that the filter could have been stuck or had been put on too tight. MRI was done and a torn muscle was found.
Employee was opening the back doors of the dragline and placed hand on the lower rail of the door. When the door swung open, EE's left hand was located in a pinch point between the door and the back house fan.
Employee was working on 308 drill outside the shop. EE was standing on the mast loosening a hydraulic hose, after they broke the hose loose, they stepped down with right foot, left foot was caught under another hydraulic hose and right foot landed on uneven footing causing them to roll the ankle. Resulting in a small fracture to the right foot.
Employee was changing out a transfer chute wear plate. The employee was directing the crane when the wear plate became hung up. The employee freed the wear plate and as the plate settled the employee's right wrist was caught and bent backwards. The employee was referred to University of Utah. A diagnosis from the employee was received on 03/29/2021 after EE's surgery.
Employee was stocking the trays on the roof bolter, when EE's hand was pinched between the mesh rack and the bolt pod. The mesh rack had been set down on an open bundle of bolts. When the plates were removed the bolts shifted allowing the mesh rack to drop down about 1 inch which was enough to pinch the employee's hand.
The Skid Steer bucket slipped off the attachment shoe edge and fell approximately 1.5 foot to 2 feet to the floor striking the employees foot behind the hard toe.
Employee arrived at R1 drill pad work area. Exited 2470 pickup, went around to get lunch box out of the passenger side. Grabbed lunch bucket with left hand, holding the door with right hand. The wind pushed the door shut and right middle finger was struck between the door and the frame. The finger was lacerated resulting in sutures.
While building crib, employee got hand caught between 2 crib blocks.
Employee was lifting hood off of D11 CAT Dozer, Hood shifted and pinched left index finger between the hood and the holding bracket for the hood. Broke index finger.
While fueling 442 Haul Truck at Ramp 53 fuel island, and was putting chock blocks in holder and jammed left middle finger resulting in an extensor tendon tear.
Employee was installing wedges into fiberglass rib pins. While performing this task they felt their eye becoming irritated and red. Medical personnel removed a piece of fiberglass and prescribed medication for the eye injury.
Employee was lifting a belt roller, the roller slipped and while catching the roller they felt pain in the shoulder and arm. Employee was referred to an orthopedic doctor and following an MRI review (09/18/2018) it was determined they required surgery (11/01/2018) for the injury.
Employee was working at a surface PC room. While working on the PC they decided to open a garage door, as they attempted to open the door they bent over to grab the middle of the door, as they bent over, left foot caught on some grating causing them to fall forward hurting shoulder. Recent medical evaluation has placed employee on Lost Time from the work place.
Employee was removing a Kennedy stopping, the stopping had a mandoor in it. After removing the majority of the stopping, the mandoor rolled off of the remaining few panels and struck the employee on top of the foot. Late reporting due to recent medical evaluation placing employee on days away from work starting on 6-5-2017.
Employee had been performing several tasks during working shift, tasks included carrying belt rollers, picking up/carrying crib blocks and rolling cable. During the shift employee felt pain in groin area and on March 10, 2017, medical professionals diagnosed employee with a hernia. Late reporting due to late medical evaluation.
Employee was hanging high voltage cable in the intake of Main West #4 Section. During the process the employee's fingers were pinched between the tub and mine rib line. Employee received stitches to close the laceration and was placed on restricted duty.

Employee was removing a guard on a dozer, when the employee picked up the guard he felt a pain in his stomach. The employee was seen by a physician and scheduled for surgery. During the surgery it was determined that the hernia was caused the injury that the employee sustained on 8/8/15. The company received the surgical report and final diagnoses on 9/29/15.
Employee was helping in moving longwall's power train, including pump car. Employee was placing cribs along rib line to steer train away from the rib. In the process, he slipped causing his foot to be caught between tank car and rib. He has been placed on lost time with fractured bones in his foot as well as laceration.
Employee stepped out of 1768 flatbed truck turned and began to walk through the mud to the dragline bucket. Were he strained his left Achilles tendon.
Employee in the process of modifying a beltline roller hanger. Employee began to measure for the proper dimensions for the hanger, he had placed the hanger on the belt structure. As he was measuring the hanger fell and struck him on the top of the hand. A bone in his hand was fractured and he was placed on restricted duty.
Miner was loading a piece of belt structure onto a trailer. As he was loading the piece of structure, the piece moved side ways, he tried to catch the piece of structure. His finger was pinched between two pieces of structure. The wound required stitches to close.
Injured miner was packing 6" water line with another employee, when a piece of wire, that was hanging down from the mine roof, went under the miner's safety glasses and struck his eye lid.
Employees were working on the emulsion line, one employee turned the line off and opened the relief valve to the line. The opened the line up and some emulsion splashed into his eye. He received a prescription for the eye.
Employee was installing a resin tube into a roof bolt hole on the longwall face. He asked another employee to open the drill guide of the pan bolter for him. The injured miner had his hand between the drill guide and frame of machine. He was unable to return to the work place following this injury.
Employee was engaged in repairing a hydraulic cylinder of a shield on the longwall face. As he was aligning the cylinder, the cylinder slipped and fractured his finger against toe of the shield.
Employee was installing a bucket rigging pin in the upper hoist chain on the company #103 dragline when he felt a pop in his left elbow.
Employee was advancing continuous miner cable, as he was performing this task he felt pain in his stomach area. Employee did not seek medical attention at the time of the incident. Recent medical evaluation determined he suffered a hernia, the hernia was repaired on November 2, 2015.
Employee picked up one end of a thread saver for an overburden drill and felt his left calf muscle pop behind left knee
Employee was lifting the cab of a skid steer loader to get filter numbers. As he was lifting the cab he felt a pain in his abdomen / stomach area. It was determined that he had a hernia. The hernia was repaired and employee was placed on restricted duty.
Employee was installing a piece of skirting around a seat base of Getman Tow vehicle. During the installation of the skirting the employee felt a pop in his elbow. He has been placed on restricted duty.
Employee was straightening out some 1" hose, as he was bending down he felt a pop and then pain in his back. He was released back to regular duty following the incident but has since seek further medical attention. He has been placed on restricted duty. Late reporting due to the later medical attention which placed him on restricted duty.
Contractor was handeling roof mesh material, a piece of the mesh hung up and the contractor attempted to free the mesh by pulling with more force. His shoulder dislocated while trying to free the roof mesh.
Contractor was in the process of hanging beltline bottom rollers. As he was lifting the contractor felt pain in his back.

Contractor was lifting a Kennedy mandoor onto a trailer, the mandoor slipped while in his grasp and the edge of the door cut his hand. His laceration required stitches to close and was placed on restricted duty.
Employee was opening a man door in the shop wash bay. The door was jammed so the employee pulled hard on the door and felt a sharp pain in his left shoulder.
Employee was exiting a ladder and over extended his left leg on the last step and felt pain in his lower back.
