Employee was working on surface at Preparation Plant cutting a 3" valve out of a water pipe when the pipe fell hitting employee on top of foot. Employee has been working since date of accident and did not start losing time until 4/8/16 due to first doctor visit showing fracture to toes and hematoma to foot.

Showing 50 of 708REPORTABLE ONLY - Employee had been using a pry bar to remove the vertical screen off the trash box. EE layed the bar on top of the trash box and continued to work on the screen, the bar fell striking employee in mouth/teeth chipping two teeth.
Employee was attempting to open door on powder box and (he had is ring finger on his right hand in the crack) when he opened the door, he mashed his finger. Surgery will be on 12/4/15. No work.
Employee was working to re-install pin on #112 shield that had backed out and when he bent over to lift clevis head he felt a burning pain in his stomach. Employee was diagnosed with a hernia on 11/25/15 and started losing time due to surgery.
After arriving at the unloading point the employee got off of his Locomotive and began disconnecting safety chains from between the four Locomotives. As he was walking by Locomotive 43, the sight glass for the coolant system popped off and sprayed the employee with coolant fluid, contacting him on the left shoulder, neck and face.
Employee was helping lay track underground and was putting tie clips around to latch the ties to the rail and the rail slipped falling on his right foot causing fracture to foot.
Two employees were unloading 4" I-beams to use as roof support. One employee picked up one end of a beam causing it to roll over pinching the injured employee's finger against the bed of the flat car.
Employee was helping install a whole belt frame, when pushing frame across bottom belt to another employee he got his hand caught between the whole frame and rail causing fracture and sutures to the ends of third and fourth fingers of his right hand.
Employee was de-trashing belt take up area when he picked up a bottom take up roller and felt pain in lower back causing lower back strain.
Employee pulled slider rail forward and it slid out of the bracket striking him on the right shoulder. Employee worked up to his surgery date which was 7/13/15.
Employee was lifting scaffold board up onto the pallet of blocks. His finger was caught between the end of the board and the edge of blocks. He was taken to the doctor and received two stitches. Returned to work on 6/6/2015.
REPORTABLE ONLY - Employee was assembling 6" Aquamine pipe, while trying to connect a 6" to 4" reducer his left thumb was hit by victaulic coupling causing fracture to tip of thumb.
Employee was pushing against the fork of the Lo-Trac to re-adjust for picking up a core box, when he felt a pop in his left elbow. EMPLOYEE HAS BEEN WORKING SINCE DATE OF ACCIDENT UNTIL TAKEN OFF WORK 7/27/15 DUE TO A PROCEDURE BEING DONE.
Employee was helping build additional cribs due to the top deteriorating at the mesh line alongside the trailing shields. As the employee set the last two crib blocks on top of the crib, the rocks came off of crib onto him causing laceration to lower leg requiring sutures.
Employee was checking rockdust hose coming off of a manifold, just after he opened a ball valve to let air through, the hose came off of rockdust pipe hitting employees right hand causing fracture to hand.
Employee was roof bolting and in the process of pushing glue into a drilled hole with one hand and with the other hand lifting a four foot roof bolt that had a plate and T-board when he felt his shoulder pop causing immediate pain in shoulder. EMPLOYEE HAS BEEN WORKING SINCE DATE OF ACCIDENT AND DID NOT START LOSING TIME UNTIL 1/12/15 DUE TO SURGERY ON HIS SHOULDER.
Employee was stepping up on bolter to travel up through bolter to start roof bolting. As he entered the bolter he struck his Rt arm on a stack of roof bolt plates which cut him on the Rt forearm.
Employee shift started on 11/9/14 @ 11:00 PM. Employee was lifting 700+ lb. pump over a dam to set in sump. While pulling with come a longs, the pump rolled to the left where employee's hand was. His hand was caught between the grating on bottom of pump and a 4x4 on the decking coming off the block dam. Employee received sutures to his left thumb.
Employee was helping move roof jacks in order to get roof bolter in cross cut. As the employee went to bend over to pickup the jack up is when he felt a pop in his lower back before picking up jack, causing a strain to his back.
Employee was raising side door on lube track door is spring loaded so when door is raised spring helps raise it. Employee had a hold of factory hand when door was raised near the top or raising the door employee's hand was pulled into frame of door causing a cut to the inside of his index finger on Rt hand.
Employee was getting an ax out of a tool box and the lid fell down onto his left hand mashing his thumb. Went for treatment and received stitches.
REPORTABLE ONLY DUE TO SUTURES - Employee was taking down a section of shuttle car cable when he punctured his hand on a cable hook that the cable was in causing laceration requiring sutures.
EE was moving a caustic line when the line fell over and splashed the chemical in his eyes.
Employee was trying to unplug discharge hose for slurry pump when disconnect came apart causing him to get water and oil in eyes. Employee did not seek medical treatment until week later receiving prescription eye drops.
Employee was lifting a bretby chain out of face conveyor when he felt a sharp pain in shoulder. EMPLOYEE HAS BEEN WORKING SINCE DATE OF ACCIDENT AND DID NOT START LOOSING TIME UNTIL 8/29/14 DUE TO SURGERY ON SHOULDER.

Employee was moving feeder cable. He was pulling on cable and felt pain in his left lower back. His first date of loss is 7/16/2014.

REPORTABLE ONLY - EE was checking hydraulic leaks on Lo-Trac when the lid fell hitting right hand. (EE was seen by Dr, x-rayed with no fracture shown and returned to work). Due to increased pain the physician did another x-ray on 6/12/14 that then showed fracture to two fingers. Became reportable on June 12, 2014 due to fracture. *EE began losing time on 6/30/14 due to surgery.
Employee went to cross over a belt that was off to help with moving monorail, he was pulling himself up on the belt when he started to raise up he felt a pull and strain to his back.
Employee was helping hang 8 inch Aqua mine discharge pipe through a brattice when the pipe that was approx. 6 to 7 feet in the air fell striking employee on foot/ankle causing fracture to ankle.

Employee was beside track rail with a track jack under the rail, raised six inches, and the track rail fell off the jack, landing on his foot resulting in fracture to the foot.

Employee was pulling up 2" dust hose in right return when he felt a pull or strain in his lower back. Conditions were muddy foot wall and water.
Employee was sweeping off side West C belt and stated that while he was sweeping, he had a pain in his right shoulder. He has been treating with physician since time of injury and due to his restrictions issued 3/3/14 - we can no longer accommodate his restrictions as of 3/4/14.
Employee was removing mandoor from brattice when a solid 8" block that was above the mandoor came loose hitting employee fingers on both hands causing fracture to a finger and requiring sutures to fingers on both hands.
Employee entered through a man door right behind another employee and caught his left finger in the door. Employee did not seek medical treatment until 1/6/14 and was taken off work on 1/13/14 due to fracture to the finger resulting in surgery.
