Employee was attempting to widen the forks on a telehandler. During the process, one of the forks slipped from EE's hand and pinched EE's left hand between the fork and the mast, resulting in a laceration to EE's thumb and forefinger which required sutures.

A non-injury accident occurred involving 7-10 hoist, Employee put a drop deck supply car on the elevator and the car was mis-aligned on the elevator causing the draw head of the car to strike the west side of the shaft resulting in elevator to be removed from service for approx. 3 hours.
Employee was operating the Lo-Trac cleaning up material when they hit a jack, shifting a timber which fell striking the Lo-Trac roll cage. The timber had struck employees hard hat pushing hat down on ears causing laceration to right ear requiring in sutures.
Employee was in the process of changing out a discharge sprocket. While releasing a come-along, elbow accidentally hit the brace of the sprocket causing a laceration.
REPORTABLE DUE TO SUTURES - Employee was working on an eight-inch Aqua-mine discharge pipe on NE-9 in the #1 Entry at XC #50 when it burst striking the employee on the left side of the body causing a laceration to the left ear and a laceration to the left upper arm which required sutures to both locations.

The Roof Fall Occurred Due To Undetected Separation With Laminations Above The Anchorage Point Of The Primary Roof Support Installed During Development.
Employee was helping install tubing inby the trunk line in the #1 entry of 12 section, when EE got EE's finger caught between 2 pieces of vent tubing resulting in stitches in L Hand finger.
Roof Fall - Occurred on the East Panel Track (East Main #4 entry) at x-cut 77, the old E6 track/belt overcast. The roof fall area measured approx. 15' long x 15' wide and fell above the anchorage bolt.
Employee was assisting with installing a bull hose on a 8"" Aqua pipe for electric pump on NE-9, when employee lost balance and fell into the water and the 8"" Aqua pipe fell on their left foot/ankle causing it to become caught between the mine floor and pipe, resulting in a sprain to their left foot/ankle.
Employee was riding in a manbus to the bottom, when a piece of draw rock fell out of the top striking employee on the left side of face/mouth, resulting in a broken tooth.

ROOF FALL - Occurred on #2 Longwall, falling from shield tips on longwall headgate shields outby for approx. 30 feet (#1 entry, crosscut 36-36-1/2). The fall was from rib to rib and fell 7 feet above the anchorage bolts.
Employee was working on the #1 Longwall when EE's Left Lower Leg became pinched between the Pan Line and a part that was laying on the #7 Shield Pontoon resulting in a contusion and swelling to left lower leg.

Employee was buffing a belt for splicing while EE moved EE was struck by the buffer when the cord got stuck. Resulting in 4 stitches.
Employee was washing down the slope when they tripped over a rock falling, injuring their left shoulder. Employee has been working since date of accident until they were taken off work on 12/13/23 due to surgery.
Employee was in the track entry to move a pump. As EE picked the pump up, EE's foot slipped causing EE to fall on their back resulting in injury to lower back. Employee has been working since date of accident until was taken off work on 12/20/23 due to surgery.
Crew loaded a piece of plywood on the outby end of the mantrip. Once they got on the main line, the air current started to lift the plywood up and down. While traveling the air current got under the plywood, flipped it back and hit the employee in the chest. EE was in the inby end of the mantrip.
Employee was installing rib pin on left rib when EE felt pain in left shoulder, was placed in sling due to rotator cuff injury.
ROOF FALL - Occurred on NE-6, 10 section in the #2 entry of the bleeder system, one crosscut inby spad #32236 crosscut #1. The Mine Roof fell above the anchorage bolts due to adverse roof conditions.
Employee was installing liner plates in the south skip While working out of a Spider basket from an elevated position, employee reported an electrical arc and instructed them to cut the power, the rope broke supporting the man basket causing the basket to fall approx. 30 feet to the ground causing fractures to employee back and ribs.
ROOF FALL - A roof fall was found during the on-shift belt examination of the North B Conveyor belt. The fall occurred at cross cut 138.5 in the #4 entry. The roof fall was approximately 21 feet wide x 35 feet in length and fell above the anchorage bolts.
Working on a Caterpillar D-11 putting a pin back in the push arm when the pin fell striking the employee in the finger cutting their finger
Employee was using a come-along to help pull face chain together. The chain on the come-along accidentally broke loose and the hook on the chain came in contact with employee's arm causing injury.
Employee was building a crib on top of a shield and smashed a finger between two crib blocks.
Employee was helping install ventilation tubing and as EE was rotating the tube for it to fit together, a bolt caught EE's left thumb causing laceration requiring sutures.

During the descent of the mine cage, ice fell from the shaft wall and struck the protective cover (metal top) of the cage. Abnormally extended cold temperatures resulted in the ice forming in the shaft, in spite of the shaft heaters being used.
Employee was pulling drill steel out of the hole when EE twisted, hitting the spin lever, causing EE's right hand to twist around resulting in fracture to wrist and finger.
Employee was helping to change the scoop battery when they climbed on top of scoop to plug jumper cable into battery, the battery slightly tilted forward catching their left foot between the plug end of battery and brace bar causing contusion to the foot.

The injured worker was driving an 18 wheeler loaded with coal that overturned after exiting the prep plant. The worker was treated at a local ER and released that same night and is awaiting an appointment with an orthopedic doctor to determine extent of injury and potential time off.

Employee was using a slide hammer while pulling a pin from a trapping shoe, when EE's left index finger was caught between the end of the slide hammer and the slide weight causing fracture to tip of finger and laceration requiring sutures.

When EE went to remove dirt from receiving end of bracket, the dirt became dislodged and the wiper fell catching EE's left middle finger between the ITC blade and drip pan causing laceration and fracture to tip of finger, requiring sutures.

REPORTABLE ONLY DUE TO SUTURES - Employee was on East C Track on the locomotive taking an empty single end car back to put in E3, when one of the sander lids came off hitting employee in the mouth causing laceration to lip requiring sutures and chipping front tooth.
Individual reported they slipped while stepping off of a step ladder twisting right knee. Based on medical information received on 11-3-21 the alleged injury is being submitted as Lost time accident.

An employee was on the 7-10 Service Cage headed down into the mine, when at about 170 feet from the top, the switch gear at the mine 7 east sub-station went phase to phase causing the cage to stop from loss of power. The employee was not injured and was entrapped on the cage from 4:30 p.m. until 6:50 p.m. when power was restored and the employee was hoisted to the surface.
Employee was walking toward the section when employee stepped off side of bridge board in roadway spraining left foot and was placed in a boot.

Incident - East C Belt had a bottom roller at cross cut 126.5 to go bad on offside of belt, CO sensor down wind gave alert to rise in CO. A 5'x 9' area in the floor was found smoldering. Water was applied to area and material shoveled to ensure the smoldering area was out.
Employee was beside the roof bolter while the operator was trying to use the ATRS to loosen the chain holding the slider, when the chain broke letting the slider and two tubes fall striking employee causing EE to fall to the ground spraining right knee and right ankle.
Employee was doing chute work along the belt and was standing on a ladder. The ladder slipped causing the chute plate to fall and hit EE's finger causing injury.
Employee was operating shear when a rock came over the handrail striking employee's left lower leg resulting in fracture to fibula.

Employee had finished cut in #1 entry and was standing talking to the day shift miner operator coming on shift, when a piece of coal fell off the rib striking employee hard hat and right shoulder causing abrasion and contusion. EMPLOYEE HAS BEEN WORKING SINCE DATE OF INJURY UNTIL 10/8/2020 WHEN EE WAS TAKEN OFF WORK DUE TO FRACTURE REQUIRING SURGERY.
Employee was trouble shooting a SC cable and individual stated they thought they felt a shock and stepped back on material (rock/coal) twisting a knee. Employee was diagnosed with a sprain/strain to the right knee and placed in a knee brace.

Employee was operating a mantrip and accidentally lost control wrecked the bus. Employee attempted to jump out before impact which caused injury to ankle, leg, and foot.

NON-INJURY INCIDENT - The West Seals 1-14 were unable to be inspected on the seventh day weekly inspection due to water in the walkway. Management withdrew the miners except miners used to pump water.

Employee was preparing to service the #1 face, as EE removed a timber that was leaning against the rib, a piece of rock fell out striking employees right foot causing contusion to EE's foot.
Employee was operating shearer mining toward the headgate when they were struck by two rocks, one struck left shoulder causing contusion and one struck left hand that was holding the shearer remote causing contusion and laceration resulting in sutures to left ring finger.

ROOF FALL - Occurred on E-12 No. 2 Longwall in No. 1 belt entry from cross cut 33 to 33-1/2. The rock fall was above roofbolt anchorage and was approx. 60 feet long, 20 feet wide and 10 to 14 feet in depth. No one was injured.
REPORTABLE ONLY DUE TO SUTURES - Employee was installing ventilation tubing and attempting to pull out slider that was stuck with left hand, EE braced with right hand catching right ring finger in the O-ring causing laceration to tip of finger requiring sutures.

Employee was standing in rear cargo area of delivery truck using lift gate to lower a Z-rail clothes cart. Z-rail cart shifted causing employee to fall from truck resulting in fractured wrist, knee and ribs.

An air reservoir tank was being installed when it was dropped approximately 36"" landing on the injured's right pinky finger causing several fractures. Treatment was given at a local hospital and referred to a surgeon for follow up. Surgery was performed on the broken finger on 1/15/19.
Employee was prying on the snake head trying to get an E-bolt started through the slot in the pan line, when EE reached down to try and start it by hand, the other employee did not see EE put EE's hand down, accidentally hitting EE's hand with a sledge hammer causing fracture and sutures to fingers.

REPORTABLE ONLY DUE TO SUTURES - Employee was walking when ee tripped over bags of rockdust falling and hitting safety chain between power centers causing laceration to bottom lip requiring sutures.
IGNITION - Occurred at East A head roller at the North Bunker, while burning a hole at the top of the North Bunker a piece of slag dropped down into bunker and ignited methane that was in the coal, in the Bunker. The ignition was extinguished with one fire extinguisher.
REPORTABLE ONLY DUE TO SUTURES - Employee went to turn Lo-Trac around and struck a pallet of Brattice Blocks, one of the blocks struck left thumb causing laceration resulting in sutures.
IGNITION - occurred on 9 section in the No. 1 entry while installing a rib pin on the third row of bolts. Ignition lasted 4 to 5 seconds and was extinguished with one fire extinguisher.
Employee had placed the skirt hold down plate across the forks of the Lo-Trac to burn holes in plate, the hose from the torch caught the plate causing it to slide pinching finger between the plate and mast causing laceration requiring sutures and fracturing tip of right hand finger.

IGNITION - Occurred as the headgate drive was raised about 7"" above the foot wall when a methane/frictional ignition occurred under the panline at headgate drive from the longwall face chain and was extinguished with water.
Employee was assisting with lifting a chute using three ton come-a-longs, when they went to unhook from the frame they unhooked from chute causing it to come down hitting their left arm causing contusion.
