While retrieving an oil supply hose from a service island the hose became hung up on the outside of the reel causing the employee to stop short straining the right shoulder.

A strap was being used to check the pivot bearing on a grader. The strap broke and struck the employee in the left hand. Ligaments in the left hand were weakened and the employee was released on light duty.

The job required removal of previous system that ran from the drivers console through the "man whole" work area, to the rear of the machine. The space was very confined. While installing the new system, the employee took many short breaks to stretch due to a timely duration of removal and installation of new system. At the end of shift the employees back was sore and tense.

An employee injured an arm on 9/26/2018. Employee did not receive treatment for this injury at that time. Later on they had an MRI indicating a muscle tear and received surgery on 11/08/2018. The incident became reportable that day.

Employee was injured when ee tried to lift a 25 lb block of oak up onto the stageloader. As ee lifted, the block struck against the corner of the SL, causing a shock load to right arm. EE felt a pop and a burning sensation.

Employee was working on the explosive crew and was carrying a 35 pound box of boosters across the pattern and felt a sharp pain in their right knee. The employee has managed the injury but required surgery on 1-29-2019 which is also the first missed shift for this employee.

EE reported they were lifting the end of the trough roller shaft to move it sideway about 4 inches. The shaft was greasy and started sliding out of their hands and they tried to set it down but had the middle finger of right hand under it and smashed the finger hard enough to bruise it.

Employee was working on a metal bracket and was trying to move it into position to lay out the bolt holes. The metal bracket slipped off the edge of the underlying channel and pinched their right little finger between two pieces of metal. This was reported on 05/13/2018.

Employee was removing an oil cooler hose on a D10 Dozer. While in process of removing the hose employee bumped a steel plate that was leaning against track. The plate contacted employee's finger resulting in a fracture.

Employee was lifting a 26 pound wheel chock and it slipped out of their hand. The employee caught the chock with both hands and felt pain on the left side of their chest. Employee was diagnosed with a cracked rib and was unable to work their next shift.

EE was assigned duties as a blaster. While backfilling coal blast holes, EE felt a pop and pain in right shoulder and neck area. EE returned to work and continued to have pain. EE was evaluated by a doctor and was unable to work starting 03/15/2018.

Employee's back became stiff and sore after several shifts of replacing/repairing overhead lights around the facility. On February 6th the employee's back went into a spasm while ee was at lunch. Inflammation had developed causing low back pain and spasm.

On 11/15/2017 an EE was separating sections of 4" waterline with a crib block. The crib block slipped off of the joint of water line and EE struck finger between the crib block and a fire valve. The EE fractured the tip of finger.

Employee was repairing exhaust on a D10T dozer and strained their right wrist. Physician released employee to work. Physician prescribed a support brace on 11-22-2017 and the wrist required surgery on 1-16-2018.

A machine cover slipped while it was being installed and the middle finger on the left hand was caught between the cover and a support beam on the machine. The impact caused a laceration to that finger, requiring 4 stitches.

Employee reported a late injury on 10/16/17 that while changing out an oxygen bottle at weld shop ee felt back pop and pain in lower back. EE lifted and twisted at the same time. EE had help available but did not ask for help. Possible herniated disc.

Employee was removing a carrier plate off of a D11 dozer and had their left thumb pinched between the plate and the swing frame. The employee pulled their thumb free and noticed it was bleeding. Local medical center doctor advised employee to come in after the shift. The wound took 2 stitches to close.

Employee was removing a wheel chock from the bumper of a haul truck and felt a pop in their right shoulder. Employee was seen at local medical center and diagnosed with non-surgical tears in the rotator cuff muscles.

The technician was installing a roll pin into repair link for chain and a small piece of metal broke off and was embedded into upper left arm. The wound caused bleeding which an EMT was able to get stopped before technician was transported by ambulance to the local hospital for care.

Employee had hand on the roller of a nose cone, from a D11 side frame, while hoisting the nose cone to the ground for maintenance. While guiding the nose cone to the ground, the right side landed on the ground first. The left side had not yet landed which caused the strap to rotate unexpectedly pinching the employees thumb.

Employee was performing daily maintenance on site boiler. The discharge auger was either plugged or not working. EE shut down blowers & opened boiler inspection door. EE hit the discharge trip lever to dump ash & that action caused hot ash to come up out of the door & hit EE in front torso & right side of face causing 1st degree burn to side of face & 2nd degree burn lip.

On the above date the employee was manually removing chunks of rock from a catwalk near a belt transfer. No treatment was immediately sought. Treatment was sought later and the employee was placed on restricted duty of not lifting more than 10 lbs and as such became reportable due to the restrictions.

Employee was climbing boarding ladder on 16G Motor Grader when EE felt a pain in lower back. Injury was not reported to the mine until 3 days later. Incident was referred to 3rd party Workmans Comp carrier to evaluate claim. Work Comp has accepted the claim as of 3/20/2016. Employee missed next scheduled shift after injury so it is reportable.
