Over-exertion (Not Elsewhere Classified)

Area was muddy. Employee was mounting a Cat10R Dozer when EE's right foot slipped causing to EE to fall back and EE landed on EE's feet. EE was holding onto the grab iron on the dozer when EE slipped causing a left shoulder strain. EE slipped because EE did not put EE's right foot on the step.

Accident type, without injuries

Grass fire on a non-active part of the Jewett Mine. We are not sure what started the fire, but there is a service road that that is accessed by gas well personal as well as mine personal. The fire did not start for the main road but started on the inside of the mine property. There is a high voltage power line located where the fire was as well, but no lines were down.

Struck by flying object

Employee was using 12lb sledge to knock a stuck ripper tooth off a D10 dozer. A small piece of metal chipped from tooth and went into employee's right calf. The employee had to have the metal surgically removed.

Accident type, without injuries

Area in mine is under a severe drought. XT0's Gail King compressor station was being routinely serviced which causes the flare stack to ignite. Oil and or debris is carried up the flare stack and ignited also. This burning debris lands on the dry grass and grass fire is started. The day was windy also.

Accident type, without injuries

The weather was hot and windy as employees were cutting and baling rolls of hay. No equipment was damaged, but 23.78 acres of grass & hay rolls were burned. We could not identify how the fire started. The fire had spread to two rolls of hay when the first employee noticed the fire. Several local fire departments helped put the fire out.

We had a Cat D10T Dozer catch fire. The fire started from under the cab near torque converter. The operator activated the automatic fire suppression system before exiting. The morning was cloudy and windy. The dozer's cab interior was a complete loss. The damage to the engine/torque converter has not been assessed. We are still investigating to find the exact cause. NO injuries.

Struck by flying object

Working on dozer. Employee was going to paint an area on dozer to help locate tracks that were loose. EE retrieved new can of spray paint out of fireproof cabinet, and stored in cabinet for a few months. First time to use, and it wouldn't come out, EE shook can and didn't hear the ball inside so tapped it on the bed of work truck. Can exploded and hit EE in the face.

This was a grass fire that was started on FM road 1512 off of the mine property. The fire progressed onto mine property because the grass was dry and conditions hot.

Gas Compressor station had been down for maintenance. Start up procedure was started. Due to compressor station being a sour gas location the flare stack has to be ignited. The mechanic said that there may have been some liquid in the system that was expelled through the stack causing the fire to start when the stack flared.

Employee was exiting training room for break. Employee's foot went under the edge of the floor mat that had curled up causing the employee to fall. Left ankle was dislocated and fractured.

EE was helping install a transmission hose on a coal hauler. The hose fell & struck ee in the upper lip, cutting lip. The laceration required three (3) stitches.

Struck by flying object

EE was driving a keeper out of a tooth base. EE struck a keeper removal tool with an 8 pound sledge hammer. A piece of metal came off of the removal tool & struck EE in the left thigh area. On Tuesday, 8/9/16, the piece of metal was surgically removed.

Over-exertion in lifting objects

EE was cleaning up in #3 swinger room of a Marion 8200 dragline. EE straddled the compartment divider, leaned into the #3 room & picked up a bag of soiled pig blanket. EE turned to sit it in the next room & felt EE's knee twist. EE sought medical care on Monday April 11, 2016

Fall onto or against objects

Stepping down off of the bed of a truck ee stepped onto the step installed above the receiver hitch. His right foot slipped off of the step & fell off, hitting his back on the step. He did not seek medical care until 10/31/15 at which time it was discovered that he had 2 fractured ribs.

Shift started 6:00pm on 8/23/15.EE was working on a maintenance truck in the maint. shop all shift. About mid shift, his left eye began to irritate him & worsened as the shift progressed. He reported this at 4:30am on 8/24. He went to an eye Dr who removed a piece of metal from his eye & gave him a prescript. for eye drops.

Employee was using a wooden handle to prop open his tool box lid. As he was closing the lid the handle came out & struck him in the mouth. It broke one of his front upper teeth off at the gum. (The tooth had a cap on it.)

Employee was dismounting a coal hauler. In the process, he struck his knee on the bumper, causing pain to the outside area of his left knee. He went to a local E.R. on 6/20 and saw another dr on 6/22. He is working restricted duty.

Caught in, under or between running or meshing objects

EE replacing fan belt on dragline tail fan, locked & tagged at power source. After tightening belt, was checking the tension by pushing on the drive belt with his hand. He could not see the fan in another compartment. Did not realize the fan was moving. The belt grabbed his hand and pulled his finger into drive pulley, cutting his finger & fracturing bone. Stitches were required.

Employee was removing a compression fitting from a fuel hose using a crescent wrench. The wrench slipped, causing him to lose his balance & fall to the ground, hurting his shoulder. He has continued to work with the injury & had surgery on April 15, 2015.

Employee was assisting a mechanic in changing out cutting edges on a Humdinger Scraper pan when his left elbow contacted a sharp edge on the scraper pan resulting in a 2.5 cm laceration to his upper left arm requiring 4 stitches.

Worker stepped down off of ladder, walked over to get a small bolt, as walking back to ladder felt like air in lower back. As worker climbed ladder, 2 steps up, felt pain. Continued to feel pain.

EE was changing the left front tire on his F-250 Ford pickup. They had jacked up the truck and removed the tire. The drill helpers tried to put the new tire on the hub, but the truck was not high enough. EE got under the truck to raise the truck to put the tire on, but the jack slipped and the truck fell on EE, on his right side.

Employee was assisting in the loading of a backhoe onto the lowboy trailer at the shop. As he walked from the back left side of the trailer to the back right side of it, he walked into the push block & cut his forehead. He was taken to a local emergency room where he received three (3) stitches to his forehead.

Two(2) employees were installing a shaft in a roller. The shaft got stuck and employee put his hand in the hub. His left small finger (pinky) got caught between the hub and the shaft. He received an open fracture to that finger. He was treated at an emergency room, the wound was closed with stitches.

At first report, miner could not identify when or where the alleged injury occurred so because of this and other information, it was considered non-work related. Later he filed a workers comp claim and after hearing, the TWC held it to be work related with an occurrence date of 1/17/13.

EE lifted a turbo to set it on the frame of an end dump truck. He felt a sharp pain in his right shoulder and was taken to a local emergency room for evaluation. He is working restricted duty.

EE stated that he lifted a chock block. When he went to put it on the end dump, he stated that he felt a pain & pop in his lower back & pain in his abdomen. After investigations & doctor visits, it was determined that this was an on-the-job injury. He is working restricted duty.

Over-exertion in lifting objects

EE stated he picked up chock blocks which were chocking a tire on the truck he was driving & felt pain in his back. This incident has been under investigation by his employer, FlexSource, & their worker' compensation carrier since 5/2912 to determine if this was an on-the-job injury. On 6/19/12, FlexSource was informed by the w/c that this incident occurred on-the-job.

Employee cut is finger with his pocket knife while trying to cut his cheese during his lunch break. He was sitting in the cab of the truck. Truck was off, parking brake was set and wheels were chalked.

EE was greasing a fitting on a backhoe. The grease gun came off of the fitting and shot grease back onto him, from the base right side of his neck to his right eye. Grease got under his safety glasses and into his eye. His eye was washed out at the mine and he was taken to a local ER for further evaluation where prescription ointment was applied to his eye.

EE using 10lb sledge hammer while changing the teeth on a dragline bucket. The 1st 3 teeth came off. The 4th tooth was stuck, so he tapped harder. He felt something hit his leg, but did not hurt. Hit tooth again. Felt something warm on leg, it was blood on pant leg at knee. Taken to ER and X-rays done, showed piece of metal in leg. Unsuccessful to remove it, Dr put 3 stitches.

A contractor bailing hay using a Vermeer 605 SM Round Bailer. A roller in the bailer fails and heated up. The heat from the roller ignited the grass inside the bailer. Operator noticed the smoke and released the bail. He moved the bailer a safe distances away and unsuccessfully used 2 fire extinguishers in an attempt to extinguish the fire. No injuries in the incident.

A grass fire started on the West side of the haul road in Area-C. The fire burned 15 minutes on permitted mine property. Fire was contained and extinguished using mine equipment and local fire departments. Fire was reported to MSHA prior to learning it burned only 15 minutes on mine permit. All personnel were accounted for and uninjured. Unknown how the fire started

EE was rolling up a grease line after greasing a backhoe. The swivel fitting between the hose & grease gun broke when the hose & gun reached his left hand, cutting his hand & injecting grease into his left hand between the thumb & 1st finger. He was taken to a local ER & transferred to another hospital for surgical treatment to the injury.

EE was helping tack weld a brace support on a dragline. Standing on fold down step and stepped down onto pony winch gear box. His foot slipped off and fell on right side, landing on the gear box. Taken to ER where his injury was diagnosed as a contusion in his rib area. He returned to work the next shift. CT scan on 9/19/11, was notified that he fractured several ribs, T8-T12.

EE had mounted a bull dozer when he got into the cab and shut the door he shut his right ring finger in the door. He was taken to a local ER for treatment and was informed that he had fractured his finger.

EE was overhead welding at dragline. Stated his back started hurting. Pain went to his side & around to his chest. He was taken by ambulance to local ER. Investigations have been conducted by Tx Westmoreland Coal Co. & Company's WC insurance carrier. On 4/19/11, after completion of WC investigation, Co. was informed that a herniated disc in EE's back was result of on job inj.

EE was walking around the back of the motor grader when his feet slipped on ice. He grabbed for the ripper and when he fell he felt pain in his left shoulder. On 3/14/11 he was informed that he will have surgery on his shoulder on 3/23/11 to repair a rotator cuff.

EE was operating a bull dozer. He stopped to reach into his lunch box to get something out. When he did, he cut his hand on a knife that was in the lunch box & had come out of it's holder. He was taken to a local ER where four (4) stitches were required for the laceration.

EE was installing pads on track when he cut his left little finger. He received 5 stitches and RX.

(The shift started at 6:00PM on 10/6/10.) EE was dismounting the water truck & stepped on a rock with his left foot. He lost footing with the left foot & his right foot slipped through the ladder rung, becoming stuck & twisting at the knee. He heard a ""pop"" & felt pain in his knee. An investigation of this incident was completed on 10/20/10, at which time it is being reported.

EE was removing a backhoe bucket tooth. He struck the pin with a 12 pound hammer, the pin chipped and cut his right middle finger. He was taken to a local ER for treatment. No stitches were required. The laceration was glued.

EE was installing a lip shroud on a dragline bucket. 2 shrouds had already been installed. The shroud was hanging from the boom truck as ee was fitting the shroud in place. The shroud slipped off the bucket lip & pinched his finger between the bucket lip, shroud & cribbing that was holding the bucket up. He was taken to a local ER for treatment.

At the end of employee's shift, he had dismounted the backhoe he had been working on and was walking to the shift change vehicle. He slipped and fell to the ground, landing on his buttocks.

Bobcat stuck in work area, pulled bobcat (FEL) with excavator with tow strap. Tow strap was affixed to excavator, attempted to pull with excavator tracks only with no progress. Began pulling with hydraulic boom of excavator. With angle change, strap slipped from excavator under tension striking operator in the bobcat front end loader, injuring his nose and lip area.

She felt like the injury occurred while moving things out of the way to be able to sweep and clean properly in the shop. While doing the cleaning there was only some aching feeling just from sweeping a lot and grabbing and pulling things around. The real pain and swelling did not occur until later in the day when she woke up. Her hand was hurting so she went to the emergency room.

Mechanic was injured while attempting to adjust a propeller on a barge pump propeller shaft in the shop for repair. 2 employees were using a 6' pipe on a pipe wrench to turn the propeller shaft of the pump. There was a burr on the end of the pipe. Wrench slipped & burr on the pipe cut EE's forearm. He was taken to ER-received 8 stitches to close the wound. full duty return.

Employee was cutting the top of a plastic jug off using a utility knife. The knife slipped, cutting his left wrist. He was taken to a local emergency room where he received two stitches for the laceration.

Employee was moving between compartments in tub, thru a man way. Employee pushed upper body through the hole w/hands on outside of manway pushing/supporting this action and felt pain in Rt shoulder. Employee was transported to hosipital & was diagnosed with dislocated shoulder. Shoulder was put back in place by treating physician.

Employee was operating coal hauler when he became distracted and steered machine to the right on haul road and came in contact with guard rail. The incident caused employee to hit his left lower leg on unknown object in the cab causing injury.

Employee was performing Welding/Air Arcing, laid gouger down & dust blew up under PPE. Employee stated that he felt that he may have scratched his eye during lunch to supervisor on 6/3. Employee received first aid at the mine. Incident occurred on 6/3 and employee was taken to clinic on 6/5/2009.

Mechanic was grinding a piece of metal on a bench grinder when the metal caught and pulled his finger into the grinding wheel.

Employee was removing a pull cable or choker from the end-dump he was operating. The choker had a wire burr on it and as it slid through his right hand it caused a cut requiring stitches.

Operator hit a bump in the road causing his head to contact the ceiling and coming down hard in seat.

Root cause analysis investigation showed that the as-built pipeline and design differed, and that this area was to have a steaper slope than designed to still allow for adequate pipe cover. The slope modification did not occur and the GPS did not incorporate the slope change.

Employee was operating a water truck. At 9:30 am, his left eye started irritating him & continued to worsen. He is unsure exactly when and what got into his eye. He was taken to a local ER & treated for a foreign body in his eye. His eye was rinsed and an antibiotic ointment was put on his eye as part of the treatment.

EE was climbing down a Cat 5130 backhoe ladder. He stated that his foot slipped off the last step and he stepped down hard on his foot. He stated that his knee couldn't bend after this. He was taken to a local ER for treatment. He started working restricted duty on 8/31/08.

Employee was installing a bushing in the drag hitch of a Marion 8750 dragline bucket. Another employee pushed a pin into the bushing. The first employee's hand was on the head of the pin as the pin was slid into position, smashing his right middle finger between the bushing and the pin keeper. He was taken to a local ER for treatment for a fractured finger.

Employee climbed onto his dozer at the start of shift. The a/c was on in the cab and, as he shut the door, he felt something go into his right eye. He went to a local emergency room for treatment and got a prescription for antibiotic eye drops.

EE changed out a grinder disk on a handheld air grinder. He set it down on a table and the disk came off, striking his left hand and cutting it. He was taken to a local ER, where he received 4 stitches to the laceration.

EE was kneeling next to a person using an air hammer to knock out a bearing race. It is unknown if a piece of the chisel bit flaked off or a piece of the race and embedded in his left knee. He was taken to a local emergency room where a piece of metal was removed from his knee. No stitches were required.

EE was working on a dump rope repair at a dragline. The wind was blowing and some dust got into his eye. He received medical treatment on 11/8/07 and a prescription for eye drops.

EE was changing the hydraulic oil filter in a Euclid CH-120. He attempted to flip the lid on the oil cart to get it closer to the hauler's tank and got his finger caught between the lid and the handle of the second lid on the cart, causing a laceration. He was taken to a local ER where he received 6 stitches for the laceration and returned to work at regular duty.

EE was operating a Cat 777 end dump. She stated that she was having problems with the operator's seat. She stated that she hit a small bump in the haul road and the seat bottomed out. Later, she felt pain in her neck & shoulders. She was taken to a local ER for treatment & released for light duty.

EE was standing at the side of his pickup truck at the south side of the maintenance shop, talking with employees of his crew. He passed out, his hard hat hit against the side mirror on the truck(knocking his hard hat off). He fell to the ground & hit the back of his head on the edge of the concrete sidewalk, causing a laceration. He was taken to hospital & got 2 staples to close.

Was putting the vehicle chocks up, when the middle finger got caught between the chock & the metal cradle, mashing it. Then pulled skin off when jerking finger out to free it.

EE stepped on something in the dirt driveway north of the shop as he was walking to an end dump in the lay down yard. He stepped on it with his right foot. His foot popped & he felt sharp pain in the center top of his foot and arch. He went for treatment on 7/9/07.

Struck by flying object

Employee was working on a Cat 5130B backhoe automatic fire suppression system. He removed the bottle & was attaching the squib to wires while holding the wires. The squib discharged in his hand, cutting & puncturing 2 fingers. He went to a local emergency room on 2/14/07 for treatment & an imbedded foreign object was removed from one of his fingers.

Employee was trimming skirt rubber with a utility knife and the knife slipped off the rubber and punctured his right upper leg. He went to a local E.R. and received 3 stitches for the wound.

Was bending over to pick-up a tire chock, when he felt a ""tingle"" in his left eye.

EE was changing the right front brake chamber on a water truck. The brake chamber bracket fell and caught his finger between it and the frame, cutting his finger. He was taken to a local emergency room, where he received stitches for the laceration.

EE was going to thread a piece of pipe. Holding the pipe in place, he started the pipe threader. His fingers were at one end of the pipe and the clamp on the pipe threader caught his left middle finger, clamping it to the pipe while twisting both the pipe and his fingers. He was taken to a local ER where he received stitches for a laceration of his left middle finger. Returned LD.

EE was lifting a dragline tooth from a pallet onto a truck bed. The tooth rolled onto his left middle finger, cutting the end of his finger. He was taken to a local emergency room, where he received stitches. He returned to regular duty that night.

9/5/06, EE was getting a panel off the truck, stepped back on concrete pad wrongly and felt a little strain, thought it would be ok. 9/6/06, let little sore but not bad. Was walking down to well site, stepped in wash in high grass, hurt it again, more pain.

EE was removing a driveline. He had problem separating the driveline from the flange on the transmission & put in a pusher bolt to assist.When the pusher bolt made contact, the driveshaft fell, smashing his fingers between the driveshaft & the shop floor. He was taken to local ER room where he rec'd stitches for lacerations.

A grass fire discovered at 7:45 p.m. on the mine. A powerline broke and fell to the ground, igniting the grass. The Fire was extinguished by Jewett Fire Department at 8:05 p.m. Some logs were smoldering but the active burning was out. The fire was reported at 8:16 p.m. because of the the smoldering logs.

3 employees were changing a 4 dragline bucket teeth. One employee was hammering the keeper when a piece of metal came off & lodged in injured ee's left thigh. Inj ee went to the dr's 8/14/06. The metal was embedded & had to be cut out. The incision required 2 stitches to close.

EE was turning the drive shaft using a one foot pry bar. The bar slipped & struck him in the face. He went to a local ER where he had 5 stitches to his left cheek.

Lifting & moving dewatering electrical panel, employee pulled or strained his arm. Didn't think it was hurt that bad, hurt worse the next day.

Employee, operating a coal hauler, had just left the backhoe after being loaded. She was turning to go up the ramp when the tires caught the mud, causing the hauler to jacknife. When the trailer hit the trailer stops, the force of the impact jarred her from side to side in the cab. She sustained two fractured ribs, bruising to her thumb, leg and ear. She was wearing a seat belt.

While dismounting End-Dump, she had a small cooler hanging from L. shoulder & was holding a lunchbox in her L. hand - the Rt. hand apparently lost its grip and employee fell backwards approx. 7'-0"" to ground, landing on her back area.

Operating an End Dump when the front tire hit a hole, which caused the steering wheel to jerk and spin around, which caught the right hand, causing the fracture.

Employee was operating an end dump, hit a bump in the road and felt pain in his back. He was treated by a doctor who placed restrictions on him for his work schedule of February 2-6, 2006.

The fire started from the result of a cloth tarp being used as a weather shield to prevent a compressor from freezing during cold weather. The tarp made contact with the muffler of a compressor and caught on fire. The wind blew the burning tarp off the well pad and into the grass causing the grass to ignite. The compressor is the property of Anadarko Petroleum Company.

EE WAS CLIMBING THROUGH A BULKHEAD FROM 1 COMPARTMENT TO ANOTHER IN THE REVOLVING FRAME OF DRAGLINE 28. HE TWISTED HIS BODY TO CLIMB THROUGH & HIS RIGHT FOOT ROLLED ON THE BAR AS HE CLIMBED THROUGH. HE HEARD A POP & FELT PAIN IN HIS LOWER RIGHT LEG. HE WAS TAKE TO A LOCAL EMERGENCY ROOM. X-RAYS REVEALED A FRACTURE IN HIS LOWER RIGHT LEG. HE IS CURRENTLY WORKING LIGHT DUTY.

Removed wheel chalk from bracket and when it came down, it caused something in arm to pull and felt a sharp pin.

The fire started in a reclamation field in Area D of the mine. The grass in the area was dry and approximatly 12 inches tall. No work was being performed in this area at the time of the fire. The fire covered an area 5.48 acres. There were no structures in the area and the fire presented no danger to mine employees. It is unknown at this time how this fire started.

EE was on top of a Komatsu 330M end dump differetial putting a pin in. When he came down off of the differential, he felt with his feet for the ladder which he had used to get up on the differential with. It was gonc and he slid down the differential and fell to the shop floor, falling on his buttock and lower back. He went to the emergency room on 10-21-05.

WHILE LOADING AN EMPTY WOODEN PALLET ONTO A TRAILER, THE EMPLOYEE TWISTED HIS BACK WHEN THE PALLET HUNG ON THE SIDE RAIL OF THE TRAILER. THE TRAILER AND PALLET WERE INSIDE ON A CONCRETE FLOOR. THE LIFT WAS APPROXIMATELY 18"" TO CLEAR THE RAILBUT THE EMPLOYEE FAILED TO WAIT FOR HELP TO LOADTHE PALLET RESULTING IN A STRAINED BACK MUSCLE.

AT APPROX 1345, THE FIRE WAS CONTAINED ON THE RIGHT-OF-WAY. A PIPELINE CLEARING CONTRACTOR HAD BEEN ATTENDING BRUSH PILE FIRES ON THE RIGHT-OF-WAY WHEN THE FIRES GOT OUT OF CONTROL.

ONE AREA OF 56.7 ACRES AND A SECOND AREA OF 6 ACRES WERE BURNED ON MINE PROPERTY. THE FIRES APPEAR TO HAVE STARTED ON THE HIGHWAY RIGHT-OF-WAY AND BURNED INTO THE MINE. ORIGIN OF FIRES IS STILL UNKNOWN.

EE WAS GOUGING KEEPERS OFF OF A DRAGLINE BUCKET. A BALL OF HOT METAL GOT INTO A CREASE OF HIS JEANS, BURNED THROUGH AND FELL INTO HIS BOOT, BURNING HIS LEFT FOOT. THE BURN GOT INFECTED AND HE WAS TREATED FOR THE INFECTION.
