Employee was attempting to clean material that had built up between the head roller and the chute on the no. 11 belt with a roof bolt when the roof bolt got up against the belt thus causing an avulsion of the left index finger.
Employee had just placed rub rail onto frame of continuous miner & needed to move the cover plate in order to permit the pin to pass through the rub rail to secure to the miner, when the rail fell off the frame of the miner onto the top of employee's feet.
Employee twisted lower back while setting fibercribs. Employee did not miss any work however after seeking treatment for ongoing pain the physician placed him on restricted duty effective 12/12/2007 now making this report classified as a medical.

WHILE EE WAS INSTALLING BELT STRUCTURE EE EXPERIENCED PAIN IN RIGHT SHOULDER. EE CON'T WORKING UNTIL 3/1/05 AT WHICH TIME HE WENT OFF TO HAVE SURGERY PERFORMED
EMPLOYEE WAS CHANGING OUT MINER BITS WHEN A SPRING CLIP THAT ATTACHES THE BIT TO THE BLOCK SPRANG BACK, STRIKING THE EMPLOYEE IN LEFT EYE. EE WENT TO THE HOSPITAL & WAS TREATED FOR A CORNEA ABRASION THUS MAKING THIS A MEDICAL.
EE, WHILE SPREADING HIGH CALCIUM LIME, APPARENTLY GOT SOME ON THE KNEE AREA OF CLOTHING NOT REALIZING THAT. LATER ON DURING THE SHIFT HE GOT HIS KNEES WET WHICH CAUSED A CHEMICAL REACTION.

EMPLOYEE WAS CHANGING BATTERIES ON THE SCOOP IN THE CHARGING STATION WHEN THE PLUGS APPARENTLY STUCK. WHILE TUGGING ON PLUGS THE ONE PLUG SUDDENLY DISLODGED THUS STRIKING HIM ON UPPER LIP.
WHILE EMPLOYEE WAS LOOKING FOR AN OIL LEAK ON A ROOF BOLTER HE EXPOSED HIS LEFT HAND IN A PINCH POINT THEN OPERATED FUNCTIONS PINCHING LEFT MIDDLE FINGER.
EMPLOYEE WAS PUTTING BOLTER SUPPIES ONTO THE BOLTER WHEN HE EXPERIENCED PAIN IN LOWER BACK.
