Employee was pulling on chain to move hoist. The employee encountered heavy resistance while pulling chain, causing left shoulder strain. Employee has continued to work with no restrictions. He had his 3rd physical therapy treatment on 10/22/2014 making this a reportable accident.

Employee tripped over a floor mat and bumped into machinery guard. Employee continued to work full time unrestricted. In January of 2015 employee sought physical therapy treatment. On 1-15-2015 employee had 3rd physical therapy appoint, making this accident report-able.

While moving a piece of pipe the employee felt a pull in his shoulder. The employee saw a doctor and received a prescription for Physical Therapy. The 1st PT session was on 8-19-14. His 3rd PT session was on 8-26-14. This accident is now report able. The employee has continued to work full time, full duty.

Employees was removing wire ties that were wrapped around a conduit with a knife.When the knife cut through the wire tie the employee's hand pulled back and he cut the thumb on his left hand. The thumb was cut by a sharp edge on the next wire tie.

EE was removing a full five gallon bottle of drinking water from it's container inside the warehouse. As he was lifting the bottle it slipped out of his hands. EE then tried to keep the bottle from falling to the floor injuring his lower back.

Injured and another employee were carrying 48" troughing idler up an outside conveyor belt, in order to replace an idler. Injured experienced lower back pain. **May 28, 2010 was his first day off work.**

Employee lifted one-half of the drive sprocket from a magnetic separator drum to move it. The magnet inside of the drum pulled the sprocket half to it, catching his right hand between the drum and the sprocket. Employee received a laceration of the right thumb requiring sutures, and contusions of the right index finger.

Employee was replacing a pipe under a head tank, while working, he jammed his head into the bottom of the tank injuring his neck. The employee originally refused any further medical treatment until he saw a doctor on 11/12/2008. To date he has completed three physical therapy treatments.

Employee was assisting in the replacement of a sieve bend screen. While removing the old screen, employee was struck by the old sieve screen and screen holder in the head, neck, and back, causing bruises and sprains.

Employee was opening the plant elevator door on the 3rd floor by lifting the top-half of the door with his arms and pushing down on the lower half of the door with his foot. The door jammed, (got tight and hard to open), and the employee felt pain in his lower back. He has been diagnosed with a back strain.

EE WAS SUPERVISING 2 EMPLOYEES DISASSEMBLE THE FEED PIPE TO 50750 FINE COAL CENTRIFUGE IN ORDER TO UNPLUG IT. THE PIPE IS CONSTRUCTED OF URETHANE & HELD TOGETHER WITH SLIP FIT COLLARS & CLAMPS. THE PIPE WAS BEING TAKEN APART AT A COLLAR. THE BLOCKED SECTION PULLED OUT OF THE COLLAR AFTER THE CLAMP WAS REMOVED. THE PIPE FELL STRIKING EE ON HIS RIGHT SHOULDER.
