Utilityman was helping in a routine cage change work. After the cage was changed the repairmen were tensioning the V-belts. In order to put even tension on all the belts the 18" cage paktor sheave has to be turned manually. During this procedure the injured employee got his hand caught betweent the sheave and v-belt.

Showing all 5Utilityman positioned a large, heavy pipe, which was rigged with a lifting device. Utilityman lifted the pipe on one end to get it to roll on a roller stand so he could get the pipe in position.
OPERATOR WAS CLOSING THE LID TO THE OXYGEN/ACETYLENE CART WHEN HE GOT HIS FINGER BETWEEN THE TOP OF THE CART AND THE LID. THE LID CLOSED CAUSING A PINCH POINT AND THE LEFT RING FINGER TOP WAS SEVERED.
EMPLOYEE WAS DESCENDING A FIXED LADDER. HE GOT HIS FOOT LODGED BETWEEN THE LADDER RUNG AND A STRUCTURE BEAM AND SPRAINED HIS RIGHT FOOR. HE CONTINUED WORKING THRU 8-6-01 WHEN HE SOUGHT TREATME NT FOR A SORE FOOT. DOCTOR TOLD EE TO STAY OFF FOOT FOR SEVERAL DAYS.
EE WAS STANDING ON A STAIRWAY AFTER COMPLETING WORK ON A CONVEYOR BELT. HE BENT OVER TO RETRIEVE HIS TOOLBAG AND FEL A PINCH/PULL IN HIS LOWER BACK. HE COMPLETED THE SHIFT AND RETURNED TO WORK THE FOLLOWING DAY. DUE TO SEVERED BACK PAIN HE WAS SENT TO THE DOCTOR.
