While transporting the smaller cage to the shop the hoist encountered a tight area. The repairman helper accidently positioned himself between the cage and a dust collector pipe. The helper tried to get out of the way but backed into the pipe and his arm was caught in a pinch.

Utilityman was helping in a routine cage change work. After the cage was changed the repairmen were tensioning the V-belts. In order to put even tension on all the belts the 18" cage paktor sheave has to be turned manually. During this procedure the injured employee got his hand caught betweent the sheave and v-belt.

Utilityman positioned a large, heavy pipe, which was rigged with a lifting device. Utilityman lifted the pipe on one end to get it to roll on a roller stand so he could get the pipe in position.

SUPERVISOR STATED EMPLOYEE COMPLAINED OF A SORE NECK AT START OF SHIFT. EMPLOYEE WAS ASSIGNED TO TAKE A COVER OFF A PIECE OF EQUIPMENT. AS HE UNBOLTED FASTNERS HE FELT LIKE HE PULLED A MUSCLE DUE TO A BOLT THAT WAS TOO TIGHT. EMPLOYEE REPORTED IT TO SUPERVISOR AND HE WAS SENT TO THE HOSPITAL. DOCTOR DETERMINED IT TO BE A CERVICAL STRAIN AND ADVISED RETURN TO WORK 8/19/03.

DURING ROUTINE CAGE PAKTOR CHANGE OR MAINTENANCE THE OPERATOR IN CHARGE AND A HELPER WERE ASSIGNED TO WORK AREA. THE HELPER WAS REMOVING OR DISCONNECTING THE CRANE HOOK FROM THE CAGE LIFTING T OOL. THE OPERATOR ACCIDENTALLY LOWERED THE CRANE CAUSING THE HELPER TO GET PINCHED BETWEEN THUMB AND INDEX FINGER. HELPER PUT BANDAID ON HIS HAND AND WENT HOME FOR THE EVENING.

EMPLOYEE INJURED LEFT ELBOW WHILE ATTEMPTING TO REMOVE MOUNTING FASTENERS ON THE HYD. PUMP ON D11N CRAWLER. THE WRENCH SLIPPED OFF THE BOLT HEAD & RESULTED IN HIS ELBOW STRIKING EITHER THE COR NER OF THE REAR BELLY PAN OR THE CONVERTER.
