The employee was lowering a guard in to position and the guard got hung up while it was being lowered. The employee had hand positioned on the bottom section of the guard and when the guard freed up, it pinched the employee's right index finger, causing a hairline fracture to the finger.

At around 5:10pm, employee was re-installing a cover plate after inspecting the inside of a rock transfer chute between two conveyor belts. All the fasteners on the cover plate were removed except the safety chain. As the employee was installing the first fastener, the plate slipped, and EE reacted by trying to catch the fall with EE's right hand.

Employee was collecting a transmission oil sample on a front-end loader. The sample catch tube was attached to the pressure test port and when it opened the valve, transmission oil was sprayed into the employee's eyes, requiring them to be flushed and eye drops administered.

A mine maintenance employee was assisting in performing an annual service on the 2-belt hydraulic take-up. The employee placed their left ring finger in-between the two guards and pulled back on the guarding, this resulted in pinching of the employee's finger. The employee was wearing leather gloves at the time of the incident.

A miner was dumping trash dumpsters. The employee followed proper shutdown procedures of the forklift and tried to assist the dumpster by pushing up on the rear. When that didn't work the employee grabbed the dumpster from the side and pulled down, causing the dumpster to tip. The pin that holds the dumpster lid failed, causing the lid to fall on the employee's right hand.

Employee was preparing to weld a broken pick holder on the north crusher. Employee was descending a ladder into the crusher hopper while carrying a MIG welder feeder box. Employee lost balance and fell from ladder. Employee was evaluated and diagnosed with fractured left tibia and fibula.

Mechanic placed hand in a pinch point between cylinder & pick attachment. Pick was not supported & shifted, pinching mechanic's thumb. Pick was moved to release the mechanic's thumb. The thumb received a displacement fracture & two lacerations, one of which required 5 stitches. The other required 0 stitches. Mechanic was released with restrictions.

Employee was removing an elevator chain pin that was wedged between the idler and elevator casing. While pulling the pin, the employee placed right hand over the end of the pin. Another mechanic who was assisting in this job was jogging the elevator. The elevator rolled back wedging the pin against the floor and the employee's right hand.

Employee was climbing (ascending) vertical ladder on the South Dust Collector. Just before reaching the catwalk, employee felt a sharp pain in backside. When employee stood on the catwalk, legs were weak and employee felt like throwing up. Employee experienced a lumbar strain.

Employee was disconnecting and rolling up 4" flat discharge hose from a trash pump. As employee attempted to unlatch the QD fitting, the right hand was cut by the end of the band-it clamp tail causing a laceration on base of right thumb. Employee saw doctor and received 2 stitches.

Two employees were removing a drill steel from a storage rack. As they pulled the drill steel towards themselves, the other end came off the racking storage arm. This caused the up swinging end to pinch the injured employee's hand against the underside of the next level racking arm, causing a laceration/avulsion to left hand ring finger base knuckle.

Employee was dragging a hose from the storage tank to the water truck. The hose slipped from the employee's hand and as ee reached down to grab the hose right thumb came in contact with a sharp metal band clamp. The clamp caused a cut requiring 7 stitches on the right thumb.

Employee was operating Ag Plant loader near PC stock pile when he noticed a rock on the boom. He parked the Loader and then got out to remove the rock so it wouldn't end up in PC pile. As he was moving the rock he felt like he pulled a muscle in his abdomen/flank on his left side/torso.

Employee was locating a hydraulic leak on a D10 dozer which could not be seen by inspection. Machine was jogged to pressurize hydraulic hoses and find leak. Employee lifted a hydraulic line to separate it from another and unknowingly placed his thumb over the pin hole sized leak causing highly pressurized hydraulic fluid to inject through his glove into his thumb causing the injury

A mill mechanic was attempting to transfer a 220lb plate from saw horses to a service cart for transport. As the mechanic was sliding plate on to back of cart the saw horse toppled over transfering substantial weight to the employee's grip. The weight of the steel plate caused a strain of the left bicep. He was released to return to work with no restrictions.

Quarry supervisor was assisting another employee to lift a grease barrel onto the drag line platform. The barrel slipped from their grasp and pinched the supervisor's right, middle finger between the barrel and a ladder rung on the drag line. The supervisor fractured the tip of the right middle finger and an avulsion to the finger tip required four sutures to close.

Employee was removing the wheel hub on the #29 service truck. He gripped the front and rear of the hub. As the front hub cleared the axle housing in which it was supported, the weight suddenly shifted and the rear hub pinched his right middle finger between the hub and the studs.

The employee reached for a pry bar to help align the cylinder with the flange hole for placement of a pin. He took his eyes off his left hand which was on the side of the cylinder. His hand moved so his little finger was now resting on top of the cylinder. It is believed that the cylinder was bound up and it dislodged moving up approx. 1/4" pinching the employee's finger.

The employee was working on the locked out dryer bag-house rotary chain and sprocket. While attempting to manually move the chain up one link on the sprocket he caught the 2nd and 3rd digits of his left hand between the rotary sprocket and chain.

EE was positioning stands under a suspended rolled conveyor with a shaft thru the center. While positioning one of the stands, he pinched his thumb of his left hand between the top of the stand and one of the ends of the shaft. He was working with a crew to install a conveyor. He used too much force while lifting up stand towards rolled conveyor.

Boom was suspended in mine shaft and when employee was positioning the boom, the boom extension cylinder guard came loose, striking the employee on the hard hat. Employee is diagnosed with cervical-thoracic strain.

Employee was performing maintenance work on a 740 drill. He was leaning over the tracks of the drill, working on a hydraulic pump. Two bolts on the air tank were pushing into his belly and eventually caused pain in the employee's groin area. The employee was performing the task for 4 hours off and on.

Foreman was assisting a Mechanic with the removal of a transmission skid plate/Belly Pan from a Bulldozer. The Pan was rigged to prevent movement; as the last bolt was removed the Pan dropped and pinned the foreman?s right hand between the bulldozer?s track housing and the belly pan. He sustained 2 lacerations at the right wrist area which required sutures to close.

2 quarry mechanics were attempting to remove a house roller from the 4600 dragline. The roller is approx 15" across & 6" deep with a weight of 250 lbs The EE's were positioned on both sides and began to lift roller out of machine. Roller slipped and pinched inj EE's finger causing avulsion & fracture to finger.

Employee suffered fractured finger with two stitches. Injury occurred as result of a crusher part that fell onto the employee's finger during an attempt to move it. The accident was a result of miscommunication with a fellow employee, not using a hoist, and the employee putting his finger in a pinch point.

The employee was removing a bracket from the top of a shelving unit. As the retaining bolt was removed, the bracket fell on his left elbow. First aid was given & he did not feel further treatment was needed. On 12/21/06, the elbow started to bother him & he was taken to see the company physician to diagnose the elbow pain.

Production Operator was removing the rock hammer attachment from the Caterpillar Backhoe rental. He reached into the coupling area with his left arm to remove the retaining pin while the rock hammer was attached. He experienced scratches to his left bicep.

The Maintenance Crew was replacing the screw conveyor in the dryer feed hopper. At about 9:00 AM on Monday 11/14/05 employee felt a burning pain in his lower left back. He was taken to the company doctor where he diagnosed a lumbar strain.

While working at the bagging station (lifting 50-lb. mica bags), EE felt a warm sensation on his right groin area. EE did not think it was a big deal but around 9:30 am as he proceeded to climb the baghouse stairs, he felt the same warm sensation. EE then went to the restroom and noticed that he had some swelling on his right groin area. Diag.: hernia (surgery probable).

Employee was shoveling fines from floor of bin room into ore bin in the headframe. He felt his left shoulder pop, but initially refused medical treatment. The pain persisted, and at his request, he saw the company physician on 10/1/04. An A-C Joint strain was diagnosed.

The Mine Maintenance Supervisor was in the process of removing a chain that binds a double entry gate to the surface air shaft. As he was removing the chain, the back side of his right hand struck the razor wire directly above the gates. He received a small laceration. Employee received 3 sutures to close the wound, and a tetanus injection.
