Employee was operating a Cat D8R dozer when a rock became lodged between the trunion arm and the track. When the employee removed the rock, about 50 pound, he felt a strain in his lower right abdomen area. Employee was examined on 11/18/2009, confirming a hernia.

Employee was checking motor grounds in plant area. When reaching overhead he felt pain in his left shoulder. Employee did not seek medical evaluation until 11/13/2008. First medical treatment resulted after MRI testing. Employee received injections on 11/26/2008 and is working with restrictions.

Employee was servicing Cat 768B watertruck after completing engine repair. Antifreeze was required and being hand-carried to the truck radiator fill neck. A 2.5 gallon container of antifreeze was being lifted to the truck deck when employee felt a sharp pain in lower back. Doctor evaluation was muscle sprain and prescribed physical therapy and modified work duty.

Employee was relocating 15 # and 50 # packages of explosives in a drop-trailer type 5 magazine. Employee felt a strain in left shoulder and was first evaluated by a physician on 4/4/2007. Physicain confirmed strain and prescribed physical therapy.

EE was removing empty conveyor belt spool from the installation hanger. His left little finger was pinched between the hanger frame and the spool shaft. Medical treatment, including sutures, were required to close the laceration. The EE was wearing leather gloves at the time of the accident.

EE WAS REMOVING BOLT-ON CUTTING EDGE FROM BUCKET OF FEL. HE WAS USING BARE HANDS AND 1" IMPACT TO REMOVE THE PLOW BOLTS. HIS LEFT HAND WAS ON HEAD OF PLOW BOLT WHEN IT TURNED UNDER THE FORCE O F THE IMPACT. HIS LEFT INDEX FINGER SUSTAINED A LACERATION REQUIRING SUTURES.

EE WAS ADJUSTING FORKS ON SKIDSTEER LOADER FORK ATTACHMENT. HE WAS USING RIGHT HAND TO PIVOT ROD WHILE USING LEFT TO GUIDE FORK ALONG PIVOT ROD. FORK SLIPPED OUT OF RIGHT HAND AND PINCHED LEFT DISTAL RING FINGER AT ROTATING PINCH POINT. EMPLOYEES GLOVES WERE OIL SOAKED AT TIME OF ACCIDENT.

EQUIPMENT OPERATOR WAS DISMOUNTING THE CAT 245 HYDRAULIC SHOVEL AT THE END OF SHIFT. WHEN HE CLOSED CAB DOOR, HIS LEFT DISTAL FOURTH FINGER WAS PINCHED IN THE DOOR JAM. FINGER SUSTAINED LACERA TION/CRUSHING INJURY REQUIRING SEVEN SUTURES. NO LOST WORK DAYS.

EE WAS LOADING WHEEL BEARINGS FOR A 50-TON HAUL TRUCK INTO A PICK-UP TRUCK FOR DELIVERY TO THE PLANT. THE BEARINGS WEIGH APPROXIMATELY 50 TO 60POUNDS.
