Employee was operating a Cat D8R dozer when a rock became lodged between the trunion arm and the track. When the employee removed the rock, about 50 pound, he felt a strain in his lower right abdomen area. Employee was examined on 11/18/2009, confirming a hernia.
EE reported that he has been diagnosed with bilateral carpal tunnel syndrome. The doctor evaluation states ""with his work duties that this would be at least a work related condition or a condition that was aggravated or exacerbated by his work"". The EE is continueing to work until the first surgery sch. for 10/14/09.

Employee was checking motor grounds in plant area. When reaching overhead he felt pain in his left shoulder. Employee did not seek medical evaluation until 11/13/2008. First medical treatment resulted after MRI testing. Employee received injections on 11/26/2008 and is working with restrictions.
EE was completing liner repairs inside the primary horizontal impactor. He twisted at his waist while picking up a 1"" impact wrench. he felt a strain in his lower back. When reported to his next shift on 7/22/08 at 07:00, he was having muscle spasms and requested a chiropractor evaluation. He subsequently has received chiropractic treatments & medication.
Employee was exiting truck, stepped in hole and pulled ligaments in ankle.

Employee was servicing Cat 768B watertruck after completing engine repair. Antifreeze was required and being hand-carried to the truck radiator fill neck. A 2.5 gallon container of antifreeze was being lifted to the truck deck when employee felt a sharp pain in lower back. Doctor evaluation was muscle sprain and prescribed physical therapy and modified work duty.

The EE was welding the hopper perimeter while in a position kneeling at ground level. The area was well ventilated & several other EE's were working nearby. For an undetermined reason, the EE lost consciousness and hit his face on the structural steel he was welding. He regained awareness & climbed from the hopper on his own power. At the ER he received sutures in his upper lip.
EE WAS USING UTILITY KNIFE CUTTING CONVEYOR BELT. THE KNIFE WAS PULLED THROUGH THE BELT & PUNCTURED RIGHT LEG OF EMPLOYEE. MEDICAL EVAULATION REQUIRED SUTURES TO TREAT WOUND.

EMPLOYEE WAS USING SKID LOADER FOR PLANT SPILLAGE CLEAN-UP. THE NUISANCE DUST GOT INTO EMPLOYEE'S LEFT EYE AROUND PROTECTIVE EYEWEAR. ON 10/05/05, EE REQUIRED MEDICAL TREATMENT FOR AN INFECTION IN HIS LEFT EYE.
EMPLOYEE WAS INSPECTING EQUIPMENT ON READY-LINE PRIOR TO DAY SHIFT. HE SLIPPED ON PARKING BERM BUT DID NOT FALL TO GROUND. PHYSICIAN EVALUATION IDENTIFIED BACK STRAIN ON 08/18 AND PRESCRIBED THERAPY ON 08/24
EE WAS EXITING THE SEAT/CAB OF RIDIG FRAME HAUL TRUCK. AS HE WAS PUTTING WEIGHT ON RIGHT LEG, HE FELT RIGHT KNEE ""POP"". HE WAS DOCTOR EVALUATED ON 07/06/05 AND PRESCRIBED PHYSICAL THERAPY
Employee accessed an elevated platform to energize an elevated electrical panel. After energizing the switchgear, he jumped off the elevated platform to the MCC concrete floor. The elevation was approximately five feet. Employee sprained lumbar.

EE WAS OPENING A MANUAL, SEGMENTED, OVERHEAD 16'X 24' DOOR ON SHOP ACCESS. THE EE COMPLAINED OF BACK & NECK PAIN. DR. EVALUATION INDICATED MUSCLE STRAIN & TREATED WITH HOT/COLD COMPRESS & PRE- SCRIBED MEDICATION.
MAINTENANCE EE WAS COMPLETING BEARING REPAIR ON A 3 DECK INCLINE SCREEN. HE WAS USING OEM THREADED ROD PULLER INSTALLING THE COUNTERWEIGHT SHEAVE ONTO THE SHAFT. WHILE PULLING THE WRENCH TIGHT ENING THE PULLER, HE STRAINED HIS LOWER BACK, PHYSICIAN TREATED ON 8/25/03 WITH HOT/COLD COMPRESSES & MEDICATION.
OPERATING FRONT END LOADER AT SHOT ROCK PRODUCTION FACE. REPETATIVE MOTION IN OPERATOR'S SEAT INDUCED MUSCLE STRAIN OR BRUISE RESULTING IN LOWER BACK PAIN. EMPLOYEE TREATED WITH PRESCRIPTION M EDICATION & HOT/COLD COMPRESSES BEGINNING 7/16.
MAINTENANCE EMPLOYEE WAS CROSS CUTTING CONVEYER BELT FOR A MECHANICAL BUTT SPLICE. UTILITY KNIFE SLIPPED FROM BELT PUNCTURING RIGHT LEG THIGH ONE INCH DEEP. EE RECEIVED 3 SUTURES AND A TETANUS SHOT.

THE EUCLID 74TD 35 TON HAUL TRUCK WAS BLOCKED, LOCKED AND TAGGED OUT FOR REPAIR. A WIRING FAULT WAS BEING TRACED AND ACCESS INTO AND UNDER THE CAB WAS REQUIRED. EE WAS INSPECTING THE WIRING UN DER THE CAB WHILE STANDING ON THE EXTENSION LADDER, HIS FOOR SLIPPED FROM THE LADDER RUNG. HE FELL APPROX 4 FEET TO GROUND LEVEL. THE INJURY WAS INITIALLY NON-REPORTABLE; HOWEVERSUBSEQUENT MEDICAL EXAMINATION HAS REVEALED THE NEED FOR SURGERY AND LOST WORK DAYS. A TEAMSTER WAS DELIVERING DRINKING WATER TO VARIOUS WORK AR EAS AT THE MINE. WHILE CARRYING A 5 GAL. KEG TO HIS PICK-UP TRUCK, HE STEPPED OFF THE EDGE OF A CONCRETE PAD. THIS MIS-STEP CAUSED HIM TO TWIST HIS LEFT KNEE. SURGERY SCHEDULED FOR 12/14/00.
EE PERFORMED GENERAL CLEAN UP DUTIES IN THE PULVERIZER UNTIL 1:00 PM. AT THIS TIME THE CHAIN ON THE DRIVE SPROCKET ON THE #3 CONVEYOR HEAD PULLEY JUMPED OUT OF THE SPROCKET TEETH, CAUSING THE CONVEYOR, & SUBSEQUENTLY THE ENTIRE SECONDARY PLANT, TO STOP OPERATING.
