EE was pulling extra hose into a coarse material washer to wash it out for maintenance purposes and ran EE's right ring finger across one of the flights lacerating the fingertip. EE received 3 sutures making this accident reportable to MSHA.

EE was helping flip a stuck Rip Rap flop gate, once the gate freed up it fell onto EE's upper left leg. EE initially said was ok but came to work the next morning with leg swollen. EE was taken to urgent care and diagnosed with a severe bruise and given restrictions against prolonged standing, climbing, etc. thus making this accident reportable.

EE was changing a steering cylinder on a haul truck. The cylinder slipped off the ear it fastens to and mashed the EE's left middle and ring fingers between the ear and cylinder lacerating the middle finger requiring stitches, thus making this accident reportable to MSHA.

EE was attempting to put a sheave on a pulley shaft that had been cut, the EE cut EE's left middle finger on a bur on the shaft. EE was taken to the urgent care where EE received stitches in EE's finger, making this reportable to MSHA.

EE was loading a hydraulic cylinder and was holding by the head cap, when EE dropped the cylinder to the floor the barrel collapsed mashing EE's fingers in between the eye and head cap. This resulted in broken fingers on both hands. EE has splints on 3 different fingers making this accident reportable.

EE was trying to diagnose an issue with the detach pins on a skid steer bucket. EE had the bucket raised 24" and when the pins released the bucket fell on EE's left foot fracturing the 3rd foot bone, making this accident reportable.

EE was helping to change a return roller, claims they hurt a shoulder stating they didn't know they hurt it but that was the only task that could've hurt it. After some discussion we have decided to file a worker's comp claim until further investigation. EE has missed work because of this making it a lost time accident and reportable to MSHA.

EE was helping change wear parts on a piece of plant equipment. While handing parts to another employee, EE over extended and felt discomfort in their back. On Tuesday August 16th EE went to the doctor and was prescribed rest and written out till Friday, August 19th making it reportable.

EE was removing a guard from the No. 2 conveyor. The guard would not come loose. When the guard finally came loose, it pinched the EE's thumb causing a fracture. The EE did not feel the need to go to the doctor until 11/18/2021.

EE was assisting installing impeller in a GIW pump. The impeller was sitting upright between the forks on the forklift. The impeller shifted a little forward on the forks, and the EE got in front of the forks to reposition the impeller. The impeller fell forward off the forks striking the EE in the right ankle and foot.

EE was lifting a bucket of ABC with EE's right hand and placed it on a hand truck. EE felt a sharp pain in EE's sternum. This was on Friday. On Monday, it was not any better, so EE went to the doctor and was diagnosed with a muscle strain and placed on light duty, thus making the incident reportable.

The EE was helping to change a return roller on the No. 1 belt. Another EE was holding one side, and the roller slipped out of EE's hands. The roller came down and hit the injured EE's hand, causing a fracture on the right hand.

EE was working on a side door on a step chute at the wash plant. The door hung up going down. Tried tapping on it to get it moving. The plate turned loose, slid down and hit the ring finger and index finger of the left hand, requiring stitches.

EEs were lowering jaw die into chute for wear purposes when the die got hung on the walls. They cut one side of the die off, the die fell, and the chain hit the EE in the right knee. The EE has a fractured patella on the right leg, causing the injury to be reportable.

EE was helping to change pulley on No.1 belt. Bearing on the pulley was stuck. The EE thought that they could free the bearing up with their hands. When they freed the bearing up, the pulley and bearing came free and pinched EE's fingers between conveyor frame. EE had to get Stitches in fingers, making accident reportable.

EE was helping to change a media panel in the #4 screen. The EE was squatted down and pushing it in place. The EE either twisted or turned wrong and could not move. EE felt a pain on the left side. The pain got worse and EE had to go to the doctor. The EE has missed time from work. They think it is a severely pulled muscle, but no definite diagnosis yet.

EE was putting v-belts on a crusher motor. As ee was rolling the sheave over, ee left hand glove got caught and pulled ee left wrist into the sheave and v-belt. The EE felt a lot of pain, went to the doctor, but had no fractured bones. Doctor restricted duty for 4 working days, thus making the accident reportable.

The EE was picking up a water hose at the plant to use to wash down. When ee picked it up, the hose clamp on the hose had a sharp edge. EE pulled the hose clamp across right palm, and the clamp lacerated palm. EE went to the doctor and received two stitches.

EE was changing bit on Gill Drill. The bit basket was stuck to the bit. When EE unscrewed bit, the bit and basket pinched left little finger. EE was wearing gloves, but the pinch lacerated left little finger requiring 3 stitches.

On Saturday, 12/5/15, EE complained of pain to his right hand and swelling. He was unsure as to cause. The doctors found metal in his hand on 12/13/15 from where he had cleaned out hydraulic tank on 12/4/15, which caused infection and hospitalization, thereby making it a reportable accident. Until 12/13/15, the doctors thought it could be a spider bite or boil under the skin.

EE was assisting in removing rebuilt screen from shop. While pulling screen from shop, back of screen got stuck on nut welded to the rail on floor. They put a piece of angle under screen to lift up on it, when screen came loose from the nut and angle hit ee in left foot causing a severe bruise. Doctor restricted duty, therefore reportable.

On 3/5/2014, the EE was attempting to pull two 5-gallon buckets apart when he felt a sharp pain in his lower back, hip, side and leg. In the last couple of days, the EE felt a bulge in his right lower abdomen, with some pain. He was sent to the doctor and diagnosed with a hernia and placed on restrictions on 4/24/2014, thereby making it reportable.

EE was working in the shop. He bent over to pick up a wheel chock and felt a pull in his back. Went to doctor and diagnosed with strain in right upper back. Doctor restricted duty until 8/5/13 (follow-up appointment), which made the accident reportable.

EE was welding in a chute. He finished with one welding rod and attempted to throw it onto a shelf close to him. When he threw it, it bounced off a plate and came back into his left shoe burning his leg. Accident occurred on 7/11/13, but EE did not feel need to go to doctor until 7/23/13, which is when we got diagnosis of 3rd degree burns, making it reportable.

EE was hooking up a welding lead on the welding truck. He looked down to step off of the concrete where his truck was parked, and then looked up as he was stepping. As he looked up, he ran into a piece of angle iron that was placed on the truck that they planned to use on the job they were working on. The angle iron hit him on the left side of his face, requiring 8 stitches.

EE was placing concrete forms on a pallet when one 2'x4' section slipped and hit his left leg. Doctor initially said was just bruised and to come back in one week. The next day the EE's leg was severely swollen and went to ER. Doctor said he had severed blood vessel in leg. Had to do surgery on 7/19/2012 to relieve pressure. EE will be out-of-work for 6-8 weeks.

The ee was pulling a drop cord off of the top of the screen after performing maintenance. When the end of the drop cord came over the edge of the screen, it hit the ee in the mouth, cutting his lip and chipping a tooth. The cut was minor, but the tooth had to be repaired by placing a cap on it.

EE was helping to install new guard on a conveyor drive. While in the seated position, the EE felt a pain in his groin area. He stood up and felt a knot in his right groin area. Went to the doctor and had a hernia. The doctor restricted activity for 10 days to no lifting over 10 lbs.

EE was helping raise up guard rails on scales at the load out. One EE picked up their end of pipe and when the other EE picked up their end, it slipped out of their hands. The pipe fell and struck the EE in the right knee, requiring 8 stitches.

THE EE WAS TRANSPORTING THE JACK SHAFT FROM THE TRUCK TO THE DREDGE. HE SET THE SHAFT ON THE HAND RAIL TO TRANSFER IT TO THE BOAT. HIS FINGER WAS PINCHED BETWEEN THE SHAFT AND THE HAND RAIL, CUTTING HIS FINGER.
