EE was cutting a piece of conveyor belting with a utility knife when the knife slipped stabbing the EE in the right thigh just above the knee. The EE received 3 stitches making this accident reportable to MSHA.

Struck against stationary object

EE was sliding hand down a wooden bedrail of a truck when EE got a large splinter under right thumb nail. EE could not get the splinter out and was taken to get it removed making this MSHA reportable.

EE was walking around a pallet when EE tripped on the corner of it. EE left ankle started hurting, EE reported it to EE's supervisor. EE said EE thought it would be ok when asked. EE came to work on 1/27/2023 and ankle wasn't any better, EE was instructed to go to urgent care and have it looked at.

EE was walking out of the surge tunnel as another EE had thrown a rock into the tunnel. The rock that was thrown hit the EE in the mouth causing the EE a laceration to the lip requiring stitches making the accident reportable.

EE was removing a guard from the No. 2 conveyor. The guard would not come loose. When the guard finally came loose, it pinched the EE's thumb causing a fracture. The EE did not feel the need to go to the doctor until 11/18/2021.

EE was tightening the packing on a slurry pump. The wrench slipped causing EE's hand to hit the main frame of the pump. The top of the little finger on the right hand was cut, requiring 2 stitches.

EE was standing on the tracks of the trackhoe and heard a pop around right knee. After that EE felt pain for the remainder of the day. There was no action that caused the pain (i.e. falling, stepping, stumbling, tripping). The ee was merely standing and talking and felt/heard the pop. EE has been written out of work.

EE was pulling the gate closed at the entrance at the end of the day. EE was pulling the gate behind self and the gate caught the back of EE's foot, causing EE to fall backwards. EE's foot swelled up and EE ended up having a small fracture in the foot.

Over-exertion in lifting objects

The EE was helping to take batteries out of a haul truck. Employee got the batteries out, twisted to put the batteries on a truck and felt a pain in the lower back. Employee was taken to the hospital. Employee suffered a lower back strain.

Struck against a moving object

EE was operating forklift repairing roads after Hurricane Michael. EE hit a small pothole/sinkhole in the road that they backed away from, and said their neck hurt. EE went to the doctor to get checked out. The doctor said nothing was wrong with them, but wrote them out of work during the next shift due to soreness. This makes the accident reportable.

On 09/17/2018, The superintendent came into the plant after Hurricane Florence had subsided. EE noticed that the Railroad pond impoundment had breached. This was at 6:57 am. We immediately called the MSHA hotline and reported the failure. There were no injuries due to the failure of the impoundment.

EE was cutting a piece of thick skirt rubber with a box cutter. The box cutter slipped and cut thumb on ee left finger, requiring three stitches, thus making the accident reportable.

Over-exertion in pulling or pushing objects

EE was helping to change a media panel in the #4 screen. The EE was squatted down and pushing it in place. The EE either twisted or turned wrong and could not move. EE felt a pain on the left side. The pain got worse and EE had to go to the doctor. The EE has missed time from work. They think it is a severely pulled muscle, but no definite diagnosis yet.

Struck by falling object

EE was helping to change take up pulley. Hoist broke and pulley/EE fell. Pulley hit the EE in the right hand/thigh, and across the chest. EE was taken to the doctor and released, no restrictions. On 6/8/17, EE came to work and complained of chest hurting, went back to doctor, and doctor wrote EE out of work until 6/12/17. EE missed 6/9/17, thus making accident reportable.

EE was assisting in removing rebuilt screen from shop. While pulling screen from shop, back of screen got stuck on nut welded to the rail on floor. They put a piece of angle under screen to lift up on it, when screen came loose from the nut and angle hit ee in left foot causing a severe bruise. Doctor restricted duty, therefore reportable.

EE was hammering some rust off of some metal in a bin in which he was getting ready to repair. While hammering, some type of debris got in his eye (he was wearing safety glasses), and extremely irritated left eye. Went to doctor, but there was no debris in eye, just irritated. Doctor gave prescription ointment for eye, thereby making it reportable.

EE was placing concrete forms on a pallet when one 2'x4' section slipped and hit his left leg. Doctor initially said was just bruised and to come back in one week. The next day the EE's leg was severely swollen and went to ER. Doctor said he had severed blood vessel in leg. Had to do surgery on 7/19/2012 to relieve pressure. EE will be out-of-work for 6-8 weeks.

Struck by falling object

EE was changing oxygen bottle on maintenance truck. EE stumbled, fell and dropped the oxygen bottle on left leg. At that time EE said he was fine. Due to swelling and pain, the EE decided to go to the doctor on 9/14/2010, the doctor wrote him out of work due to the condition, thereby making the accident reportable. The EE did not miss any work until 9/15/2010.

EE was adjusting valves on the 111M Marion Dragline and removed his hard hat to look at the dial indicator. He raised his head and struck a bracket above, causing a laceration approximately one inch in length on the top of his head. The laceration required 5 stitches, thereby making this accident reportable.

EE was removing bolt from Sheave while in a vice. The EE pulled on the bolt with a wrench, the sheave slipped in the vice and pinched the EE's left small finger between the sheave and the vice.

EE was walking through the office when she caught her foot on a rug on the floor. As she tripped, she hit just above her right eye on the door facing, causing a laceration requiring three stitches.

Fall from machine

While checking the oil level in the waste oil tank, the employee stepped onto an engine holder stored under the tank. When he stepped up with his right foot, he slipped, hitting his left arm on the tank and then catching himself with the same hand on the way to the ground.

WHILE CHECKING THE CLUTCH ON TRANSMISSION AFTER REMOVING THE INSPECTION PLATE, THE EMPLOYEE LOOKED INTO THE CLUTCH & DEBRIS BLEW INTO HIS LEFT EYE.

WHILE TRAVELING TO THE PIT TO BE LOADED, THE DRIVER OPENED THE WINDOW; DUST BLEW IN HIS EYES. THE EUCLID TURNED TO THE RIGHT AND DROPPED INTO THE DRAIN DITCH CAUSING HIS RIGHT SHOULDER TO HIT THE RIGHT SIDE OF CAB AND WINDOW.

The employee was cutting a bolt off of #4 screen. The torch was leaking at a loose fitting. The fitting ignited burning his right wrist. The injury was not reported until 3-21-05.

THE EE WAS TRANSPORTING THE JACK SHAFT FROM THE TRUCK TO THE DREDGE. HE SET THE SHAFT ON THE HAND RAIL TO TRANSFER IT TO THE BOAT. HIS FINGER WAS PINCHED BETWEEN THE SHAFT AND THE HAND RAIL, CUTTING HIS FINGER.

WHILE SETTING BLOCKS INTO POSITION WITH CRANE, THE BLOCK BEGAN TO SWING AWAY. THE EE TRIED TO STOP THE BLOCK AND INJURED HIS LOWER BACK. THE EE CONTINUED TO WORK AND REPORTED THE INJURY ON 10- 2-03 TO THE SUPERINTENDENT. THE EE WAS SENT TO THE DOCTOR ON 10-03-03 AND RETURNED TO WORK.

WHILE STEAM CLEANING THE D-7 DOZER, THE STEAM CLEANING HOSE BECAME ENTANGLED ON THE DOZER BLADE, CAUSING THE EE TO LOSE CONTROL OF THE WAND AND SPRAYING HIS RIGHT FOOT AT THE BOOT LACES, HOT W ATER ENTERED HIS BOOT BURNING HIS ANKLES.
