Showing 50 of 88Employee was discarding a bag of garbage that contained a peace of metal siding. When he attempted to throw bag into the dumpster it reflected off the side of the dumpster and bounced back. The piece of metal that was in the bag stuck through, cutting the employee on left bicep.
Employee was climbing down the ladder of the water truck. His finger got caught on the bracing of the ladder, lacerating his pinky finger on the outside of his right hand. Employee received stitches from local emergency room.
Employee was helping with changing impact crusher hammers. He got his left little finger smashed between the hammer wedges. His injury required stitches.
Employee was handling a rubber hose which places concrete. Rubber hose was being cleaned out after use. The rubber hose "flopped" around when being cleaned, striking employee in left inner arm (bicep area). Causing a contusion.
While socketing cable clive pin it suddenly went free into place causing partial severing of fingertip (right ring fingertip).
Employee manually handling a 8 foot long piece of channel iron which "rolled" up onto his right ring finger causing a fracture to finger.

Lower back strain when lifting a box of paper. Went to the doctor and was release for duty. Had follow up visit on 10-18-06 and was prescribed 12 session of physical therapy.
Employee was releasing water hose from water truck to wash out bed of customer's truck. Employee was unreeling hose when his left index finger got caught in the reel. Employee suffered laceration to index and had to have stitches. Employee was wearing PPE (gloves).
EE was helping put in mainframe bolt on crusher. Another ee dropped the bolt in place. When the bolt dropped a burr on the bolt caught EE's thumb and lacerated it. He received 11 stitches.
Picking up a box of orange boosters, felt something pop in left side of back.
EE was installing under table plates. The plate was bumped by another employee and fell in the mill through the beam opening. It struck EE knocking him down. EE got back to feet, came out of small door and was going down stairs. Another employee made him lay down to check injuries, rescue was called.
Employee was removing lower thrust plate from cone crusher when it slipped and caught his finger between the plate and the crusher mainframe. Swelling of right index finger.
Employee was climbing back into sweeper truck on 4-18-06. He felt a pain in his left shoulder. He notified his supervisor & did not want to fill out incident report & did not want medical treatment. On 4-24-06 @ 6 AM he went to his supervisor & reported pain. Then requested medical attention.
Employee was unplugging chute on Hazemag crusher when a rock fell down on right hand causing laceration
Pulled lower back while opening lower gate.
EE was doing maintenance on mantle. Mantle dropped suddenly, causing EE's body to twist and pull, as he was startled by the movement of the mantle. Initially EE was fine, but as time went by, he decided to seek medical treatment several days later.
While moving a file cabinet into an office, bottom drawer flew open and lacerated employee's left forearm.
Cement Dust
Employee was cleaning cat walk over river using a 5 gal bucket to carry rock to shore, approx 100' then throwing it over the top rail to the ground below. First pain in his back when dumping bucket and called on the radio for help.
Employee was hooking up 1000 gallon fuel tank to land leveler. Smashed his 5th finger, left hand and cut his ring finger.
Employee was changing out belts on a conveyor drive motor, when the metal railing he had the come-along attached to broke, causing his finger to be pinched between the belt and pulley. The injury required stitiches.
Ee had just pulled cable up barge. cable was tight under cleat. Cable popped out from under cleat. Hitting him in the right upper mid leg.
Employee was installing an air canon on the South Docksider North vessel with witness. A piece of pipe with flanges on both ends was threaded to the discharge valve of the air canon. When he pulled on the 40 lb pipe it came out of the threads and fell 4 ft onto his right foot. The pipe landed right where the end of the steel cap ends on his shoe and crushed his big toe.
WHILE CLOSING THE VALVE ON PORTABLE PROPANE TORCH, PALM OF HAND SLIPPED ON ROUGH EDGE OF NOZZLE, CAUSING LACERATION.
Block moved unexpectedly while greasing. Caused bruises to upper right side of body and left knee
Employee was shoveling under a hopper. It was a tight area and she used bad body position. She pulled a back muscle.
EMPLOYEE WAS SLIDING A BOARD ACCROSS CRUSHER, BOARD SLIPPED OFF OPPOSITE SIDE CAUSING THUMB TO BE PINCHED BETWEEN HYD. CYLINDER AND BOARD.
Cat 730 truck was stuck in mud on uneven ground. Operator had assisted in hooking up tow cables to haul truck. As operator was climbing back into the cab, the door slammed shut on his right thumb. Thumb had slight bruise. Operator was given medication for pain causing restricted duty for two days.
While lifting a 94 lb. bag of cement to stack on a cement pallet, employee felt a pop in his left shoulder.
Employee was lowering extension ladder into empty barge. Lower portion of ladder suddenly extended all the way down to bottom of barge. This causing employee's left hand to twist and causing injury mainly to his left thumb.
Employee was working in area 1. He was pulling on 6" punch plate to attach chains on the crane. His left thumb was caught between the plate he was working on and the next plate up.
WHILE UNLOADING MILL SCALE AT RAIL UNLOADING, EE CLIMBED INTO CAR TO REMOVE MATERIAL THAT WAS BRIDGED ON THE WALLS OF THE RAIL CAR. WHILE ENTERING RAIL CAR EE STEPPED INTO HOLE IN MATERIAL CAUSING TO FALL FOWARD
VITIM WAS WEARING PROTECTIVE GEAR WHEN RESOCKETING A DRAG CABLE FOR A BE 1250W DRAGLINE. HE HELD ON TO THE CABLE TOO LONG POSITIONONG HIS MIDDLE FINGER AGAINST THE DRAG CHAIN. THIS RESULTED IN A FRACTURE AT THE FIRST KNUCKLE. EMPLOYEE HAD PREVIOUSLY SERVED NOTICE AND THIS WAS HIS LAST DAY OF EMPLOYMENT FOR THE COMPANY.
Employee was checking rock shaker to see if it was locked down. The counterweight was stopped on the topside, and when the employee moved the sheave, the counterweight rolled to the bottom, catching his hand between the belt and the sheave. This caused lacerations to the right index and second fingers.
While putting belts on the coal mill fan, employee was rolling the belts on when his glove got hung between the belt and sheave at which time the tip of his left middle finger was pinched causing him to snatch his hand out tearing the tip of his finger off above the fingernail.
WHILE ASSISTING MAINTENANCE PERSONEL IN ADJUSTING A GEAR BOX, EE PLACED HANDS UNDER THE HOUSING OF THE BOX. THE BEAR BOX SLID DOWN THE ADJUSTING TRACK AND PINCHED INDEX FINGER ON RIGHT HAND CAUSING A LACERATION. EE WAS ADVISED NOT TO PLACE HANDS IN THIS POSITION.
EE was throwing a piece of tri braze off of screen tower onto ground when his finger along with the gloves he was wearing caught the steel.
While opening watergate valve his right index finger got caught in between packing adjusting screw and handle.
Windshield wiper traveled off glass area during heavy rain storm & he attempted to move it back.
While holding the hyrdraulic control unit lid, which was propped with a piece of steel, was cleaning debris away when the prop slid off bottom and it came down on his right hand. This resulted in severe laceration of right thumb and small finger.
Worker was standing under scalper conveyer platform, removed his hard hat to wipe sweat from his forehead and worker on platform kicked a rock off the platform while walking, the rock struck worker's head causing laceration requiring stitches.
ALLEGED SHOULDER INJURY WHILE WORKING IN CONGESTED AREA UNDER INCLINED SCREEN.
8 days after helping other ee's work on #4 screen support tubes he complained of soreness in left shoulder.
EE WAS ATTEMPTING TO CATCH SCREENS AS THEY WERE BEING TOSSED BELOW TO LOWER DECK
While moving boxes of parts in the shop employee strained his shoulder. Note: This event was treated as a plant first aid until surgery was diagnosed (with restrictions) on 7/2/04.
EE'S FINGER WAS CUT WHILE OFF LOADING SCREENS.
WHILE SETTING MOTOR WEIGHT MOTOR SHIFTED CAUSING EE TO LOSE BALANCE. WHILE TRYING TO MAINTAIN HIS BALANCE HAND WAS PINCHED BETWEEN MOTOR AND SHIVE CAUSING CUT TO PALM OF RIGHT HAND.
EMPLOYEE WAS MOVING OXYGEN CYLINDER, FELT MINOR PAIN IN KNEE. INJURY WAS INITIALLY DOCTOR FIRST AID ONLY. EMPLOYEE CONTINUED TO HAVE PROBLEMS WITH KNEE RESULTING IN FOLLOW-UP TREATMENT. AS A R ESULT, INJURY WAS RECLASSIFIED AS A RESTRICTED DUTY INJURY, EFFECTIVE FEBRUARY 20, 2004.
