Caught in, under or between a moving and a stationary object

While removing the guard off the oversize belt tail pulley, employee caught his ring finger (right hand) between guard and the guard mount.

Excavator operator was lifting pipe section for splice when pipe swung and mashed EE right hand against his knee causing his fingers to jam and possibly a hairline fracture in his small finger knuckel.

Struck against a moving object

The haul road on hill was washed away leaving a large pothole. Driver was going up hill and truck down shifted right as tire hit pothole causing seat to slam down on frame, resulting in a pulled muscle in his back.

Employee was shoveling under a hopper. It was a tight area and she used bad body position. She pulled a back muscle.

Cat 730 truck was stuck in mud on uneven ground. Operator had assisted in hooking up tow cables to haul truck. As operator was climbing back into the cab, the door slammed shut on his right thumb. Thumb had slight bruise. Operator was given medication for pain causing restricted duty for two days.

EE was attempting to clear a frozen clog from hopper gate with a pry bar. Clog came free, forced the bar down pinning his hand between bar, belt and hopper gate.
