EMPLOYEE WAS CHANGING BITS ON JOY MINING MACHINE WHEN HE BEGAN HAVING MUSCLE SPASMS IN HIS LOWER BACK. BECAME REPORTABLE ON 10-12-03 DUE TO MEDICATION GIVEN.
WHILE WELDING THE UNDERSIDE OF THE MAINFRAME ON A JOY MINER, THE SUPPORT JOY INSTALLED FOR THE REAR STABILIZER CAME LOOSE AND THE REAR STABILIZER CAME DOWN STRIKING EMPLOYEE. BECAME REPORTABLE ON 10-10-03 DUE TO MEDICATION GIVEN.
EE WAS IN CAGE WITH 5 OTHERS FROM COLLAR TO 5TH LEVEL TO BEGIN SHIFT. A SAFETY BELT AND LANYARD WAS SET ON THE FLOOR OF THE CAGE. PART OF THE BELT GOT UNDER THE CAGE DOORS AND HUNG ON SHAFT. P ERSON ACROSS FROM INJURED HIT HER WITH THE BRIM OF HIS HARD HAT.
EE MOVED THE MAGNET AT THE HEAD OF 3105 BELT TO REMOVE TRASH METAL THAT HAD COLLECTED ON MAGNET. HE STRAINED HIS SHOULDER WHILE DOING SO. HE WAS SEEN BY THE COMPANY DOCTOR ON 4-30-03, WAS DETE RMINED THAT HE HAD SUFFERED A TORN ROTATOR CUFF. HE CONTINUED TO WORK UNTIL 6-18-03, SURGERY WAS DONE TO REPAIR THE ROTATOR CUFF. STARTED LOSING TIME 6-18-03.
EE WAS SQUATTED DOWN BEATING ON PLUGGED TAIL LINE. WHEN HE ATTEMPTED TO STAND UP HE FELT PAIN IN LOWER BACK. THIS WAS GY SHIFT OF 3-25-03. THIS INJURY RESULTED IN RESTRICTED DUTY 3-25-03.
A JOINT OF 3" PIPE ON ROLLER STAND WITH ONE END NEXT TO STATIONARY SIDE OF THE CLAMP ON THE BANDSAW. THEY WERE IN THE PROCESS OF SETTING THE SAWTO A 45 DEG. ANGLE, WHEN THE PIPE ROLLED OFF THE STANDS. THE END NEXT TO THE SAW CAME UP MASHING EE'S RT. HAND.
MOVING PORTABLE A FRAME TO SET 480 VOLT ELECTRIC MOTOR IN PLACE A FRAME CAME IN CONTACT WITH SALT CONDITIONER HOUSING CATCHING FINGER BETWEEN. A DOCTOR VISIT REVEALED NO BROKEN BONES, 5 STITCH ES WERE REQUIRED.

EE WAS CHANGING THE AIR COMPRESSOR ON LUBE VEHI-CLE. AFTER REMOVING MOUNT BOLTS AND HOSES, HE TILTED THE COMPRESSOR TO REMOVE THE V-BELT AND MASHED HIS LITTLE FINGER BETWEEN THE SIDE OF THE ENGINE AND COMPRESSOR.
EE WAS CLEANING FEED BIN ABOVE #4 COMPACTOR FEED SCREW. THE ELECTRICIAN TOLD CREW THAT HE WAS GOING TO CLEAN MATERIAL FROM TOP OF BIN AND THEY SHOULD STAND CLEAR. A CHUNK FELL STRIKING A BAR, THE BAR FLEW UP AND STRUCK EE IN THE MOUTH & NOSE.
EE WAS PULLING ON MINER CABLE WITH A HOOK, WHEN HE SLIPPED AND FELL, HURTING HIS SHOULDER. HE WAS SEEN BY THE CO. DR. ON 3 OCCASIONS WITH DIAGNOSIS OF A STRAINED SHOULDER. AN MRI WAS DONE ON 1 -30-02 WITH DIAGNOSIS OF A ROTATOR CUFF TEAR IN THE LEFT SHOULDER. STARTED LOSING TIME ON 2-7-02.
EMPLOYEE WAS RESETTING A SLIDE GATE TO REGULATE FLOW OR MUCK FROM ORE BIN. FINGERS WERE CAUGHT IN PINCH POINT WHEN HE WAS ATTEMPTING TO HOLD STOP BOLT IN PLACE.
EMPLOYEE WAS MOVING A 480 VOLT POWER CABLE AND NIP. WHILE HE WAS PLACING IT ON THE YELLOW BELT STRUCTURE, HE FELT A SHARP PAIN IN HIS RIGHT GROIN. BECAME A LOST TIME ON 11/6/01.
BACK STRAIN- MECHANIC TOOK RUB BAR DOWN TO ACCESS MOTOR AND CONTROLLER. ATTEMPTED TO LIFT BAR BACK INTO PLACE. STRAINED LOWER BACK.
EMPLOYEE HAD CHANGED MOTOR IN TUNNEL UNDER WAREHOUSES. REMOVING 4 WHEEL MOTOR DOLLY FROM MANHOLE. A 3" X 2" X 14" CHUNK OF CONCRETE FELL FROM MAN HOLE AND STRUCK LOWER LIP. THIS INJURY REQUIRE D 1 STITCH TO CLOSE. NO LOSS OF TIME. NO RESTRICTED WORK.
EE REMOVING COVER TO ACCESS DRIVE LINE. HIT RIGHT KNEE WITH COVER. WHEN HE BENT DOWN AND PUT PRESSURE ON KNEE, FELT PAIN. KNEE CAP WAS BRUISED. DR. ADVISED RESTRICTED DUTY FOR FEW DAYS.
ATTEMTPING TO CHANGE BELTS ON PUMP-GOT LEFT THUMB CAUGHT BETWEEN SHEAVE AND BELT. JERKED THUMB OUT OF PINCH POINT, LACERATING THUMB AND BREAKING TIP OF BONE. INJURY REQUIRED STITCHES.
ATTEMPTING TO MOVE FLAT BAR 1/4" THICK BY 8" WIDE AND 6' LONG. IT FELL ON LEFT FOOT BRUISING FOOT. WENT TO DR ON 7-9, FOUND BROKEN BONES. BECAME RESTRICTED DUTY 7-10-01.

REACHED OUT TO LIFT COVER FOR CONTROL PANEL ON JOY MINER, FELT SNAP IN LEFT ARM. DR. DIAGNOSED BICEP TENTON TORN. CONTINUED TO WORK AT REGULAR DUTY UNTIL SURGERY ON 7-17-01. BECAME LOST TIME O N 7-17-01, SURGERY ON SHOULDER TO RE-ATTACH TENDON.
EE WAS INSTALLING A NEW GUARD AROUND THE TAIL ROLLER OF 8105 BELT. WHILE LIFTING THE GUARD INTO PLACE, THE GUARD HUNG ON A BOLT. HE STRAINED HIS SHOULDER WHILE LIFTING GUARD OFF THE BOLT. HE C ONTINUED TO WORK BUT WAS HAVING A PROBLEM WITH THE SHOULDER. HE WAS TAKEN TO A DR. ON 4-12-01. AFTER EXAM, RESTRICTIONS WERE GIVEN FOR LIFTING &FURTHER TESTS WERE ORDERED: TORN ROTATOR CUFF.
EE MASHED HIS RIGHT THUMB BY THE CAM SHAFT INSIDE THE GEARBOX WHEN THE BOOM WAS RAISED ON THE 9196 ROOF BOLTER. THIS REQUIRED 6 STITCHES.
EE PULLED A JACK HAMMER OFF TAIL GATE OF PU TRK THE WEIGHT OF THE HAMMER PULLED & JERKED HIS LEFT SHOULDER. HE DID NOT REPORT INJURY AT FIRST BUT CONT'D TO WORK FOR SEVERAL DA;Y. THE PAIN CO NTD TO WORSEN UNTIL IT GOT TO A POINT HE HAD TO SEE A DR. BECAME A RESTRICTED WORK ON 1/30/01
EE WAS MOVING A WOODEN CABINET ACROSS THE BELT DRIVE SHOP. THE DOOR CAME LOOSE AT THE TOP, FELL DOWN AND MASHED HIS LEFT HAND.
EMPLOYEE WAS WORKING ON THE ENGINE OF A TRACKMOBILE; THE DOOR OF THE OPERATORS COMPARTMENT WAS IN AN OPEN POSITION. HE GOT OFF THE TRACKMOBILE TO GET A PART AND AS HE STEPPED UP ON THE SIDE O F THE TRACKMOBILE, THE LEFT SIDE OF HIS HEAD CAME IN CONTACT WITH THE BOTTOM OUTSIDE CORNER OF THE DOOR, CAUSING INJURY THAT REQUIRED STITCHES.
