ALL EMPLOYEES WORKING IN THE MINE WERE NOTIFIED AND THE MINE WAS EVACUATED IN A N ORDERLY MANNER BY THE #2 HOIST. A TEMPORARY CABLE WAS INSTALLED AND POWER RESTORED. AFTER TESTING THE #1 ORE HOIST WAS RETURNED TO SERVICE. NORMAL OPERATIONS RESUMED AT 4:40 P.M.
EE WAS IN THE PROCESS OF CHANGING BATTERY PACKS ONT HE RAM CAR WHEN HE STEPPED ON UNLEVEL FLOOR, CAUSING HIS RIGHT ANKLE TO TWIST, RESULTING IN RESTRICTED DAYS OF WORK.
EMPLOYEE WAS CLEANING A ROCK OFF THE MINER THAT WAS BLOCKING THE LIGHT. WHEN IT FELL, THE ROCK ROLLED AND HIT HIS RIGHT FOOT ON THE LITTLE TOW BEHIND THE STEEL TOE. X-RAYS SHOW THE RIGHT LITTL E TOW HAS A FRACTURE.
EE WAS DRIVING A UTILITY VEHICLE ALONG AREA-512-EAST POWER ROOM WHEN HE HIT A BUMPT AT BT-2, AND THE LEFT SIDE OF HIS HEAD HIT THE MINE ROOF, LACERATING HIS EAR. SUTURES REQUIRED.
HOIST OUTAGE IN EXCESS OF 30 MINUTES. NORTH SKIP STUCK IN SCROLLS AS A RESULT OF BIN BEING OVER-FILLED. SKIP WAS UNSTUCK & RELEASED FROM POSITION. HOIST DOWN 2:30 PM. HOIST OPERATIONAL 4:45 PM . MINE WAS EVACUATED.

AFTER SPLICING 3155 BELT, EE WAS RELEASING THE TENSION ON A COME-A-LONG. COME-A-LONG KICKED BACK, MASHING FINGER BETWEEN HANDLE AND BELT CLAMP.
EE WAS HELPING A CO-WORKER CONNECT A 2 1/2" AIRLINE WHEN A ROCK APPROXIMATELY 8" ROUND STRUCK EE'S HAND. NO LOST TIME - FRACTURED BONE ON LEFT HAND MIDDLE FINGER.
HOIST OUTAGE, WHILE SERVICING AND MAKING ROUTINE CHECKS ON THIS HOIST, EE FOUND ONE COMMUTATOR INTERFACE BAR LOOSE IN THE HOIST MOTOR. PERSONNEL IN THE MINE WERE NOTIFIED AND PRODUCTION WAS SH UT DOWN. CREWS WERE TOLD TO EAT LUNCH & STAND BY TO TRAVEL TO SHAFT AREA. THE COMMUTATOR INTERFACE BAR WAS CLEANED & SOLDERED. TEST RUNS MADE & RELEASED FOR SERVICE AT 1:05 P.M.
EMPLOYEE WAS CHANGING BITS ON JOY MINING MACHINE WHEN HE BEGAN HAVING MUSCLE SPASMS IN HIS LOWER BACK. BECAME REPORTABLE ON 10-12-03 DUE TO MEDICATION GIVEN.
EE WAS WALKING DOWN AISLE IN CHANGE ROOM TO SHOWER AND HIT HIS RIGHT SMALL TOE ON METAL BENCH SUPPORT. FRACTURED SMALL TOE.
WHILE WELDING THE UNDERSIDE OF THE MAINFRAME ON A JOY MINER, THE SUPPORT JOY INSTALLED FOR THE REAR STABILIZER CAME LOOSE AND THE REAR STABILIZER CAME DOWN STRIKING EMPLOYEE. BECAME REPORTABLE ON 10-10-03 DUE TO MEDICATION GIVEN.
EE WAS IN CAGE WITH 5 OTHERS FROM COLLAR TO 5TH LEVEL TO BEGIN SHIFT. A SAFETY BELT AND LANYARD WAS SET ON THE FLOOR OF THE CAGE. PART OF THE BELT GOT UNDER THE CAGE DOORS AND HUNG ON SHAFT. P ERSON ACROSS FROM INJURED HIT HER WITH THE BRIM OF HIS HARD HAT.
EMPLOYEE LEAVING FACE ON MATRIP. HIT HEAD ON TOP. REQUIRED STITCHES TO CLOSE CUT TO SCALP. MEDICAL (STITCHES) NO LOST TIME.

WHILE THE ELECTRICAL POWE R WAS OFF NO WORK WAS DONE IN THE MINE. AFTER THE POWER WAS RESTORED, TESTS WERE MADE WITH BOTH HOISTS AND PRODUCTION RESUMED.

WHILE THE ELECT RICAL POWER WAS OFF NO WORK WAS DONE IN THE MINE AFTER THE POWER WAS RESTORED, TESTS WERE MADE WITH BOTH HOISTS AND PRODUCTION RESUMED.
USING EIMCO SCOOP TO CLEAN UP SPILL AT BOTTOM OF SHAFT. LOADED BUCKET, STARTED TO BACK UP SLOPE. SCOOP BUCKED AT ARTICULATING JOINT, PINNING OPERATOR INTO TOP CAUSING A CRUSHING INJURY TO UPPE R BODY. RESULTED IN 1 BROKEN RIB AND A TORN MUSCLE.
CREW WAS DRILLING A ROOF BOLT HOLE INTO MINE ROOF ALONG A NEW SLOPE. THE VIBRATION CREATED BY DRILLING CAUSED THE BOLTER TO CREEP A LITTLE DOWN THE SLOPE. THIS CAUSED THE STEEL TO BEND AGAINST SIDE OF HOLE. WHEN ATTEMPTING TO ADD A 2' EXTENSION OF DRILL STEEL EE'S FINGER GOT CAUGHT BETWEEN DRILL STEEL AND MINE ROOF AT COLLAR OF HOLE CAUSING INJURY. STARTED MISSING 5-7-03.
EE MOVED THE MAGNET AT THE HEAD OF 3105 BELT TO REMOVE TRASH METAL THAT HAD COLLECTED ON MAGNET. HE STRAINED HIS SHOULDER WHILE DOING SO. HE WAS SEEN BY THE COMPANY DOCTOR ON 4-30-03, WAS DETE RMINED THAT HE HAD SUFFERED A TORN ROTATOR CUFF. HE CONTINUED TO WORK UNTIL 6-18-03, SURGERY WAS DONE TO REPAIR THE ROTATOR CUFF. STARTED LOSING TIME 6-18-03.
EE WAS CUTTING "V" NOTCH IN BELT TO PUT CHAIN THROUGH TO PULL BELT THROUGH TAILPIECE. HE WAS CUTTING TOWARD LEG, KNIFE SLIPPED, HE CUT INSIDE OF LEFT KNEE. 3 STITCHES WERE TAKEN. NO LOST TIME.
EE WAS SQUATTED DOWN BEATING ON PLUGGED TAIL LINE. WHEN HE ATTEMPTED TO STAND UP HE FELT PAIN IN LOWER BACK. THIS WAS GY SHIFT OF 3-25-03. THIS INJURY RESULTED IN RESTRICTED DUTY 3-25-03.
EE WAS AT OFF SITE LOCATION SALVAGING EQUIPMENT. HE STEPPED INTO OPEN DITCH AND SCRAPED AND BRUISED LEFT SHIN.
CREW WAS MINING IN FACE OF LONG ROOM WHEN RELIEF HOLES, LOCATED AT ENTRANCE TO LONG ROOM & ANOTHER ABOUT 150'INTO L.R. STARTED BLOWING A LITTLE.AFTER CUTTING ANOTHER CAR OF MUCK, THE HOLES STA RTED BLOWING HARDER. THE ROOF BOLTER WAS MOVED INTO PLACE & BEGAN DRILLING ANOTHER RELIEF HOLE.FOREMAN WAS CALLED TO MONITOR AIR. OXYGEN METER ALARMED, INDICATING OXYGEN WAS BELOW 19.5%.
EE WAS DISMOUNTING THE D6M CAT DOZER WHEN HIS FOOT SLIPPED AND WENT BETWEEN THE BLADE PUSH ARM ASSEMBLY AND THE TRACK ADJUSTMENT HOUSING. HE FELL BACKWARDS WITH HIS FOOT CAUGHT, CAUSING INJURY .

AT 5:15 AM THE HOIST WENT DOWN, WAS PULLING HIGH AMPS WHEN HOISTMAN ATTEMPTED TO MOVE THE SKIPS. AN IMMEDIATE INVESTIGATION REVEALED THAT THE INSIDE GUIDE SHOE, WAS ROLLED UP & WEDGED AGAINST GUIDES. THIS WAS CUT OUT, NEW GUIDE SHOE WAS INSTALLED, DURING THE SHAFT INSPECTION THEY FOUND A SPLICE BAR ABOUT 200' FROM THE BOTTOM OF SHAFT THAT NEEDED BOLTS. THE BOLTS WERE REPLACED.

EMPLOYEE WAS OPERATING AN EIMCO 935 LHD SCOOPTRAM. HE SET THE PARK BRAKE AND WALKED BEHIND THE SCOOPTRAM TO HELP PIN THE CONVEYER CHAIN OF AN EIMCO CONT MINER. THE PARK BRAKE FAILED ALLOWING THE SCOOPTRAM TO ROLL, PINNING THE OPERATOR BETWEEN THE REAR OF THE SCOOP TRAM AND THE END OF THE BOOM OF THE CONTINUOUS MINER.

A JOINT OF 3" PIPE ON ROLLER STAND WITH ONE END NEXT TO STATIONARY SIDE OF THE CLAMP ON THE BANDSAW. THEY WERE IN THE PROCESS OF SETTING THE SAWTO A 45 DEG. ANGLE, WHEN THE PIPE ROLLED OFF THE STANDS. THE END NEXT TO THE SAW CAME UP MASHING EE'S RT. HAND.
MOVING PORTABLE A FRAME TO SET 480 VOLT ELECTRIC MOTOR IN PLACE A FRAME CAME IN CONTACT WITH SALT CONDITIONER HOUSING CATCHING FINGER BETWEEN. A DOCTOR VISIT REVEALED NO BROKEN BONES, 5 STITCH ES WERE REQUIRED.
EE WAS USING THE DRILL PRESS TO DRILL OUT A BEARING. THE DRILL BIT HUNG UP CAUSING THE BEARING TO SPIN AROUND. THE DRILL BIT BROKE & THE BEARING FLEW OFF & HIT HIM IN THE LEFT PALM. 4 STITCHES REQUIRED.
EE WAS A PASSENGER ON A WALLACE MANTRIP WHEN HIS SEAT CUSHION SLIPPED. HE GRABBED THE SEAT CUSHION AND WHILE DOING SO, HE PLACED HIS HAND ON THE EDGE OF THE FLOOR WITH HIS FINGERS EXTENDED UND ER THE FLOOR. THE VEHICLE HIT A BUMP IN THE ROADWAY AND THE BOTTOM OF THE VEHICLE CONTACTED ROADWAY, MASHING THE 2 MIDDLE FINGERS OF RIGHT HAND.
AT END OF SHIFT, EE & CO-WORKER WERE RETURNING TO SHAFT BOTTOM TO CAGE OUT OF MINE. CO-WORKER LOST CONTROL OF VEHICLE HE WAS DRIVING & RAN INTO MINE RIB. EE, WHO WAS A PASSENGER, WAS THROWN OF F FRONT OF VEHICLE & ROLLED ON MINE FLOOR FOR SEVERAL FEET, CAUSING HIS INJURIES.
AT THE END OF THE SHIFT, EE AND OTHER WERE RETURNING TO THE SHAFT BOTTOM TO CAGE OUT OF THE MINE. HE LOST CONTROL OF THE VEHICLE HE WAS DRIVING AND RAN INTO THE MINE RIB. THE LEFT SIDE OF THE VEHICLE WAS PUSHED BACK SEVERAL INCHES BY THE IMPACT. THE DASH AND STEERING WHEEL PINNED EE INT HE DRIVERS SEAT CAUSING HIS INJURIES. IT TOOK APPROX. 1 HOUR TO FREE HIM FROM VECHILE.
EE WAS CUTTING HOLE IN 5 GALLON PLASTIC BUCKET, WHEN THE BLADE OF THE RAZOR KNIFE BROKE AND HE PULLED THE BLADE ACROSS THE PALM OF HIS RIGHT HAND. REQUIRED 8 SUTURES TO CLOSE.
PULLING HOSE FROM GREASE WAGON. TURNED TO LOOK BACK, STEPPED INTO LOW PLACE IN FLOOR AND STRAINED LOWER BACK. PLACED ON RESTRICTED DUTY ON 3-11-02 THRU 3-18-02. NO TIME LOST FROM WORK. \
WHILE STRINGING OUT NEW ELECTRICAL CABLES BETWEEN THE CCD MOTOR CONTROL CENTER AND THE SULPHATE PLANT, EE STEPPED ON A LOOSE ROCK AND TWISTED HIS ANKLE.
