Employee was replacing a piece of catwalk on our truck access platform and while lowering new catwalk into place it slipped out of hand pinched right pinky finger between catwalk and frame.

Showing all 22Employee was getting large wheel chock from the service truck when EE lost grip on the chock. The chock glanced off of the truck bumper and struck EE's right shin, causing a laceration that required sutures.
Employee was dragging the cleanout hose around plant for cleanup. Employee felt a pain in back. Employee declined medical treatment at time of incident. EE did not think it was serious. Employee sent to company doctor on 8/17/16 for evaluation.
3/21/16: Employee was climbing steps at plant 1 when EE felt a pain in left knee. Employee wanted to wait to see if knee was fine before going to doctor. 3/24/16: Employee sent to company doctor. Received a pain injection. No work restriction.
While working on the Stone Plant material chute, employee felt a pop in his right knee. He continued to work rest of shift.
Employee was attempting to move dredge cutter head drive shaft by hand when he felt a pain in his left wrist. Left hand was at a 90 degree angle under the driveshaft lifting it 2" to allow a pallet to be slid under it. Pain was noticed after driveshaft was lowered. Pain went away after 15 minutes but later returned after moving the dredge heat exchanger. Light Duty
Employee was moving conveynor stop back onto product 1 beltline runway. Employee felt pain in lower back when he tried to lift the stop. Sent to company doctor.
Employee was loading pallet into sack machine when he felt a pop in his knee. Sent to company doctor.
Employee was changing screens @ Stone plant. While pulling himself out of screen, employee felt a pain in his right arm. Sent to company doctor.
Employee was shoveling @ Primary 1 and felt a pain in his lower back. Injury reported on 6/15/10 and sent to company doctor.
Employee was moving a 1" frozen water hose.
EMPLOYEE WAS ATTEMPTING TO REMOVE MOTOR SHEAVE & FELT TWINGE IN LOWER BACK.
Ee was walking from dredge to the work boat and felt a twinge in his knee. ee did not notify anyone and went home after shift was over. ee was at home for about 45 minutes, when while he was walking down hallway his knee gave out on him.
EE WAS SHOVELING TWISTING BACK. EE DID NOT REPORT INJURY UNTIL 40 DAYS AFTER INJURY OCCCURRED.
WHILE A CHUTE WAS BEING LOWERED AT SHAW LINE, INTO PLACE IT SLIPPED. FINGER GOT PINCHED BETWEEN SHARP EDGE OF CHUTE & ANOTHER PIECE OF METAL, LACERATING SM. FINGER.
WHILE LOADING 100# BAGS ON TO PALLET, ONE SLIPPED AND EE GRABBED TWISTING HIS BODY, CAUSING PAIN IN NECK.
WAS CARRYING PIPE AND BENT HIS RIGHT THUMB BACK.IT SWELLED UP AND HAD TROUBLE MOVING IT.
INSTALLING SUPER SACK ON BAG MACHINE. HIT LEVEL TO TIGHTER STACKER. RIGHT HAND TO CLOSE. LEVER PUNCHED FINGER.
WHILE FILLING FLUIDS ON DUMP TRUCK, FILLED HYDRAULIC FLUID, BROUGHT DUMP DOWN, NOTICED CAP NOT TIGHT. WENT TO TIGHTEN DOWN CAP, IT BLEW OFF SPRAYING FLUID EVERYWHERE, FLUID WENT INTO RIGHT EYE .
ADVISED OFFICE ONT HIS DATE (8-9-00) THAT HE HAD HURT HIS BACK SHOVELING OFF SURFACE SAND FROM THE SCALE ON 7-26-00.
FOLDING FLOATING ROLLER ON PORTABLE SCREEN PUSHED BACK AGAINST FINGER.
WHILE PUTTING SHRINK WRAPPED BAGS ON PALLETS, HEGOT SAND IN HIS LEFT EYE.
