Employee was walking down the office stairway and rolled EE's right ankle while taking EE's last step. Medical evaluation done the following morning revealed a closed fracture of EE's right ankle.

Employee was replacing a piece of catwalk on our truck access platform and while lowering new catwalk into place it slipped out of hand pinched right pinky finger between catwalk and frame.

Employee was getting large wheel chock from the service truck when EE lost grip on the chock. The chock glanced off of the truck bumper and struck EE's right shin, causing a laceration that required sutures.

Employee was smoothing out a piece of cut metal when a tiny shaving got into left eye. Employee was using a grinder with a buffer wheel. Employee was wearing safety glasses and gloves.

Struck by falling object

Maintenance man was removing the output shaft on the front of the Caterpillar engine that drives the hydraulic motor for the dredge. In the process of removing the shaft, the vibration dampener that the shaft was bolted to fell off and hit the maintenance man's right middle finger causing break to the right middle finger.

Employee was dragging the cleanout hose around plant for cleanup. Employee felt a pain in back. Employee declined medical treatment at time of incident. EE did not think it was serious. Employee sent to company doctor on 8/17/16 for evaluation.

3/21/16: Employee was climbing steps at plant 1 when EE felt a pain in left knee. Employee wanted to wait to see if knee was fine before going to doctor. 3/24/16: Employee sent to company doctor. Received a pain injection. No work restriction.

Employee slipped and fell to his wrists on ice. Temp was 15F. Employee was inspecting bottom hopper feeder and when he stepped down to exit he slipped injuring right wrist. Rock salt is currently being used to correct icey condition.

Employee was grinding down dredge cutter head shaft when the cup style grinding wheel on the 7"" grinder came apart and struck his arm causing lacerations and an arm fracture. Employee was wearing PPE.

Employee was attempting to move dredge cutter head drive shaft by hand when he felt a pain in his left wrist. Left hand was at a 90 degree angle under the driveshaft lifting it 2"" to allow a pallet to be slid under it. Pain was noticed after driveshaft was lowered. Pain went away after 15 minutes but later returned after moving the dredge heat exchanger. Light Duty

Customer driver was climbing a pallet in the bag house loading bay from ground level to concrete dock level when the pallet slid down and hit his elbow on the concrete.
