Employee was walking down the office stairway and rolled EE's right ankle while taking EE's last step. Medical evaluation done the following morning revealed a closed fracture of EE's right ankle.

Employee was replacing a piece of catwalk on our truck access platform and while lowering new catwalk into place it slipped out of hand pinched right pinky finger between catwalk and frame.

Employee was getting large wheel chock from the service truck when EE lost grip on the chock. The chock glanced off of the truck bumper and struck EE's right shin, causing a laceration that required sutures.

Employee aggravated a previous back injury. Not sure exactly what action caused the flare up.

Driving Loader.

Employee was smoothing out a piece of cut metal when a tiny shaving got into left eye. Employee was using a grinder with a buffer wheel. Employee was wearing safety glasses and gloves.

Maintenance man was removing the output shaft on the front of the Caterpillar engine that drives the hydraulic motor for the dredge. In the process of removing the shaft, the vibration dampener that the shaft was bolted to fell off and hit the maintenance man's right middle finger causing break to the right middle finger.

Employee was dragging the cleanout hose around plant for cleanup. Employee felt a pain in back. Employee declined medical treatment at time of incident. EE did not think it was serious. Employee sent to company doctor on 8/17/16 for evaluation.

3/21/16: Employee was climbing steps at plant 1 when EE felt a pain in left knee. Employee wanted to wait to see if knee was fine before going to doctor. 3/24/16: Employee sent to company doctor. Received a pain injection. No work restriction.

Employee slipped and fell to his wrists on ice. Temp was 15F. Employee was inspecting bottom hopper feeder and when he stepped down to exit he slipped injuring right wrist. Rock salt is currently being used to correct icey condition.

Employee was grinding down dredge cutter head shaft when the cup style grinding wheel on the 7"" grinder came apart and struck his arm causing lacerations and an arm fracture. Employee was wearing PPE.

While working on the Stone Plant material chute, employee felt a pop in his right knee. He continued to work rest of shift.

Employee was attempting to move dredge cutter head drive shaft by hand when he felt a pain in his left wrist. Left hand was at a 90 degree angle under the driveshaft lifting it 2"" to allow a pallet to be slid under it. Pain was noticed after driveshaft was lowered. Pain went away after 15 minutes but later returned after moving the dredge heat exchanger. Light Duty

Got piece of rust in eye.

Customer driver was climbing a pallet in the bag house loading bay from ground level to concrete dock level when the pallet slid down and hit his elbow on the concrete.

Employee was in Pitman man bucket being moved into position to perform work when he hit his chest against the bucket brake controller. Employee was wearing fall protection.

Employee was moving conveynor stop back onto product 1 beltline runway. Employee felt pain in lower back when he tried to lift the stop. Sent to company doctor.

Employee was loading pallet into sack machine when he felt a pop in his knee. Sent to company doctor.

Employee was changing screens @ Stone plant. While pulling himself out of screen, employee felt a pain in his right arm. Sent to company doctor.

Employee was shoveling @ Primary 1 and felt a pain in his lower back. Injury reported on 6/15/10 and sent to company doctor.

Climbing down off Powerscreen ladder @ Port Pond, slipped and twisted ankle. Sent to company doctor for exam.

Employee was climbing down into the BH pit when he slipped on some ice. He tried to catch himself with his left hand. Reported pain in his left pinky finger. Sent to company doctor for an x-ray. X-ray was neg, no broken bone.

Employee was moving a 1"" frozen water hose.

EE was climbing down a ladder at gravel plant. EE slipped and fell backwards 3-4ft to the ground. EE was taken to Dr to check bruised arm.

Operator was climbing stairs at his plant when he felt a the with in his knee.

Foundry driver (Truck3S) was crossing over back of his truck, tripped over his hoses and slipped off truck. Called ambulance, result was a dislocated shoulder.

EE was walking into storage shed and right foot went through pallet and twisted knee. Took EE to hospital, sprained knee.

FOUNDRY SERVICE TRUCK 16. DRIVER FELL OFF TRAILER WHILE LOADING (FELL BETWEEN END OF FLATBED & LOADING DOCK). HURT RIGHT KNEE. CALLED RESCUE SQUAD AND TOOK HIM TO THE HOSPITAL.

EMPLOYEE COMPLAINED OF CHEST PAINS. EMPLOYEE WAS TRANSPORTED VIA AMBULANCE TO HOSPITAL. EMPLOYEE WAS CHECKED AND RELEASED FROM HOSPITAL.

Empoyee was lowering steel grade into baghouse sandhopper when grade cut into his pinky finger.

CLIMBING ON DREDGE AND SLIPPED CAUSING ANKLE TO TWIST.

EE WAS SHOVELING SAND FROM HOPPER. DIZZYNESS, VOMITING AND EXCESSIVE SWEATING.

EMPLOYEE WAS ATTEMPTING TO REMOVE MOTOR SHEAVE & FELT TWINGE IN LOWER BACK.

Ee was walking from dredge to the work boat and felt a twinge in his knee. ee did not notify anyone and went home after shift was over. ee was at home for about 45 minutes, when while he was walking down hallway his knee gave out on him.

EE WAS SHOVELING TWISTING BACK. EE DID NOT REPORT INJURY UNTIL 40 DAYS AFTER INJURY OCCCURRED.

WHILE A CHUTE WAS BEING LOWERED AT SHAW LINE, INTO PLACE IT SLIPPED. FINGER GOT PINCHED BETWEEN SHARP EDGE OF CHUTE & ANOTHER PIECE OF METAL, LACERATING SM. FINGER.

WHILE CUTTING & PULLING CABLE, A PIECE OF METAL FLEW BACK OVER GLASSES AND STRUCK EYE.

WHILE LOADING 100# BAGS ON TO PALLET, ONE SLIPPED AND EE GRABBED TWISTING HIS BODY, CAUSING PAIN IN NECK.

WAS CARRYING PIPE AND BENT HIS RIGHT THUMB BACK.IT SWELLED UP AND HAD TROUBLE MOVING IT.

TORCHING STEEL PLATE, MATERIAL BLEW BACK AT HIS FACE. A PIECE WENT OVER SAFETY GLASSES INTO EYE.

WHILE WELDING AT THE SHAWLINE HAD RIGHT KNEE RESTING ON GROUND, SWELLED UP.

WHILE LEAVING BAG HOUSE, JUMPED OFF LOADING DOCK AND TWISTED HIS ANKLE.

INSTALLING SUPER SACK ON BAG MACHINE. HIT LEVEL TO TIGHTER STACKER. RIGHT HAND TO CLOSE. LEVER PUNCHED FINGER.

WORKING ON WORK TRAILOR-SCOOTED UNDER TO TIGHTEN BOLT WHILE UNDER, PUSHED OFF WITH RIGHT FOOT AND TWISTED RIGHT KNEE. UNEVEN RUTTED GROUND.

WHILE FILLING FLUIDS ON DUMP TRUCK, FILLED HYDRAULIC FLUID, BROUGHT DUMP DOWN, NOTICED CAP NOT TIGHT. WENT TO TIGHTEN DOWN CAP, IT BLEW OFF SPRAYING FLUID EVERYWHERE, FLUID WENT INTO RIGHT EYE .

WHILE DRIVING 930 LOADER AND FILLING POWER SCREEN, DUST ENTERED EMPLOYEE'S LEFT EYE- WAS NOT WEARING SAFETY GLASSES. (CAB DOORS OPEN)

WHILE WELDING A BRACKET ON THE LOADER LEAKING OIL CAUGHT FIRE AND FLASHED IN HIS FACE.

WHILE DRIVING 930 LOADER EMPLOYEE HIT A HOLE IN THE ROAD, SHE WAS THROWN AROUND IN CAB (SHE WAS WEARING A SEATBELT.).

WALKED OUT FRONT DOOR OF OFFICE BY SCALE HOUSE LEFT FOOT CAME DOWN ON GRATING AND TWSITED FOOT AND ANKLE.

WHILE HAMMERING ON A PIECE OF PIPE, SOMETHING HIT HIM IN THE EYE.

WAS CUTTING WITH A TORCH AND A HOT PIECE OF SLAG FELL ON HIS FINGER. SHOOK HIS HAND TO GET IT OFF AND HIT HIS WRIST ON A PIECE OF PIPE.

WHILE PULLING OUT A SCREEN ON THE WASH EMPLOYEE STEPPED BACK AND TWISTED HIS BACK. HE TRIPPED OVER A BOX OF PULLYS.

ADVISED OFFICE ONT HIS DATE (8-9-00) THAT HE HAD HURT HIS BACK SHOVELING OFF SURFACE SAND FROM THE SCALE ON 7-26-00.

FOLDING FLOATING ROLLER ON PORTABLE SCREEN PUSHED BACK AGAINST FINGER.

GOING UP STAIRS AT DRYER HOUSE, TRIPPED AND PUT A GASH IN LEFT KNEE.

WHILE PUTTING SHRINK WRAPPED BAGS ON PALLETS, HEGOT SAND IN HIS LEFT EYE.

EMPLOYEE WAS GETTING PUSH BROOM IN BAG HOUSE. THE LIGHTS IN THE BUILDING WERE OUT DUE TO A POWEROUTAGE. HE WALKED AROUND THE FORKLIFT AND FELL OFF THE LOADING DOCK INTO A PILE OF SAND.

EE WENT TO CLIMB UP ON BAGGING MACHINE TO OPERATE. HIS FOOT SLIPPED OFF THE PEG AND HE FELL AGAINST BAG MACHINE )MUD/SAND ON BOTTOM OF SHOE CREATING SLIP).
