EE dumped a portable dumping hopper. EE was returning the dumping hopper to its upright position. EE had hand in a pinch area. While pushing down on the top edge of the hopper EE's hand/fingers were pinched between hopper and forklift load backrest . X-ray was negative. EE was put on restrictive duty by medical professional.

The employee was setting a new roll of shrink wrap onto the bracket. The employee did not move hand out of the way when EE laid the metal pipe (that goes through the middle of the roll) down onto the bracket. The employee's middle finger got caught in this pinch point which caused a laceration to the middle finger. The employee received several stitches to close the wound.

The employee was lifting 50 lb chemical bags from the pallet to the feeder when the employee felt a pop and sudden pain in the employee's back. The employee's back continued to be sore after the incident. On 8/17/2020 the employee saw a chiropractor and received medical treatment for the injury.

While lifting an empty pallet the employee felt pain in their back. The employee did not want to see a medical professional at the time of the incident. On 6/17/2020 the employee decided to see a chiropractor. The soonest available appointment was scheduled for 6/25/2020 at which date the employee saw a chiropractor and the employee was put on restrictive duty.

A new employee was being trained on how to seal and stack 50 lb bags. During this time the employee's left forearm and wrist became sore. On 9-30-2018 the employee reported that the employee's forearm/wrist area was swollen and it was hard to close the employee's hand. On 10-01-2018 the employee saw a physician and was diagnosed with possible tendinitis and put on restrictive duty.

EE was lifting 100 lb. bags off of the ground and restacking them. While doing so, tripped on some bags on the ground possibly straining their back. The repetitive lifting and the trip caused them to have a sore back. The next day their back was having spasms and was in pain. Employee was diagnosed with a back strain on 01/11/2018.

EE assisting with knife replacement on pugmill. When turning drive belt by hand to rotate shaft, addition of knives made it easier to turn and momentum easier. Finger was caught between belt and sheave resulting in a cut to ring finger.

On 2/17/17 while working on the outside of the Process Elevator the EE was pulling a bearing off from the outside while another employee had cut the shaft with an acetylene torch that the bearing was connected to. EE picked up the bearing by the shaft causing burns on EE's hand.

EE was assisting in a four person team lift of an electric motor. During the course of the second lift, he indicated that he felt his shoulder pop. Two hours later he was taken to the hospital where an examination was performed and it was recommended by the physician that the EE be given restricted duty.

EE was arranging augers on the drill truck when EE caught their thumb between two auger. This resulted in the thumb needing stitches to re-attach the nail. There is also a fracture on the tip of the thumb that will require follow up examinations and further treatment.

EE was closing the access door on the bag house when he adjusted his hands causing the right hand to slip and bringing the door down upon his left hand. The middle finger of the left hand was caught between the door and the seal edge severing the tip of his middle finger.

Supervisor observed on 2/4/14 that EE's eye was tearing up. EE said he had gotten dust in it on 2/3/14 while working on the packer. Supervisor asked if he needed medical attention and EE said no. On 2/5/14 EE informed Supervisor he had gone to the doctor and his eye was scratched. EE received prescription eye drops.

EE was cleaning a plugged chemical induction pipe when some chemical (ferric sulfate) came out of the pipe and got in EE's eye. EE was wearing safety glasses. EE had eyes flushed with eye wash solution and received a prescription for eye drops.

EE informed supervisor 13 hours after his shift ended that he had injured his back while clearing a bag jam in a packaging line. EE did not indicate any accident or injury prior to leaving his shift that day. There were no witnesses or indications of an injury during his shift.

While freeing the last bolt in a joint of two 8-inch suction lines the unsupported end of one pipe dropped roughly 4 inches. When the pipe dropped it pinched his index and middle fingers in between the flange on the pipe and a metal support bracket. Instinctively he tried to jerk his hand out of harms way. The nail on the index finger was lost and the tip of the finger was broken.

EE was behind the packer bin working to unplug the #2 spout. When he turned the wheel to open the back of the spout, it came open and smashed his fingers between the spout back and the wheel to loosen the spout back. EE fractured the tip of his middle finger.

The miner was putting a bump stop on a Grizzly Feeder when a clamp that was holding the metal pipe in place slipped smashing his right hand between the pipe and a concrete wall. His middle finger on his right hand was also cut and required three stitches.

Employee was removing a counter weight on a Doosan D-25S forklift to investigate a steering column leak. The counter weight fell on top of employee's legs. Employee recieved severe laceration to the inside left thigh resulting in muscle, tissue, and ligament damage. Employee was transported to Washakie County Hospital for surgical repair.

While working on a flourescent light from an extension ladder, the ladder came in contact with the light bulb and fixture causing the bulb to break. A shard of glass from the bulb cut the employee's left arm. The employee saw a physician to have the wound cleaned and bandaged.
