Employee was getting out of the blade when EE felt pain in EE's upper right arm.

Over-exertion in pulling or pushing objects

Employee hurt EE's shoulder while dragging a fuel hose. We were waiting upon diagnostics to figure out what was wrong to know whether it was reportable or not. Employee has been placed on light duty until surgery for a torn rotator cuff.

Employee was exiting a scraper located at the shop when EE's foot slipped causing EE's ribs to make contact with the floor of the scraper cab.

The employee was fixing a coolant leak of a D-10 dozer at Johnson SL-8 location, when the water pump being used to pump coolant from the top of the radiator fell, contacting the top of the employee's head.

Employee was backing dozer up into a cut and ran over a large rock. The employee had EE's hand resting on the steering controls and was belted in, however, the motion caused pain in EE's shoulder. The employee is now off of work.

When climbing into the loader, the employee skipped a step. Because the employee skipped a step, the step the employee took with right leg was high and awkward. Employee attempted to stand up when the right foot slipped resulting in the employee falling 3 feet to the ground. Employee felt pain in an area where EE had plate installed in neck.

Employee was in the process of climbing of the bulldozer when his left foot slipped and he fell approximately 5 feet to the ground. The employee was not using 3 points of contact. The employee suffered a broken collarbone and 5 broken ribs.

Employee was attempting to wash the windows on the loader when he slipped and fell to the ground. The employee bypassed guardrail to reach the window.

Employee was operating a scraper when she felt hit a bump and felt a sharp pain in her lower back. The employee was seen by a Doctor and the results of an MRI revealed that a compression fracture occurred in the L1 vertebrae.

Struck against stationary object

Employee was poked in the finger by a wire from an Hydraulic line. The wire broke off inside the employees finger. The employee's finger became infected because of the wire. The infection spread through the rest of the employee's body.

Over-exertion in lifting objects

Injured his left shoulder while replacing the fan clutch on a haul truck. Employee was supporting the majority of the weight of the fan clutch in his left hand and was guiding the part with his right hand when he felt a sharp pain in his right shoulder.

Employee was in the process of assisting another employee change out a starter on a Cat 988H loader, and during this process, the employee who was actually extracting the starter out of the machine, as he was attempting to lower the starter, it slipped out of his hand and struck his partner's finger, causing a laceration to his finger.

Employee was standing on the south east corner of the hopper barring out the corner of the hopper. As employee stepped back her foot caught on the corner of the hopper causing the employee to fall backward. As employee fell backward toe caught on corner of hopper and strained her foot.

Employee lifted a pallet and radiator using a forklift. When he noticed the radiator was not square on pallet, he stops forklift and straightens radiator by lifting one corner. As he lifts, he felt a ""pop"" in his right bicep.

EE states he injured his right knee while kneeling down to repair a pump, EE states it didn't hurt He did not report it he then went on 3 days off. Returned to work worked 1 day. Next day reported to supervisor he hurt himself on 5/12/06. Co con- tested with WC. On 8/21/06 WC would pay this as on the job injury.

Employee was washing haul truck at wash bay located at mine shop. Employee tripped on the drain and fell backwards. While attempting to brace his fall against the wall he slid down striking a five gallon plastic bucket on his lower back.

Employee was removing a drill stem when he crushed his right index finger between the auger and the bed of the truck. He worked all shifts prior to being seen by a doctor. He returned on his next shift.

EE had finished loading haul truck and was climbing off the loader. she slipped and bumped her elbow. she tried to countinue to work, but the pain persited. she was seen by a dr for her injury.

EE WAS UNTHAWING FROZEN BRAKES WITH A HAMMER WHEN LOWER BACK POPPED CAUSING PAIN. EE CONTINUED TO WORK UNTIL 01/03/2005, THEN WENT TO THE DOCTOR.

EE WAS REPLACING CUTTING EDGE ON A 637 SCRAPER (EQUIPMENT #S12). EE WAS USING A CUTTING TORCH TO REMOVE A BOLT THAT HAD BECOME STUCK. WHEN THE BOLT WAS CUT LOOSE, THE CUTTING EDGE SNAPPED AND STRUCK THE EE IN THE RIGHT SHOULDER, AND THE RIGHT AND BACK OF HEAD.
