Employee was getting out of the blade when EE felt pain in EE's upper right arm.

Employee hurt EE's shoulder while dragging a fuel hose. We were waiting upon diagnostics to figure out what was wrong to know whether it was reportable or not. Employee has been placed on light duty until surgery for a torn rotator cuff.

Employee was exiting a scraper located at the shop when EE's foot slipped causing EE's ribs to make contact with the floor of the scraper cab.

Employee was cleaning back window of a field drying tractor when they slipped and fell.

The employee was fixing a coolant leak of a D-10 dozer at Johnson SL-8 location, when the water pump being used to pump coolant from the top of the radiator fell, contacting the top of the employee's head.

Employee was backing dozer up into a cut and ran over a large rock. The employee had EE's hand resting on the steering controls and was belted in, however, the motion caused pain in EE's shoulder. The employee is now off of work.

When climbing into the loader, the employee skipped a step. Because the employee skipped a step, the step the employee took with right leg was high and awkward. Employee attempted to stand up when the right foot slipped resulting in the employee falling 3 feet to the ground. Employee felt pain in an area where EE had plate installed in neck.

Employee was in the process of climbing of the bulldozer when his left foot slipped and he fell approximately 5 feet to the ground. The employee was not using 3 points of contact. The employee suffered a broken collarbone and 5 broken ribs.

Employee was attempting to wash the windows on the loader when he slipped and fell to the ground. The employee bypassed guardrail to reach the window.

Employee was operating a scraper when she felt hit a bump and felt a sharp pain in her lower back. The employee was seen by a Doctor and the results of an MRI revealed that a compression fracture occurred in the L1 vertebrae.

Employee was poked in the finger by a wire from an Hydraulic line. The wire broke off inside the employees finger. The employee's finger became infected because of the wire. The infection spread through the rest of the employee's body.

Injured his left shoulder while replacing the fan clutch on a haul truck. Employee was supporting the majority of the weight of the fan clutch in his left hand and was guiding the part with his right hand when he felt a sharp pain in his right shoulder.

Employee was in the process of assisting another employee change out a starter on a Cat 988H loader, and during this process, the employee who was actually extracting the starter out of the machine, as he was attempting to lower the starter, it slipped out of his hand and struck his partner's finger, causing a laceration to his finger.

Employee was standing on the south east corner of the hopper barring out the corner of the hopper. As employee stepped back her foot caught on the corner of the hopper causing the employee to fall backward. As employee fell backward toe caught on corner of hopper and strained her foot.

Employee lifted a pallet and radiator using a forklift. When he noticed the radiator was not square on pallet, he stops forklift and straightens radiator by lifting one corner. As he lifts, he felt a ""pop"" in his right bicep.

This is an re-aggravation of an an pre-existing injury. Injury was re-aggravated by getting up and down off his knee's while working on concrete.

EE states he injured his right knee while kneeling down to repair a pump, EE states it didn't hurt He did not report it he then went on 3 days off. Returned to work worked 1 day. Next day reported to supervisor he hurt himself on 5/12/06. Co con- tested with WC. On 8/21/06 WC would pay this as on the job injury.

Employee was washing haul truck at wash bay located at mine shop. Employee tripped on the drain and fell backwards. While attempting to brace his fall against the wall he slid down striking a five gallon plastic bucket on his lower back.

Employee was removing a drill stem when he crushed his right index finger between the auger and the bed of the truck. He worked all shifts prior to being seen by a doctor. He returned on his next shift.

EE had finished loading haul truck and was climbing off the loader. she slipped and bumped her elbow. she tried to countinue to work, but the pain persited. she was seen by a dr for her injury.

Employee was performing backfill operations when the machine tipped. Employee experienced minor back pain the next day. He was seen by a chiropractor making this a lost time accident.

Employee was driving a haul truck when he came upon an icy corner. as he applied the brakes his truck jack-knife.

EE WAS UNTHAWING FROZEN BRAKES WITH A HAMMER WHEN LOWER BACK POPPED CAUSING PAIN. EE CONTINUED TO WORK UNTIL 01/03/2005, THEN WENT TO THE DOCTOR.

EE WAS REPLACING CUTTING EDGE ON A 637 SCRAPER (EQUIPMENT #S12). EE WAS USING A CUTTING TORCH TO REMOVE A BOLT THAT HAD BECOME STUCK. WHEN THE BOLT WAS CUT LOOSE, THE CUTTING EDGE SNAPPED AND STRUCK THE EE IN THE RIGHT SHOULDER, AND THE RIGHT AND BACK OF HEAD.

EMPLOYEE EXPERIENCED NUMBNESS IN RIGHT HAND. HE WAS EVALUATED BY A DR. AND WAS DIAGNOSED WITH CARPAL TUNNEL SYNDROME.

EE WAS REFUELING THE CAT 988F LOADER WHEN HE SLIPPED & LANDED ON HIS SHOULDER. HE WAS ABLE TO COMPLETE ALL SHIFTS & WAS SEEN BY A DR ON FRIDAY 2/27/04.

EE WAS SCRAPING OUT BELLY DUMP TRAILER WHEN HER FINGERS WENT NUMB. SHE IS CLAIMING THAT REPETITIVE MOTION CAUSED THIS CONDITION.

EE WAS WORKING UNDER A HAUL TRUCK WHEN A WRENCH FROM ABOVE FELL AND STRUCK HIM IN THE MOUTH. HIS TOOTH WAS BROKEN IN HALF AND REPAIRED BY THE DENTIST.

EE WAS PLACING SPRINKLER ON THE WATER TRAILER AND PULLED HIS SHOULDER.

EE WAS REMOVING A POWER TAKE OFF UNIT (PTO) WHEN IT SUDDENLY CAME LOOSE AND HE HIT HIS RT. ELBOW ON THE CONCRETE FLOOR. HE WAS TAKEN TO THE DR. AS A PRECAUTION.

EE WAS WORKING ON THE BRAKES OF A LOADER WHEN HE SLIPPED OFF THE TIRE AND TWISTED HIS KNEE. THIS EE REFUSED A MEDICAL CHECK-UP AT THE TIME OF THE ACCIDENT.

EMPLOYEE WAS WALKING ON SIDEWALK FROM SHOP TO THE BREAKROOM, AND TWISTED HIS LEFT ANKLE.

EMPLOYEE WAS RUNNING CAT 988F LOADER IN THE RAINWHEN SHE SLIPPED WHILE CLEANING THE WINDSHIELD. SHE HAD MINOR INJURIES (BUMPS AND SCRAPES) TO HER LEFT LEG.

EE WAS SMOOTHING OUT PILE WHEN THE CAT SLIPPED ROLLING ON ITS SIDE.

OPERATOR OF THE WATER TRUCK WORKING ON THE HAUL ROAD ATTEMPTED TO GO AROUND A CORNER WHEN WATER INT HE TRAILER SHIFTED, CAUSING THE TRUCK TO ROLL PINNING THE DRIVER IN THE CAB. THIS ACCIDENT R EQUIRED MEDICAL ATTENTION FOR HIS INJURIES.

EMPLOYEE WAS WORKING ON EQUIPMENT WHEN HE SUDDENLY LIFTED AWKWARDLY AND POPPED LEFT ELBOW OUT OF PLACE. THIS ACCIDENT REQUIRED MEDICAL ATTENTION.

EE WAS CHANGING BELTS ON A 631 CAT SCRAPER WHEN HIS SHOULDER POPPED CAUSING PAIN.

EE WAS OPERATING A 637 CAT SCRAPER WHEN HE ENCOUNTERED ROUGH TERAIN AND JARRED HIS BACK.

EMPLOYEE WAS WORKING ON A PIECE OF EQUIPMENT WHEN TIGHTNESS OCCURRED IN HIS RIGHT ELBOW.

OPERATOR WAS RUNNING CAT 988F LOADER AND ENTERED THE STOCKPILE. GROUND WAS FROZEN AND EJECTED OPERATOR INTOWINDSHIELD. NO SEAT BELT WAS WORN, WHICH WAS IN GOOD, OPERATING ORDER.

EMPLOYEE WAS EXITING CAB OF LUBE TRUCK, WHEN FOOT BECAME LODGED IN STEP CAUSING SEVERE PAIN IN BACK.

THE HOT WATER WASHER WAS LEFT ON BY ANOTHER EMPLOYEE. AS HE WENT TO TURN THE MACHINE OFF, HE SLIPPED AND INJURED HIS RIGHT ELBOW.

EMPLOYEE WAS GETTING INTO HAUL TRUCK, LOST HIS BALANCE AND FELL TO THE GROUND.

CUTTING HYDRAULIC HOSE WITH CHOP SAW, WHEN METAL FLEW OFF SAW BOUNCED OFF SAFETY GLASSES AND INTO LEFT EYE.

MOVING A PIECE OF METAL WITH A SLEDGE HAMMER THE METAL SPLINTERED AND LODGED IN RIGHT EYE. THE WIND WAS BLOWING WHICH MAY HAVE BEEN A FACTOR.
