Changeling a punch plate on the second deck of scalping screen. EE stated he felt a pull in his lower back. Declined med attention at this time. Called supervisor at 5:30pm on 10/31/11 and stated his back was stiff and sore, but he would wait until morning and see how it felt. Called supervisor at 5:30 am on 11/1/11 and requested to see a doctor because his back still hurt.

Showing all 4Employee was lifting on a pulley guard that was stuck in semi-frozen mud. About 45 min. later he began to have pain in the lower right abdomen/groin area. He said he did not initially injure himself at work; that he had severe pain the previous week and he had to stop doing physical activity. We sent him to the doctor as a precaution and he was prescribed Naprosyn 800mg.
Employee was filing something in a metal file cabinet and as she turned to walk away, she accidentally closed the cabinet drawer on her right little finger.
EE WAS ATTEMPTING TO REPOSITION A CRUSHER HAMMER ON A PALLET WHEN THE HAMMER MOVE UNEXPECTEDLY, CATCHING HIS LITTLE FINGER BETWEEN TWO HAMMERS. BOTH HAMMERS HAD JUST BEEN RELOCATED TO THE SHOP , BUT ONE WAS NOT DOWN COMPLETELY ON THE PALLET.
