Miner was complaining of back and chest pain for a couple of weeks. At 2:00pm, miner provided a description of pain similar to a heart attack. Miner was transported to hospital by prod. mgr.

EE was splicing a water pump discharge hose. EE slipped on a rock and twisted back as EE fell.

Employee is unable to substantiate any of the above details and while we are unsure this incident actually happened at work, we are reporting on the side of caution. Employee only reports that they thought they had something in their eye when they left work on a Friday and sought treatment later that evening.

The employee was cutting conveyer belt with a retractable knife. As he pulled the knife, toward his body, it slipped and lacerated his right leg. The laceration is about an inch long, but it did require stitches. He was treated and released back to work without restrictions.

Employee was lifting on a pulley guard that was stuck in semi-frozen mud. About 45 min. later he began to have pain in the lower right abdomen/groin area. He said he did not initially injure himself at work; that he had severe pain the previous week and he had to stop doing physical activity. We sent him to the doctor as a precaution and he was prescribed Naprosyn 800mg.
Employee was filing something in a metal file cabinet and as she turned to walk away, she accidentally closed the cabinet drawer on her right little finger.

EE WAS ATTEMPTING TO REPOSITION A CRUSHER HAMMER ON A PALLET WHEN THE HAMMER MOVE UNEXPECTEDLY, CATCHING HIS LITTLE FINGER BETWEEN TWO HAMMERS. BOTH HAMMERS HAD JUST BEEN RELOCATED TO THE SHOP , BUT ONE WAS NOT DOWN COMPLETELY ON THE PALLET.
EE WAS USING A FACTORY WEDGE FOR TAKING THE CONE CRUSHER APART WHEN HE HIT THE WEDGE WITH A SLEDGEHAMMER, A PIECE CHIPPED OFF OF WEDGE AND HIT HIS LEFT ARM.

EE WAS TAKING IT UPON HIMSELF TO LOOK AT THE MOTOR ON THE CONVEYOR TO GET A SERIAL NUMBER. WHEN HE CLIMBED UP ON THE CONVEYOR(NOT RUNNING). HE SLIPPED AND FELL ONTO THE CONVEYOR, HITTING A ROL LER(?) INJURING HIS BACK.

EE STARTED OPERATING THE HITACHI EXCAVATOR WITH RAMER HYDRAULIC HAMMER IN 3/1999. EE COMPLAINED OF PAIN & BURNING & EVENTUALLY HAD BLEEDING IN HIS STOOL. ON 3/7/00 EE WAS DIAGNOSED BY THE CO DR AS HAVING A TEAR OR FISSURE INSIDE THE RECTUMSURGERY IS SCHEDULED AT WHICH TIME THE SURGEON HAS STATED HE WILL DETERMINE WHETHER THE INJURY IS WORK RELATED OR NOT
