CONTRACTOR/CUSTOMER After loading their truck with stone at our site, C R Cozino Inc's employee was injured. The driver was attempting to remove some stone & punctured their left hand when the tailgate latch fell on their hand that was positioned on the tailgate pin. NOT A LUCK STONE EMPLOYEE.

While repairing the jaw crusher with another associate, EE was putting the nuts on the wedge bolts. The access for EE was limited. The wedge slipped out of co-worker's hand and then ricocheted toward EE, hitting EE's hand.

Associate came up to the metal yard to pick up a couple pieces of metal for a chute rebuild. As they started sliding the large angle iron off the sheet of metal they needed, they lost track of their surroundings & pinky figure was pinched between angle in their hand and another piece of metal behind them. Employee was wearing neoprene coated fabric gloves.

While opening the front gate the associate inserted a bolt with washer used to keep the gate from swinging shut. Before the bolt was inserted completely, the gate sprung backwards pinching the associates finger between the gate and the washer on the bolt causing a laceration to one finger that required stitches.

While cleaning out ice build up in the socket sealing ring, the associate made contact with one of the edges on it, resulting in a laceration on the back of the wrist through the glove. The laceration resulted in 7 stitches.

Employee was dragging a wash hose, stepping backwards and stepped on a rock approximately 3" in diameter. This caused pain to the right foot and ankle.

Employee was clearing obstruction out of end of shotcrete hose. Took safety glasses off to see in the hose when the obstruction came out, striking the employee in the face with shotcrete. Resulted in chipped tooth and flushing of eyes with water.

The associate was moving punch plates off the pallet to take in the screenhouse. The associate was sliding one of the plates off the pallet to put a clevis on it so they would be able to hoist it into the screenhouse. While sliding it, thumb got caught in the holes and almost severed the tip of the thumb.

As EE was leaving work for the day on Thursday, EE noticed the hangar gate was open so EE pulled in to shut it. While in the process of closing it, EE tweaked EE's lower back. EE worked all day Friday, but it continually got worse & EE was unable to work Saturday or Monday.

While removing torque out of truck, ee reached over frame and started pulling wrench towards ee and felt and heard something pop in shoulder .

While removing a zip tie from a hardhat, employee received a small puncture wound from the cut end of a zip tie. There was no immediate pain or even blood, so employee shook it off and went about the day. The next morning the finger had become very swollen and infected.

Two associates installed and tightened the four liner bolts. Another associate was trying to determine if the liner was seated correctly by running their fingers behind the bottom edge of the liner. At that instant, the liner snapped into position crushing the tips of the index and middle finger on their left hand.

Removing bearing from loader frame. Track bar skipped off the bearing sidewall and into the bearing bore. Caught technician's left pinky finger between the track bar and loader frame. Partial amputation of pinky finger tip.

Employee had lifted the lid on the transfer box and climbed in to measure some wear metal. Lid was held open by a chain. When reaching to reposition self the lid fell down onto EE's hand. We believe that the wind, the employee touching the chain with EE's hand or vibration of the chute from EE's movement caused the lid to close.

WHILE REPLACING OLD SCALE DECK, EMPLOYEE REACHED DOWN TO SUPPORT THE EDGE OF A SECTION OF THE DECKING. WHEN THE SECTION OF THE DECK CUT LOOSE, THE PLATE DROPPED DOWN RESULTING IN CRUSHING HIS LEFT MIDDLE FINGER.

We had a stack of cribbing timbers to be used for the project. The timbers were 10"x10"x8". They were stacked 5 high and 4 rows wide with one band around the center. When employee cut the band on the timbers, the first stack toppled over. The timber that was on the top landed on his left foot. The incident resulted in a broken bone on the top of his foot.

The driller was changing a drill steel when he noticed the steel was cross threaded on the power head. He pushed on the steel with his rt hand to correct the problem & the steel suddenly disconnected from power head & dropped onto the table, crushing the tips of his 2nd & 3rd fingers on his lt hand which he had rested on the table. The 1st joint of his ring finger was amputated.

Associate was working on bin 3 crusher with use of a contract crane service. When the crane began lifting the box one side of the box became wedged on its seating surface. One associate tried to apply pressure to one end of the box using downward pressure by pulling on the box by hand. The result was the box shifted approximately 3 inch injuring the his right hand.

2 associates were relining the chutes in the tunnel at Richmond, they needed to get a 200 lb plate into the tunnel feeder at the back. They carried it halfway and sat it down to take a break, when they started to pick it up EE didn't have good enough grip and it slipped out of his hand, falling about 2 inches, pinching his finger between the plate and the concrete.

Assoc was opening an underground valve at water truck fill station by turning a T-valve. Got it to the stop point, wiggled it a couple of times to make sure it was fully open. When he finished, he felt immediate pain in the left arm.

Troubleshooting the hydraulic adjustment system on the crusher. Installed a pressure gauge on the pump by attaching the pressure hose to a malleable black iron tee that had the pressure gauge threaded in the other end. Pump was turned on and hydraulic pressure split the tee that was in his hand. The hydraulic fluid, under 4000 psi, cut his thumb and damaged a tendon.

Employee was pulling waterhose to spray-clean equipment. This led to a twisting motion as he was jerking the hose forward. He twisted his left knee and felt a pop as he was pulling the hose.

While installing a distributor plant on 5 1/2 shorthead crusher, assoc. finger was smashed between distributor plant and distributor head. The crane was hooked to the plate and there was a very little tension and when assoc. attempted to move the plate it fell into place on the associ. finger
