While working to replace a cylinder on a motor grader, the associate pulled the cylinder back to realign it & pinched EE's finger between the cylinder block & the frame of the motor grader.

Associate was replacing some rubber with a safety knife. As the associate was scoring the bucking rubber the safety knife kept getting stuck. They squeezed the handle 3 more times, & on the 3rd attempt the blade popped out and was too close to their inner thigh causing the blade to cut through pants leaving a ¾ of an inch cut in their thigh.
ASSOCIATE WAS REPLACING A TOOTH ON A CONCRETE MUNCHER. WHILE HOLDING THE RETAINING PIN WITH LEFT HAND & SWINGING A HAMMER WITH THE RIGHT, EE GLANCED OFF THE PIN & HIT LEFT HAND.
An EE was operating the Cat skid steer, as EE was backing up, the skid steer and the Dingo IE was operating made contact.

Fire detected by evening cleaning crew in shop. Hot work being performed on the haul truck earlier in the day could have been a factor.
Associate came up to the metal yard to pick up a couple pieces of metal for a chute rebuild. As they started sliding the large angle iron off the sheet of metal they needed, they lost track of their surroundings & pinky figure was pinched between angle in their hand and another piece of metal behind them. Employee was wearing neoprene coated fabric gloves.
BECAME REPORTABLE ON 2/15/2022 WHEN EE HAD TO ATTEND PHYSICAL THERAPY While climbing down from haul truck, boot lug caught & caused EE to lose EE's grip with EE's left hand & grabbed with right hand causing a strain/injury.

When cutting a lifting eye in the welding shop, EE knocked the center piece free with a chipping hammer, the hot metal fell on EE's pant leg, burned through the material and fell into the top of EE's boot.

Associate was attempting to dislodge a rock from a conveyor with a 24"" bar. The associate's hand got pinned between the bar & the conveyor.
While operating a manlift from one location to another, driver hit a dip that caused driver to bounce up with left foot landing on the material in the basket resulting in a sprain to calf from the twisting. We are reporting this incident because driver was prescribed physical therapy that driver attended on 7/30/20.

While walking to the truck for tools & supplies, EE walked through a wet & muddy area & slipped, causing EE to fall on knee.
While working to flip a jaw dye at the Granite Plant, EE hopped out of the crusher and was walking on the surrounding catwalk when EE's boot slipped and EE's leg extended forward, causing pain in knee. EE indicated the sole of the boot was in disrepair and caused the incident. EE did not alert supervisor or plant management of the injury.
When moving a crane from work area, our associate began to raise the crane unaware that one of the hooks from a two-way chain had become lodged under the flange of a beam. This created tension on the chain until it dislodged and flew upward. The chain hook struck the associate in the forehead.

Employee was removing a drive belt when employee's right pinky was caught between the pulley & belt. The injury caused a hematoma to the tip of finger, X-rays were negative for fracture/break

3 7/16"" by 60"" shaft resting on 8x8x16"" wood block to allow grinding of end. Shaft rolled off block striking left middle finger when employee tried to stop shaft. Left hand middle finger received cut requiring medical attention.

Employee was on ground wielding lifting eyes on Jaw Cheek plates. The plates were stacked on top of each other and as he would finish wielding one he would slide the plate off to the side. During this process one of the plates fell on his right foot smashing his right toe causing a Closed Fracture.
Employee was completing his normal daily task of cleaning around conveyor 19 when he heard a noise coming from a return roller. Employee reached up to a return roller to remove a rock. The return roller caught his sleeve and pulled his arm into the roller causing the belt to burn his right arm.
Crane was lifting a 60ft section of truss conveyor with two spreader bars equally spaced for balance. C-clamps were attached to spreader to prevent movement of truss. One clamp slipped allowing the conveyor to shift slightly. The employee was in a manlift basket to help guide the conveyor. The shift in the conveyor struck his left hand which was on the safety rail.

Employee was preparing a cable belt for splicing by using a hook knife as recommended by the manufacturer to perform that process. The hook knife slipped off the cable, and forward momentum carried the knife into the associates left forearm requireing stitches. Zero lost work time.
Employee was climbing down ladder at cone crusher when he stepped on a rock at the base of ladder twisting his right ankle.

In preparation for painting a new conveyor, one end was resting on a cross-tie and the other end was raised by placing fork lift forks under a tail pulley. As the conveyor was raised to install blocking, the conveyor rolled off the forks striking the left ankle of the individual, fracturing his ankle.

Employee fell off 4' retaining wall while cleaning conveyor chute and suffered a lumbar strain/ back contusion
Employee was using a come-a-long to pull two I beams together while wielding a cross beam. The anchor point broke and the come-a-long popped back and hit his right hand.
The employee was walking by the boom truck when the hook struck him on the right side in the ribs.

Employee was removing grizzly bars. When he removed a pin, the bars fell striking him in the side of the face and neck.
Employee was preparing to place platform in place for stacker. Employee stepped close to edge of one block, which was four feet from concrete floor. A 10 inch by 22 inch piece of concrete block broke off, causing employee to fall to the ground, injuring his left hand and left knee.
