On 1/2/2025 employee was removing QR caps from a rotary crusher. As EE was removing one of the caps it fell before EE had a good grip smashing EE's right middle finger between the cap and housing. EE received 4 stitches and also a fracture. Employee was on restricted duty for three weeks. Other workers were in the area. Communication and readiness were contributing factors.

On 5/12/2023 at approximately 4:40pm EE was removing a trough roller from under a crusher plant. While EE was pulling the roller EE mashed right thumb between the roller frame and the frame of the crusher. EE was treated at the ER and received 2 stitches and a fracture to a bone in thumb. EE was put on restricted duty.

EE reported on 11/14/2022 that EE had been injured on 11/10/2022. EE was working setting a chute, when EE shifted EE's weight on EE's legs and heard something pop in EE's right hip. EE was advised to seek medical treatment but declined. On 11/22/2022 EE sought out medical attention. EE has been referred for an MRI, diagnosed suspect labral re-injury.

Miner was working the maintenance crew installing a belt on a conveyor. EE was threading a belt around a tail pulley. While pulling on the belt the miner strained left hand causing swelling and pain.

Miner was assisting in a belt install on a conveyor. The belt was being pulled on with a rope. The miner placed a gloved hand on the head pulley to get a look at the progress. The rope broke under tension and struck the miner in the right hand. Miner sustained a fracture to the thumb of the right hand.

On 9/19/2019 EE reported at 10:00am they exited a loader put on a harness and went up on to a conveyor to clear off some rocks from a belt and a screen feeder. EE said they tweaked their back. EE called to report incident at 1:45. EE told that they had been having issues with back at home and wasn't sure where the injury happened.

SNI was cleaning up after finishing changing the conveyor on our #3 belt. He was carrying a board back to the service truck. As he placed the board onto the back of the truck, the back of his right hand made contact with a belt re-grooving tool that was cooling- leaving a 2 inch laceration.

Employee mentions that he was placing processed stone on to pallets and felt a quivering in the center of his lower back before it gave out on 6/12/07. The employee thought he was simply tired and sore. After rest and over a week of time his back was still sore with pain extending down his right leg. Incident reported on 6/21/07 to a Foreman.

EMPLOYEE WAS OPERATING A HAUL TRUCK ON 9/9/05 & A SMALL ROCK FELL FROM THE BED OF HIS TRUCK. HE EXITED HIS TRUCK & ATTEMPTED TO ROLL THE ROCK FROM THE HAUL ROAD WHEN HE EXPERIENCED A STINGING PAIN IN HIS GROIN AREA. HE IMMEDIATELY REPORTED THE INCIDENT TO FOREMAN. HE HAS BEEN INSTRUCTED IN PROPER LIFTING TECHNIQUES & IMPORTANCE OF ALLOWING LOADERS TO CLEAN UP SPILLAGE.
