Employee was cutting rubber with a utility knife for installation on a pulley. Utility knife slipped and cut his thigh. *Note* Employee states that he was cutting away from body.

Caught in, under or between two or more moving objects

Employee reports that has he was moving dimension stone rocks in a production area, two rocks pinched his right middle finger. The result was a fracture in his finger tip.

Over-exertion in lifting objects

Employee mentions that he was placing processed stone on to pallets and felt a quivering in the center of his lower back before it gave out on 6/12/07. The employee thought he was simply tired and sore. After rest and over a week of time his back was still sore with pain extending down his right leg. Incident reported on 6/21/07 to a Foreman.

Struck by flying object

Employee was standing outside of his assigned CAT 771D Off - Road Haul Truck and mentioned that a gust of wind blew some fugitive dust around which entered his right eye. He reported the incident to a Foreman on Monday, 6/4/2007. He was treated by a physician and a corneal abrasion of the right eye was diagnosis. Employee was given antibiotics.

EE was reaching from the exit belt side of the rock splitter machine underneath of the veneer gauge to remove a stone and mentions that his right pinkie finger was smashed between a rock being processed by the machine and the veneer gauge. *NOTE* He was wearing his leather gloves at the time of the incident.
