Showing all 8Employee was bucketing product out of bulk bag and transferring into a 55 lb bag and said when he twisted he felt a pain in his side/groin area.
Employee was bending down to open a small door on our derrick screening system when his lower back started hurting.
EE was working on the bagging line and bumped his hand on a metal table/ledge. He went to doctor a couple days later. He stated he was okay and then started complaining that hand was hurting again and went back to doctor. EE nor doctors have let given us any diagnosis.
The incident happened on Oct. 13, 12 @ 1:20 am. EE reported on 10/16/12 that he bumped his head on a pipe in the automated line while packing buckets of zeolite. He never reported to other workers that he was hurt or that he needed to see a doctor. I was not aware of medical treatment of any type until 10/24/12. He was wearing his hardhat at the time of incident.
EE was shoveling rock for approx 15-20 min then he set shovel down and walked down to supervisor and stated that his shoulder was hurting and his supervisor sent him to the hospital. According to Dr's findings ee had stated he had a shoulder that dislocated before he started work @ Bear River Zedlite
EE was shoveling when she strained her wrist.
EE was assisting on the contruction of the L-3 Raymond Mills. EE was lifting & moving ducting for the mill, in the process he strained his back.
