EE was standing by some pallets, moving a tarp when another ee was bringing pallets over. ee did not move and the other ee had pallets in front of self. EE did not see ee and ee did not move out of the way. Other EE hit ee ankle with tire.
Employee was working near the Raymond Mill by the feed conveyor/silo and was trying to break up frozen material.

Employee was bucketing product out of bulk bag and transferring into a 55 lb bag and said when he twisted he felt a pain in his side/groin area.
IE was up at the Raymond Mill by baghouse. Two other EE's and IE were feeding product through auger, auger was running and IE stuck hand in to move product and cut hand severely. He was immediately take to emergency room.
Employee was bending down to open a small door on our derrick screening system when his lower back started hurting.

EE was working on the bagging line and bumped his hand on a metal table/ledge. He went to doctor a couple days later. He stated he was okay and then started complaining that hand was hurting again and went back to doctor. EE nor doctors have let given us any diagnosis.
Employee was up on the landing with all other employees pulling tarps over rock pile. A rock came out from under him and he fell and sprained wrist. He went to the doctor the next day.
Employee was waling by warehouse and slipped and fell due to icy conditions. He has been to the chiropractor a few times but has not returned to work.

The incident happened on Oct. 13, 12 @ 1:20 am. EE reported on 10/16/12 that he bumped his head on a pipe in the automated line while packing buckets of zeolite. He never reported to other workers that he was hurt or that he needed to see a doctor. I was not aware of medical treatment of any type until 10/24/12. He was wearing his hardhat at the time of incident.

We were thawing out some barrels that were frozen inside a warehouse with a generator and kerosene heaters. The doors were closed and the EE's were going in and out of the warehouse only to check barrels. The IE was going in and out approx. 5 hours. He told his supervisor and was sent home. He went to the doctor @8:00p.m. and was released from hospital @ 9:30 a.m.
EE was shoveling rock for approx 15-20 min then he set shovel down and walked down to supervisor and stated that his shoulder was hurting and his supervisor sent him to the hospital. According to Dr's findings ee had stated he had a shoulder that dislocated before he started work @ Bear River Zedlite

EE was shoveling when she strained her wrist.
EE was assisting on the contruction of the L-3 Raymond Mills. EE was lifting & moving ducting for the mill, in the process he strained his back.
He was cleaning with line shutdown, then he went in & started the line & went back out to clean the tail roller & not sure how he got in the tail roller. The guard was off at the time of the accident & had been off for quite a while.

EE was grommeting 2lb bags and hit the foot pedat prematurely and the die came down and took off the tip of her right thumb.
The injuried EE was helping the lift operator with the load, not watching where they were at, the ee got his fingers caught between the forks of the lift and the spout of an auger causing lacerations on 3 fingers. EE recieved 30 stiches.

Walking to check generator at 2130. Stepped on rock wrong, fell to ground. Looked at ankle, was bent inward. Stood up and popped back into place.
Employee put his hand in his glove and was bitten by a brown recluse spider.
EE WAS GOING TO TURN ON THE PUMP TO THE DIESEL TANK. IT WAS A SNOWY MORNING. EE SLIPPED AND FELL ON THE PLASTIC LINING WHICH WAS SLICK DUE TO SNOW, CAUSING A BACK AND SHOULDER INJURY.
EE WALKING THROUGH YARD SLIPPED AND FELL ON ICE RESULTING IN LOST WORK DAYS DUE TO BACK STRAIN. EE IS STILL OFF WORK
