Showing all 15While closing the door of end loader with left hand and bracing self with right hand on the door frame, employee closed the door on right hand causing crushing injury & fracture to end of right middle finger.
Operator was attempting to replace drivebelts on a de-energized fan assembly for the dryer dust collection system. EE attempted to roll the belts onto the pulley when EE's left pinky became pinched between the belt and the pulley, severing the tip of that finger.
Employee was assisting with manual installation of v-belt onto drive motor pulley assembly when fingers were pinched between the belt being installed and the pulley surface. Employee was not aware of hand position relative to the belt being installed. Employee had hand in an exposed position. This resulted in a crushing type injury middle and index fingers.
The EE was manually lowering the dumper hopper attachment for the forklift and his index finger was pinched between the hopper handle and the forklift mast.
EE was lifting the end of a pipe to place it on the forks of the front end loader when he felt a strain in his back.
