Showing all 5The crew was positioning and leveling a screen deck prior to start-up. Employee was grasping a jack stand and positioning the stand under the screen deck, their hand was too high up on the stand when the screen deck was being lowered and pinched the palm side of the left index finger between the stand and the screen deck.
Manually loading screen into skid steer bucket. Wet and muddy conditions. Slipped in mud and caught right hand thumb in screen resulting in laceration and possible broken bone in tip of thumb.
Two employees were dismantling a conveyor belt stand to move portable crusher. The attachment points on the metal foot of the stand were rusted into place. When the foot came loose from the stand it fell on one employee's foot resulting in a fractured bone.
