Fall from scaffolds, walkways, platforms

The injured employee was standing on a small metal step platform next to the jaw crusher. When EE stepped back to one side of the platform, it flipped over causing the employee to fall forward and land partially on the ground and partially on the legs of the platform. The platform is 18 inches high and was placed on the ground.

Over-exertion in pulling or pushing objects

The employee was changing rock screens on a screen deck and was pulling on the welding side strap of a screen when they felt pain in their hand.

Caught in, under or between a moving and a stationary object

The crew was positioning and leveling a screen deck prior to start-up. Employee was grasping a jack stand and positioning the stand under the screen deck, their hand was too high up on the stand when the screen deck was being lowered and pinched the palm side of the left index finger between the stand and the screen deck.

The employee was using a sledge hammer hitting the inside of a cone. He missed the mark he was aiming for and the employee's right index finger was crushed between the handle of the sledge hammer and the rim of the cone creating accumulation of blood under the finger nail. The blood was drained by the treating physician, no other medical treatment is required.

EMPLOYEE WAS INSTALLING BOLTS IN CONVEYOR STRUT DURING SETUP. OTHER EMPLOYEES HAD HEATED THE STRUT WITH A TORCH TO STRAIGHTEN A BEND. EMPLOYEE REMOVED GLOVE TO HANDLE BOLT AND NUT. THE STRUT S HIFTED POSITION AND HE GRABBED IT TO STABILIZE. HE RECEIVED BURNS ON THE PALM OF HIS LEFT HAND.

2 PART EPOXY MATERIAL USED IN CHANGING LINES OF CONE CRUSHER. EE WAS NOT INVOLVED IN MIXING OR APPLYING MATERIAL BUT WAS APPARENTLY EXPOSED TO VAPORS THAT RESULTED IN CHEMICAL ALLERGY. SKIN RA SH APPEARED ON FACE, ARM, AND LEG.
