EE was inserting a keeper pin into the reclaimer sprocket, to prevent the sprocket from motion, so the necessary repairs could be made. As the pin was being inserted, the sprocket rolled, causing the pin to thrust upwards. Pinching the EE's left index finger between the pin and the guard for the reclaimer sprocket.

EE was closing the door to the passenger elevator. EE pulled the rope to close the door to the elevator, the door came down faster than expected and smashed EE's left hand between the top and bottom doors. Left hand smashed. X-ray revealed 2 broken bones in EE's hand.

Miner was performing work on a 770G rigid frame water truck when EE walked into the water fill pipe on the haul truck cutting EE's right eyebrow. Miner was sent to urgent care where EE received 5 stitches for the cut.

EE was helping to put parts in a crate and the part was set down on a sling. One employee lifted the part by the sling to move it off of the sling. At the same time EE reached down to move the part off of the sling. The part pinched EE's right pinky finger between the wood block and the part resulting in the smashed finger tip.

Miner's personal pocket knife came open in miner's pocket and tore through the material of miner's jeans exposing the blade. Upon sitting down the miner ran hand down leg contacting the exposed blade and cutting hand. 3 stitches were given by medical to close the wound.

Miner was adding engine oil to a 775G haulage truck. As EE was finishing the task, EE closed the engine compartment access hood/door. EE wasn't aware that EE's right thumb was placed on a pinch point area and struck EE's right thumb causing a laceration. EE was treated and received stitches. Miner was given restricted duty.

Miner was climbing the ladder of a 775 haul truck during servicing of equipment. While climbing miner felt a pull in lower left abdomen. Upon medical review miner was given restricted duty for a strain.

EE reported pulling a muscle in EE's left upper arm on 05DEC21 while removing a tail pulley guard. Incident was initially a first aid only/ no medical case. On 21DEC21 EE requested medical evaluation because EE's arm still hurt. EE was medically evaluated and diagnosed with a strain of the left deltoid muscle. EE was prescribed P/T and released to work with restrictions on 21DEC21.

EE Was cleaning under mill rolls in preparation to set shims when dust fell from above and got behind EE's safety glasses into EE's right eye. EE flushed EE's eye on site but still had discomfort so EE was taken to be evaluated by the eye Dr. EE was treated, prescribed eye drops and released to work with no restrictions.

EE was climbing down the ladder on the side of the scrap steel dumpster and over stretched getting to the trailer deck. EE was medically diagnosed with a groin strain and is being monitored by the Dr. for further evaluation/diagnosis.

While miner was greasing mobile equipment miner went to step back into service truck and felt a pop in lower back. Miner was sent to medical for evaluation as miner requested and was placed on restricted duty due to a reported strain of muscles in lower back by medical.

EE was holding onto a shackle while getting down from a roll assembly. The tip of EE's left middle finger got stuck in the pin hole of the shackle (the pin was removed) and EE fractured the tip of the left middle finger. EE was diagnosed with a fracture of the left middle finger tip and returned to work with restrictions.

Miner was removing a 12" stuck rock off the grizzly bars from the jaw crusher when the rock shifted. As a result the rock struck miner's left ring finger cutting the finger requiring stitches to close the wound.

EE was stowing the outrigger pads for the RT crane. As EE lifted an outrigger pad (Pad weight was 86lbs.) EE heard a loud pop in their left shoulder. EE was taken for medical evaluation where EE was scheduled for an MRI and released to work with restrictions.

While EE was changing out hammers at the rotary crusher EE caught L-thumb between the old hammer and the rotor as the hammer shaft was being removed. EE was taken to clinic for medical evaluation and diagnosed with a crushing/laceration injury of L-thumb. X-ray negative for fracture, soft tissue damage only.

EE was changing liners on RM-450. (Vertical Mill) The liners had worn extremely thin and as EE gripped a liner to lower it, it slipped in EE's hands and cut through EE's leather glove causing a small laceration to EE's left pinky finger. EE received 3 stitches and was released to work without restrictions.

Miner was replacing cutting edges on a 980G loader and in the process EE was pulling the center plate from underneath the bucket so it can be picked up with a boom. As the miner released the cutting edge and stood up, EE felt a "pop" in the lower right side of the back. This resulted in a muscle strain in EE's lower back.

EE was unplugging an air slide when material puffed and they got cement dust behind safety glasses. EE was unable to remove all of the material with flushing and was taken for medical treatment and eye evaluation. EE was diagnosed with a corneal abrasion to the left eye.

Employee was installing return rollers when they felt a pop in right groin area. Miner reported it as a first aid only report at the time. On 03/24/2020 the miner requested a medical evaluation due to it not healing. EE was sent to urgent care where they were placed on restricted duty pending a specialist follow up for a hernia.

At the end of the miners shift on 9/12/2019 the miner felt EE had something in EE's right eye. The next morning 9/13/2019 at work EE's right eye was red and sore so EE requested medical evaluation. EE was sent to the doctors office where the doctor prescribed eye drops to be placed into EE's right eye.

Miner was shoveling under a Jaw Crusher when they strained their left wrist and right elbow. Employee was sent for medical evaluation and placed on restricted duty. Miner returned to work and is currently working restricted duty.

EE got CKD on socks on 5/13 and was instructed to cleanup & change socks. EE did not change socks. On 5/15 it was discovered that EE had alkali burns around both ankles (in the crease where the socks gathered). EE was sent to the company Dr. and taken off work for 3 days to heal.

EE was picking up cube molds from a shelf (43.2lbs) when EE felt a sharp pain in back. EE was sent for medical evaluation on 6/4, diagnosed with a back strain, kept off work and referred for P/T. Case is ongoing.

Miner was carrying a stack of five 12" x 12" screen panels weighing approx 25 lbs up the stairs to the screen deck. At the top of the stairs when the miner attempted to raise the panels above their head to place them onto the screen deck they felt a pop in the lower back. This resulted in a strain to the miners lower back.

EE was preparing to rig a rotary hammer for replacement in the mill when EE inadvertently knocked another hammer off of the edge of the pallet and it rolled onto EE's right foot. EE was diagnosed with a small toe fracture on right foot. EE was wearing steel toe boots at the time of the incident but the injury was outside of the protection of the toe cap.

EE was removing a steel jig with another employee and asked helper to release the end helper was holding. The injured employee (who was only using one hand) lost grip and dropped the jig. As the jig hit the ground and rolled, it struck EE in the mouth knocking out one tooth and cracking two others. EE was medically evaluated, cleared and sent to a dentist for dental repairs.

6/6/2016 Miner pinched right middle finger between a clamp rail and angle iron while changing screens when EE laid the rail onto the screen box to use later. Miner refused medical treatment and it was reported as a first aid only that day. On 6/8/2016 miner went to the hospital to have fingers looked at and it was reported by the hospital as broken.

EE reported a muscle strain to EE's left shoulder while transferring steel parts from one pallet to another on 28JAN16 but declined medical evaluation or attention at that time. On 16FEB16 the EE requested to be medically evaluated for the strain. EE was taken the Dr., diagnosed with a "Should Sprain" and returned to duty with lift restrictions making this case MSHA reportable.

Was preparing to repair Komatsu 750 tracks. Placing chock blocks in front/rear of the tracks. Discovered something unusual next day went to doctor. Possible hernia. An MRI ordered; employee immediately placed on "no lifting status." Not scheduled to work 11/13/15 (Fri normal day off). Reported to work on Monday. Allowed if he didn't lift anything.

EE smashed the tip of his middle finger on his left hand while reinstalling a spring box. No laceration or fracture. Diagnosis was a dislocated knuckle. Initial treatment was to immobilize the fingertip and observation. No restrictions. On 29JUL15 during a follow-up visit the attending PA-C placed restrictions on the use of his left hand making this injury reportable.

Miner (A) was helping another miner (B) lift and position a 3/8"x50x40 piece of plate steel onto a cut table to make cuts on. Miner B felt his hand starting to get pinched so he let go of the metal plate. This resulted in the metal plate falling onto miner A's left ring finger resulting in a cut and fracture of the finger tip.

On February 25th, three workers were unloading an excavator bucket from a trailer deck. Injured employee saw that the load wasn't center. As he grabbed hold of the chain to center the load, co-worker lifted the bucket catching injured employee's middle finger between the bucket and chain.

EE was removing a metal door from a damper and felt a "strain" in his back while he was holding it up. He did not report the incident until the following day when he requested to be medically evaluated. EE was sent to the Dr., diagnosed with a back strain and was returned to work with restrictions and follow-up PT.

EE was helping clean a plugged pipe and material blew behind his safety glasses and got into his eyes. He flushed and was sent to the Eye Dr. for evaluation. EE was diagnosed with mild scratching and irritation and returned to work. Treatment included another flush and application of a prescription medication to prevent infection. No foreign material was embedded or removed.

Miner was lifting a metal object when he felt a pop in his right shoulder. No medical treatment was required. There was no lost time or restricted duty and no doctors visits. 11/12/2015 Miner went to medical doctor and was given a shot of cortisone for this old injury. As of 11/13/2015 there has been no restricted work days and no lost time for this injury.

On 3/20/2014 ee was shoveling at the cone crusher and other areas of the plant. While shoveling he felt a pain down his right leg coming from his lower back. It was late in his shift so he sat down the rest of the day. On 3/24/2014 ee notified safety of the incident and was sent to the doctors. The doctor placed him on restricted duty until he his back and leg feels better.

EE Was helping another employee "walk" a 350lb hammer (for a rotary mill) onto a pallet when he heard and felt a "pop" in his right shoulder. EE reported the incident to his supervisor on 02/14/14. EE was sent for medical evaluation and diagnosed with a Right shoulder strain/sprain, prescribed PT and returned to work with restrictions.

EE was positioning plate steel on the layout table and felt pain in his lower back. EE reported the incident as a first aid case on 07NOV13 and declined medical treatment. On 12NOV13 EE was still experiencing pain in his lower back and requested to be medically evaluated. EE was taken to the Dr., diagnosed with a lumbar strain, prescribed PT and returned to work with restrictions.

EE was trying to lift a shim on a thrust roll and felt a pop in his right arm. EE did not think he was injured or request medical evaluation until 07/02/13. At that time he was diagnosed with a "strain", returned to work and scheduled for follow-up. On 07/24/13 he was diagnosed with Other tenosynovitis of right hand and wrist and given restrictions affecting his duties.

EE was driving his pick-up truck to his task when a gust of wind blew dust into the cab. EE noticed irritation and swelling in right eye. He flushed on site and was sent to the Co. eye Dr. for evaluation. No foreign material found in his eye. EE was diagnosed with "Allergic Conjunctivitis", given a prescription and released to work without restrictions.

While using the batch plants wash rack hose one handed (Right Hand) to clean the street sweeper out, there was a surge in the water pressure. This resulted in a jolt in the water line that employee was holding causing his arm to receive a strain. The surge in water pressure resulted from another hose being shut off delivering full pressure to the hose employee was holding.

While working on Stacker # 21-117 and trying to telescope in the tail section you must remove the (2) locks on the idler pulley sections to fold them over. Employee removed the first lock when one of the idlers sections unexpectedly folded over and contacted his left arm resulting in a bruised arm.
