Checking sheeting on roof to see if placed right to screw down, there was condensation on roof sheeting and EE started sliding off the roof and landed on EE's feet.

EE was inserting a keeper pin into the reclaimer sprocket, to prevent the sprocket from motion, so the necessary repairs could be made. As the pin was being inserted, the sprocket rolled, causing the pin to thrust upwards. Pinching the EE's left index finger between the pin and the guard for the reclaimer sprocket.
EE was closing the door to the passenger elevator. EE pulled the rope to close the door to the elevator, the door came down faster than expected and smashed EE's left hand between the top and bottom doors. Left hand smashed. X-ray revealed 2 broken bones in EE's hand.
EE was hitting a stuck drill head hammer with a pin hammer and a brass hammer. A piece of the drill head hammer metal fractured off, and entered EE's left leg on EE's inner thigh between EE's knee and groin. Doctor's wanted to wait a few days before performing the removal of the metal, in order to have a good OR room for the removal.

EE was helping to put parts in a crate and the part was set down on a sling. One employee lifted the part by the sling to move it off of the sling. At the same time EE reached down to move the part off of the sling. The part pinched EE's right pinky finger between the wood block and the part resulting in the smashed finger tip.
Originally went to ER. Was released to return to work. After follow-up with ortho DR. was placed on restricted duty on 11/06/24. EE was walking to their work truck when they tripped on some uneven concrete and fell. Injuring their right shoulder.
EE was closing the doors on the passenger elevator and got right hand and middle finger caught between the doors. EE reported the incident on 9/18 as a first aid. On 09/23 EE requested to see a Dr. EE got an exam and X-ray. X-ray was negative. On 09/27 EE went for follow up and was placed on restricted duty because finger looked infected and was given an antibiotic.
EE was conducting a preventative maintenance check on some belt lines. EE reported a trough roller had a hole in it. EE said EE used a pry bar to check for the hole in the roller. During this check, EE's sleeve was unbuttoned, and EE's sleeve was caught by the roller and belt and pulled EE's arm between the conveyor belt and a trough roller. EE pulled pull cord to free self
EE was given an annual audiogram. The first one EE agreed to have done since 2011. EE was reported to have an STS in right ear.
EE was given an annual hearing test. EE had chosen to not have a hearing test conducted since 2009. EE was reported with an STS in both ears. Previous hearing test results do not seem accurate from 2009.

EE was walking routine for Preventative maintenance. EE came down a flight of stairs. At the bottom, EE turned to the right and started walking. EE stepped on an uneven joint in the concrete and twisted EE's left ankle.
EE was inspecting a chute and gate area. EE entered the gate area, and the gate cycled. EE was pinned between the gate and the chute. EE sustained some crushing injuries. The employee sustained a fractured rib.
The EE was standing next to the cart located at the bottom of the preheater tower, having a conversation with the Manager. EE was wearing eye protection, but a gust of wind blew calciner dust off the deck above the EE, and into EE's right eye. EE reported it as a first aid, but the next morning due to irritation, went to the Dr. EE was prescribed anti-inflammatory eye drops.
EE was inspecting RM-306(raw mill) roller deck. EE tripped on a support bracket for the lubrication system lines. The tripping motion caused the EE to fall forward into the rocker arm assembly, causing EE's right arm to strain backwards causing shoulder pain. Originally reported as a first aid, but due to continued discomfort medical attention was sought on 5/10/23.
EE was stepping down the metal stairs from bucket wheel on reclaimer. EE went to turn to walk the to the ladder, the EE's left knee gave out and caused EE sharp pain in EE's knee. On 3/20/23 EE said knee was still sore, and a little swollen. EE went to Doctor, and returned to duty with restrictions. No kneeling, squatting, climbing.
EE reported on 1/16 that EE hit EE's head on a beam, the EE was wearing a hardhat. Reported as a first aid only. On 1/17 EE requested to be evaluated, because of pain. Diagnosed with a neck strain and released to work with restrictions. EE was released for full duty on 01/21.

Cleaning area with forklift, employee attempted to lift forks over the divider, forks were not able to lift high enough resulting in the fork to drop back down smashing employee's left ring finger.

Incident on 16SEPT2022 was originally a first aid only/no medical case after a 35lb. jack hammer went thru some loose material pulling down on EE's r-shoulder. On 19OCT2022 EE requested medical evaluation because EE still had pain. EE was medically evaluated and released to work the same day (19OCT2022) with restrictions awaiting follow up appointment.
EE felt a pop and pain in right ankle when standing up from desk. Suspects related to a F/A only on the same ankle on 04/11/22. Requested medical evaluation. EE was diagnosed with a strained calf muscle and ankle ligament and returned to work with a brace and restrictions. Treatment is on going.

EE reported pulling a muscle in EE's left upper arm on 05DEC21 while removing a tail pulley guard. Incident was initially a first aid only/ no medical case. On 21DEC21 EE requested medical evaluation because EE's arm still hurt. EE was medically evaluated and diagnosed with a strain of the left deltoid muscle. EE was prescribed P/T and released to work with restrictions on 21DEC21.
EE Was cleaning under mill rolls in preparation to set shims when dust fell from above and got behind EE's safety glasses into EE's right eye. EE flushed EE's eye on site but still had discomfort so EE was taken to be evaluated by the eye Dr. EE was treated, prescribed eye drops and released to work with no restrictions.
EE was climbing down the ladder on the side of the scrap steel dumpster and over stretched getting to the trailer deck. EE was medically diagnosed with a groin strain and is being monitored by the Dr. for further evaluation/diagnosis.
EE was holding onto a shackle while getting down from a roll assembly. The tip of EE's left middle finger got stuck in the pin hole of the shackle (the pin was removed) and EE fractured the tip of the left middle finger. EE was diagnosed with a fracture of the left middle finger tip and returned to work with restrictions.
EE was stowing the outrigger pads for the RT crane. As EE lifted an outrigger pad (Pad weight was 86lbs.) EE heard a loud pop in their left shoulder. EE was taken for medical evaluation where EE was scheduled for an MRI and released to work with restrictions.
EE was trimming conveyor belt sections in preparation for a job when EE's hands slipped and EE's utility blade malfunctioned releasing the blade. EE sustained a small puncture injury to EE's left palm. EE was taken for medical evaluation where EE received 4 stitches and was released to work without restrictions.

While EE was changing out hammers at the rotary crusher EE caught L-thumb between the old hammer and the rotor as the hammer shaft was being removed. EE was taken to clinic for medical evaluation and diagnosed with a crushing/laceration injury of L-thumb. X-ray negative for fracture, soft tissue damage only.
EE was changing liners on RM-450. (Vertical Mill) The liners had worn extremely thin and as EE gripped a liner to lower it, it slipped in EE's hands and cut through EE's leather glove causing a small laceration to EE's left pinky finger. EE received 3 stitches and was released to work without restrictions.
EE was driving a wedge behind a ball mill liner to remove it when EE accidentally struck EE's left hand with a hammer. EE was taken for medical evaluation and diagnosed with a contusion. X-ray was negative for fractures. EE was on restricted duty for 4 days.
A brush fire started along the railroad tracks adjacent to the facility (on mine property but not part of the mining operation) as the Railroad company was moving rail cars. Fire discovered at 12:23. Reported to MSHA 800# (not extinguished in 30 min) @ 12:56. Fire extinguished at 13:35. Fire did not make it into the plant. No injuries, no plant equipment involved.
EE was doing a PM on DBC-404 when the wind blew something behind EE's safety glasses and into EE's eye. EE was unable to flush it out because EE rubbed the eye before using eyewash. EE was sent to the eye Dr. who removed a foreign body with forceps, prescribed antibiotic eye drops and released EE to work.
EE was unplugging an air slide when material puffed and they got cement dust behind safety glasses. EE was unable to remove all of the material with flushing and was taken for medical treatment and eye evaluation. EE was diagnosed with a corneal abrasion to the left eye.

Employee tripped over material discharge hose while operating gunite machine, landing on left wrist.
EE got CKD on socks on 5/13 and was instructed to cleanup & change socks. EE did not change socks. On 5/15 it was discovered that EE had alkali burns around both ankles (in the crease where the socks gathered). EE was sent to the company Dr. and taken off work for 3 days to heal.
EE was discovered unresponsive laying on the floor in the restroom. Site personnel began CPR and called EMS. EMS took over life saving efforts upon arrival but were unable to revive EE.

Keller Electrical employee was directing coworker who was driving a pickup truck. Coworker turned the wheel of the truck and the tire drove up the right leg of employee causing employee's right fibula to fracture and other lower right leg injuries.
EE was picking up cube molds from a shelf (43.2lbs) when EE felt a sharp pain in back. EE was sent for medical evaluation on 6/4, diagnosed with a back strain, kept off work and referred for P/T. Case is ongoing.
Contractor EE was stripping wire at the MAAG MCC when knife slipped and cut right index finger. EE was taken for medical evaluation where EE was diagnosed with a laceration to the right index finger, given 8 stitches and returned to duty with restrictions. EE returned to full duty on 05/03/2018.

EE was hammering out a jib pin when a piece of foreign material (presumably paint) went under safety glasses and into left eye. EE was sent for evaluation and diagnosed with a scratch to the cornea of left eye. No material was found or removed from eye and EE was released with no restrictions. EE was issued prescription eye drops making this event reportable.
Fire extinguisher on pickup truck did not have a visible annual inspection conducted. No damage or impairment resulted at this time.
EE was preparing to rig a rotary hammer for replacement in the mill when EE inadvertently knocked another hammer off of the edge of the pallet and it rolled onto EE's right foot. EE was diagnosed with a small toe fracture on right foot. EE was wearing steel toe boots at the time of the incident but the injury was outside of the protection of the toe cap.
EE walked a few feet to pick up a caulking gun laying on top of a screw housing (EE did not have to bend over) and when EE turned to walk back the other direction EE felt a pain in the middle back. EE was sent for medical evaluation, diagnosed with a back strain and released to work with restrictions.
EE was removing a steel jig with another employee and asked helper to release the end helper was holding. The injured employee (who was only using one hand) lost grip and dropped the jig. As the jig hit the ground and rolled, it struck EE in the mouth knocking out one tooth and cracking two others. EE was medically evaluated, cleared and sent to a dentist for dental repairs.
EE reported noticing stiffness in left knee during shift (no accident/ event) on 22JAN17 as a first aid only and wanted to wait and see if it went away. EE was still experiencing intermittent stiffness and discomfort and was sent for medical evaluation on 07FEB17. EE was diagnosed with a strain/sprain of the left knee and released to work with restrictions.

EE was exiting the quarry office building and fell onto the platform/steps outside the doors. EE cut EE's right shin and was sent for medical evaluation. EE received 5 stitches to close the wound and was returned to duty without restrictions.
On 7/18/16 EE filed a F/A report stating that EE strained EE's left shoulder while using a pole hook to align a loading spout into a bulk trailer. EE declined to be medically evaluated at that time. On 7/28/16 EE requested medical evaluation and was sent to Occ Med. EE was diagnosed with a muscle strain and returned to duty with restrictions making this case reportable.
Unplanned fire not extinguished within 30 minutes of discovery. No plant equipment or structures involved. Discovered at 15:00, Reported to MSHA 800# at 15:33, fire out at 15:36. Fire is believed to have been started by locomotives of the local railroad company that was making a delivery. Fire involved only brush in the wash adjacent to the railroad tracks entering mine property.
EE reported to supervisor on 08APR16 that EE had been welding on 07APR16 and EE was o.k. at the end of EE's shift but by bedtime EE's eyes began to bother EE. EE was taken to the eye Dr. where EE was diagnosed with Superficial Keratitis and foreign material was removed from EE's left eye. EE was issued a prescription to prevent infection and returned to work without restrictions.
EE reported a muscle strain to EE's left shoulder while transferring steel parts from one pallet to another on 28JAN16 but declined medical evaluation or attention at that time. On 16FEB16 the EE requested to be medically evaluated for the strain. EE was taken the Dr., diagnosed with a ""Should Sprain"" and returned to duty with lift restrictions making this case MSHA reportable.

He was loosening a bolt with another employee when the bolt fell out of the impact wrench and hit him in the mouth.
EE was tightening a bolt with a 3/4"" ratchet when he felt a ""pop or pull"" on the right hand side of his chest around the lower ribs. EE was sent for medical evaluation and returned to work with restrictions. Diagnosis pending.
The JTT miner was doing demolition. When chipping, the miner lost his grip on the chipping gun, causing him to lose his balance. Stepping forward to regain his footing, the miner bumped his right knee on a scaffold pipe.
EE smashed the tip of his middle finger on his left hand while reinstalling a spring box. No laceration or fracture. Diagnosis was a dislocated knuckle. Initial treatment was to immobilize the fingertip and observation. No restrictions. On 29JUL15 during a follow-up visit the attending PA-C placed restrictions on the use of his left hand making this injury reportable.

EE was pulling a grease line to the Primary Crusher when he missed a step and reported straining his neck while catching his balance. EE was taken to the Dr., diagnosed with a neck muscle spasm and released to work with restrictions.
EE was removing a metal door from a damper and felt a ""strain"" in his back while he was holding it up. He did not report the incident until the following day when he requested to be medically evaluated. EE was sent to the Dr., diagnosed with a back strain and was returned to work with restrictions and follow-up PT.
EE was helping clean a plugged pipe and material blew behind his safety glasses and got into his eyes. He flushed and was sent to the Eye Dr. for evaluation. EE was diagnosed with mild scratching and irritation and returned to work. Treatment included another flush and application of a prescription medication to prevent infection. No foreign material was embedded or removed.
EE was removing an oil drain plug on a gear box when his allen wrench broke and he fell backwards into a grease fitting which struck him in his back just below the right shoulder. EE was diagnosed with a contusion/ myalgia and released to work with restrictions.
EE was coming down the stairs from the second level on R-202 and reported losing his footing and feeling a ""pop"" in his right shoulder as he held on with his right hand to steady himself. EE reported as a First Aid Only case on 06/21/14 and did not seek medical evaluation until 06/27/14. He was diagnosed with a ""right shoulder strain"" and released to work with restrictions.
EE was using a box knife to cut a hose and the knife slipped in his hand causing a laceration to the tip (pad) of his right index finger. EE received 6 stitches and was released to work with no restrictions. EE was not wearing gloves at time of incident.
EE Was helping another employee ""walk"" a 350lb hammer (for a rotary mill) onto a pallet when he heard and felt a ""pop"" in his right shoulder. EE reported the incident to his supervisor on 02/14/14. EE was sent for medical evaluation and diagnosed with a Right shoulder strain/sprain, prescribed PT and returned to work with restrictions.

Walking on platform and slipped on klinker and fell forward twisting ankle.
EE was positioning plate steel on the layout table and felt pain in his lower back. EE reported the incident as a first aid case on 07NOV13 and declined medical treatment. On 12NOV13 EE was still experiencing pain in his lower back and requested to be medically evaluated. EE was taken to the Dr., diagnosed with a lumbar strain, prescribed PT and returned to work with restrictions.
EE reported straining his back while helping carry a section of Vactor hose. EE was sent to Company Dr. and diagnosed with a back strain and returned to work with restrictions.
EE was drilling a hole in a piece of steel and the drill bit stuck and the drill twisted out of his hands. He was diagnosed with a metacarpal bone fracture in his right hand and returned to work with restrictions.
EE was trying to lift a shim on a thrust roll and felt a pop in his right arm. EE did not think he was injured or request medical evaluation until 07/02/13. At that time he was diagnosed with a ""strain"", returned to work and scheduled for follow-up. On 07/24/13 he was diagnosed with Other tenosynovitis of right hand and wrist and given restrictions affecting his duties.
This is a re-occurrence and further treatment of an old injury. There is no new incident/injury event. Original MSHA 7000-1 was closed on 03/08/1995. EE successfully petitioned Worker's Comp to have his claim re-opened and as of 04JUN13 he is receiving further medical treatment for the original injury.
EE was changing an air seal on a pump and using a piece of round stock and hammer to rotate the shaft collar into position. The round stock dislodged after being struck and hit the EE in the lower lip causing a laceration. EE was taken for medical evaluation, received 1 (one) suture and released to work with no restrictions.
EE reported falling on 29Mar13 but did not seek medical attention until 22Apr13. EE was opening a door to a storage shed when wasps flew towards him. As he was stepping backwards to get away from the wasps, he tripped over some pallets and fell to the ground. EE was diagnosed on 22Apr13 with a contusion to the right shoulder and released to work with restrictions.
EE was driving his pick-up truck to his task when a gust of wind blew dust into the cab. EE noticed irritation and swelling in right eye. He flushed on site and was sent to the Co. eye Dr. for evaluation. No foreign material found in his eye. EE was diagnosed with ""Allergic Conjunctivitis"", given a prescription and released to work without restrictions.

EE was preparing to retrieve a material sample at mill and stepped on an unseen mill ball causing him to lose his balance. He caught himself without falling but reported feeling back and knee strain in doing so. EE was sent to the company doctor and diagnosed with a lower back and knee strain and returned to work with restrictions.
EE was rolling a piece of metal and his glove got caught between the metal and roller and his finger was pulled in. EE was diagnosed with a fracture to the right index finger and released to work with restrictions.
EE Was bent over showing mech. a problem with his Haul Truck when he felt a sharp pain in lower back. EE was sent for medical evaluation, diagnosed with lumbar strain. EE was released to work with restrictions.
EE was striking a slide gate with a hammer and he struck the tip of his left pinky finger with the hammer. EE reported the injury as a first aid on 10Oct12 and sought medical evaluation on 11Oct12. EE was diagnosed with a subdural hematoma and fracture to the tip of his left pinky and released to work with restrictions.
EE was walking down the stairs of the 992G loader while carrying a bucket and slipped from one step to the next step down. The steps were wet due to rain and his heel slipped from the edge of the traction control on one step to the next step down. (8""drop) He was using the handrail and did not fall. EE was diagnosed with a back strain and had work restrictions for 1 week.
When tearing out refractory a spring on the rivet buster broke. Employee after being told not to use lower exit climbed down the ladder, knocked off the rivet buster from the deck hitting himself on the upper shoulder and back.
EE was helping another employee lift a plate compactor over a concrete form when he felt a sharp pain in his lower back. He was sent to the doctor who diagnosed him with a back strain and released him to work with restrictions.
EE was looking into an access port inspecting refractory in the T/A duct. As he twisted his upper body to get a better view, he heard and felt a ""pop"" in his left shoulder and experienced pain in his left shoulder. EE saw company Dr. on 04APR12 and is initially diagnosed with a strain and released to work with restrictions. Final diagnosis is pending evaluation of MRI.

EE was lubricating a tail pulley and reported slipping in mud created by recent rain storms and twisting his left knee. EE reported the incident to his supervisor the following day 9/29/11 as a first aid only and did not request medical attention until 10/05/11. EE was diagnosed with a ""strain"" to the left knee and released to work with restrictions.
EE was carrying tools out of the clinker cooler, stepped on a large clinker ball and felt a ""pop"" in his left knee. EE declined medical treatment on the day of occurrence (29th) however he requested medical evaluation the following day (30th) and was diagnosed with a ""strain"" to his left knee and was returned to work immediately but with restrictions.
EE was pulling on a big wrench to tighten a nut when he felt a pop in his left shoulder. He initially reported it as only a first aid and did not think he needed medical attention. The following day he had pain and swelling in his left shoulder and was taken to the company doctor in the morning. He was diagnosed with an ""strain/impingement"", given meds and placed on light duty.
EE was helping two co-workers lift a jack stand to the deck of a maintenance yard crane (CO#MCRN-004) and as they were lifting it up he pinched his left hand between the jack stand handle and the front end of the yard crane. EE was sent to the company doctor and diagnosed with a contusion to the top of his left hand. He was released to work with restrictions.
EE was exiting the cab of the skid steer and felt a ""pop"" and sharp pain in his back as he stepped down to ground level. EE was sent to the company physician for examination and diagnosed with a ""strained"" back muscle. EE returned to work the same day with minor restrictions.

EE was operating the packer(bagging mortar). As he looked over the door to the carousel a defective mortar bag ripped and covered the EE with mortar cement. Some material got behind his safety glasses and into his left eye. The eye doctor found no embedded material however a precautionary prescription ointment was administered to his left eye to prevent infection.
EE was holding onto a pipe while he re-positioned his footing and felt a pop in his left wrist.
While sitting he turned his head to acknowledge a fellow crewman and felt tightness in his neck and shoulders.
As he was unloading a bit basket from the back of a pickup and onto the drill catwalk, he smashed his finger between the bit basket and the handrail of the drill catwalk.
While replacing a cotter pin he tweaked his back when he twisted to get into position.

Tacking rail in place and twisted knee moving to tack other side of railing.
While cutting scrap expanded metal to size down for disposal and as he went to move a motor and he caught his forearm on a jagged edge of a protruding piece of metal.
Employee was pressure washing a drill; he was lifting the engine cover when it slipped out of his hand. Employee's finger was cut on the index finger of the left hand between the first knuckle and finger nail. Employee was taken to emergency room and then sent to a larger facility where a pin was inserted into his finger.
Walking down the stairs he twisted his right knee.
Transferring parts from an old pallet to a new pallet he felt a sharp pain in his back. The employee refused medical attention at the time of injury. However he decided to seek medical attention for this injury at a later date and did not notify his employer until Wednesday August 20th, 2008.

He was dumping a wheel barrow in back of the sample station and felt a pop in his chest.
East hatch, was being opened, and there are 2ea of 1 1/2 nuts to keep the door closed. While removing west lock nuts the door broke loose to the inside of mill and caught the employee's finger between the bottom of the nut and the locking bar.
While he was mucking out the kiln the ramp collapsed beneath him and caused the uniloader to drop. at the time of the accident the employee did not feel the need for medical attention until 3/27/07.

As he went to return the fuel hose to the stand, he twisted his ankle and fell.
While splicing a hose, his hand was sucked into the hose, up to his shoulder.

He was removing the bucket tooth with a hammer, and as he struck the tooth a piece of metal flew off and hit him in the stomach.

Spotting bulk truck using sample spoon to align boot when boot started into trailer. Driver let clutch out and the truck lurched forward pulling the boot and sample spoon straining the employees wrist.
He was trying to stop a noise of an idler on the conveyor belt and his arm was pulled over the idler between the idler and the belt. This employee declined medical attention at the time of his injury, until Nov 6 he asked to see a Doctor for this injury.
He twisted his ankle on material that was on the road.
When placing the chocks under the haul truck wheel he twisted his back.
He was removing a tractor protection valve at the rear of the #3 Dart, when removing one of the air lines, air rushed out and hit him in the face, knocking off his glasses and sent particles into his eye.

As he dismounted the haul truck he felt a pull in the groin area.
Injured had been working to get the eductor unplugged. The task was completed and injured was removing his protective clothing when some dust on the outside of the clothing fell off and passed between his safety glasses and his face entering his left eye.
As he started the grinder the guard fell of and hit him on his left thigh.
Preparing to finish wire wheeling the seal of BM-304 door, injured was wearing safety glasses and when he lowered his face shield, clinker or cement dust fell into his eye behind his glasses.
Lifting a guard and he slipped on a piece of metal twisting his back as he tried to keep from falling.
While inside the surge bin jackhammering to unplug the bin, he stepped on a chunk of clinker causing his foot to slip and become wedged between the chunk of clinker and the bottom of the bin.
He was prying on a bar to tighten the belts on a fan, using a ratchet at the same time and strained his back.
When he lifted a sample bucket to dump into the reject bin he felt a sharp pain in his right shoulder. At the time of his injury he felt that he did not need medical attention. He decided to seek medical attention on 9/29/05.
Spider bite. While driving the 980G loader, pushing up the gyp shed, he noticed a burning pain and swelling on his left leg.
He was jackhammering out BC-203 transfer chute from the outside. The jackhammer fell back and smashed his finger between the chute and the jackhammer.

He was operating the skid steer and ran over a rock causing sharp motion and injuring his back.

He was using a jack hammer near a take up pulley. There was a backhoe working in the area that slid pinning the employee between the out rigger of the backhoe and the conveyor tail pulley guard.
He was air lancing the ends of BC-205, also jack hammered the end - was using his left arm to hammer with.
He hit the top of his hard hat on the door frame jamming his neck while getting into his work truck.
As he was walking down the stairs at the feed end of the kiln onto the ground level there is a 2"" drop from one concrete slab onto another and as he stepped off he twisted his left ankle.

He was lifting wheel chocks from holders mounted on the front of a truck approximately shoulder height. This caused a shoulder strain. Employee did not seek medical attention at the time of injury until January 6, 2005.
Closing inspection ports on the tower vessel, using a sledge hammer and pipes. the sledge hammer slipped striking him in the hip area.
When he finished fueling a cat truck, he disconnected the quick-connect hose. The spring got stuck causing the fuel to come back out of the tank splashing him from head to toe with fuel.

2 EMPLOYEE'S WERE CHECKING OUT BC213 WHEN THEY GOT TO HORSING AROUND. ONE STATED THAT THE OTHER WAS POKING HIM WITH A BAR, HE TOLD HIM TO STOP & HE DIDN'T. HE THEN THREW INJURED TO THE CATWALK.
While he was working in mud he stepped down on a rock and twisted his left ankle.

He was stacking bags of cement and as he attempted to stop a bag from falling on top of another bag this action caused him to strain his ribs.
HE WAS CLIMBING ON THE DRILL AND HIS FOOT SLIPPED, AND AS HE CAUGHT HIMSELF, HE PUT ALL HIS WEIGHT ON HIS RIGHT ARM AND HE FELT A SHARP RIPPING PAIN IN HIS SHOULDER.
He was installing liners in the lower wobbler chute and noticed that his lower back was bothering him. This employee didn't report the injury until 8/17/04.
Ee was heading towards the stairs that go to RM 305 to check area out. Before reaching the stairs ee stepped on a clinker ball that was on the concrete floor causing him to twist his right ankle.
EMPLOYEE WAS CLEANING OUT A RAIL CAR. THE MATERIAL WAS STUCK IN THE HOPPERS AT THE BOTTOM OF THE CAR. HE USED A SLEDGE HAMMER TO TRY TO CLEAN THE MATERIAL OUT AND WHILE DOING THIS HE FELT A STRAIN IN HIS BACK.
Employee was running a paletizer to stack cement bags on pallet; ee opened slide door to drop bags on to pallet, when slide door started to close it caught the left arm and wrist between a cement bag and slide door.
EMPLOYEE WAS MOVING TWO BUSHINGS FROM ONE LOCATION TO ANOTHER. HE WAS KNEELING AND PARTIALLY PICKED UP THE BUSHINGS AND SLID THEM INTO A CART, DOING THE SAME AT THE NEXT LOCATION. BEARINGS WERE APPROXIMATELY 50 LB. EACH. EMPLOYEE FELT A PULL ON HIS RIGHT SIDE.
Ee was filling out a truck log at the security office when a bug flew into her eye. Ee went to doctor & was given a prescription.
EE WAS CUTTING ZIP TIES FROM A HOSE AND THE KNIFE SLIPPED, CUTTING HIS FINGER.
THE FLOOR WAS SLICK AND AS SHE WENT TO STEP OUT OF HER OFFICE SHE SLIPPED BACKWARDS AND FELL AGAINST THE WALL AND TO THE FLOOR HITTING HER LEFT SIDE. THIS EMPLOYEE DID NOT SEEK ANY MEDICAL ATTENTION UNTIL 08/11/04
PART TIME SUMMER STUDENT. EE WAS CLEANING UP AROUND THE PLANT IN DIFFERENT AREAS. EE REPORTED THE NEXT DAY THAT HER FINGERS ON HER RIGHT HAND WERE SWOLLEN & HER ARM WAS SORE & IT HURT TO HOLD THINGS IN HER HAND.
EMPLOYEE WAS CHANGING OUT A DUST COLLECTOR BAG AND DURING THE PROCESS OF PULLING THE CASE OUT OF THE BAG, THE LEFT THUMB CAME IN CONTACT WITH THE METAL CAGE CUTTING IT.
EE WAS CLEANING CATWALK UP TO DOME. EE STARTED BACK DOWN ON A SLANTED CATWALK AND OVEREXTENDED HER LEFT KNEE COMING DOWN. EE FELT SWELLING AND BURNING SENSATION BEHIND HER LEFT KNEE.
THE KILN WAS IN AN UPSET CONDITION PUSHING HOT DUST THROUGH THE PAN CONVEYOR CATCHING THE RUBBER SKIRTING ON FIRE. THE EMPLOYEE WAS IN THE PROCESS OF EXTINGUISHING THE SMALL FIRE AND THE HOT MATERIAL SLUFFED OUT BURNING THE EMPLOYEE ON THE RIGHT LEG.
EE WAS KNEELING ON THE FLOOR CHANGING FILTERS OUT OF THE XRAY MACHINE. WHEN HE STOOD BACK UP HIS KNEE POPPED AND HE FELT A SHARP PAIN AND HE COULD NOT PUT ANY WEIGHT ON IT.
EE WAS LIFTING BUCKET SEGMENTS FOR E502. HE WAS BENDING DOWN TO MOVE SEGMENTS AND FELT A PULL IN HIS BACK.
""RAIN & MUD CONDITION"" EE WAS RETURNING A TROUGHING ROLLER SET TO THE BACK OF THE MAINTENANCE TRUCK. HE THEN BACKED AWAY FROM THE TRUCK AND SLIPPED IN THE MUD. WHEN HE STARTED TO SLIP, HE TRIED TO CATCH HIS BALANCE. HE THEN FELT AND HEARD HIS BACK POP AND INSTANTLY HAD LOWER BACK PAIN.
EE was getting off a rail car to disconnect empty car. it was a long step to the ground and ee's shoe caught on the step of the car, twisting his knee.
Employee walked into transformer, knee high when tracing conduit raceway above his head. Left knee struck the corner of the transformer & the blood vessel under the skin or knee cap was broken. Bruising the knee cap & leg. No blood was extracted through skin.
Dust fell off the inside of cab onto EE & into his eye. While moving piles of dirt in the storage area. Eye was flushed out & EE went home & came back to regular shift on the next day, he worked 1/2 his shift & then wanted to see a Dr. It was cleaned out and EE returned to work.
SERVICING 992 LOADER, REMOVING FILTER, PULLED ON WRENCH TO REMOVE FILTER, FELT A POP IN RIGHT SHOULDER.

WAS REMOVING CARDBOARD FROM SACK PALLET, PULLED AT WRONG ANGLE, FELT EXTREME PAIN IN LOWER BACK. OUR EMPTY BAGS COME ON A PALLET WRAPPED WITH PROTECTIVE CARDBOARD, EE TRIED TO RIP IT OFF INSTE AD OF CUTTING IT.
EE COMPLAINED OF WRIST PROBLEMS OFF AND ON FOR LAST FEW MONTHS. AS SEEN A DOCTOR ON 12-22-03 AND WAS DIAGNOSED AS HAVING CARPAL TUNNEL. THAT'S WHY I CAN'T GIVE A DEFINATE TIME OF INJURY. HE HA S LOADED BULK TRUCKS SINCE DAY OF HIRE, USING RIGHT HAND FOR CONTROLS.
CAT TRUCK WAS TRAVELING TO B-6 PIT. THE TRUCK SLID OUT OF CONTROL ON A MUDDY, ICY CURVE. THE 35T WATER TRUCK WAS COMING DOWN THE HILL FROM THE SAME PIT. THE HAUL TRUCK SLID SIDEWAYS, THE BED OF THE HAUL TRUCK STRUCK THE CAB OF THE WATER TRUCK.
