While doing a power move, employee was taking down power cable and felt a pain in his right shoulder. Employee has been working his regular job since the injury but was recently diagnosed with a shoulder tear requiring surgery. Surgery scheduled on 11/19/07 making this injury reportable.

EE felt a pop in her thumb while turning off water valve. EE worked until 10/10/07, when she went off for medical treatment, thus making the injury reportable.

Empoyee pinched finger while installing filter leaves requiring sutures.

Employee was pulling a feed tube funnel and pinched his hand between the funnel and the feed tube. EE's hand continued to bother him so he went to the doctor on 8/20 and they took x-rays. They determined he had a fractured 5th metacarpal.

Employee jammed left wrist while barring centrifuge restricted to no overtime for 10 days.

Anchor plate for sheave wheel pulled loose from weld causing cable to strike employee on the ear.

Employee cut finger on sharp edge of glass in door.

Employee was lifting a roll of 12-3 cable and felt a pull in his right groin area.

Employee was helping change out belt on conveyor and snub roller fell and hit him on the leg. He was given a restriction of no overtime for 10 days, making this injury reportable.

Employee smashed his thumb while installing a race ring in a centrifuge.

Employee was lifting tool box out of maintenance truck and twisted his back. Employee reported injury on 1/31.

Employee stated that he had knee discomfort from climbing ladder all day. Employee saw the doctor two days later and cannot work overtime for two weeks.

Employee was setting down an electrical cabinet and set the cabinet down onto his right middle finger causing a laceration requiring (2) two sutures.

Employee was dropping conveyor belting to the ground and belting whipped him in the leg causing a friction burn abrasion.

Employee was performing maintenance on a calciner bull gear and was struck L. the ankle by a piece of falling metal.

Employee was removing worn flights out of a screw and cut his hand on a sharp flight requiring sutures.

EE was loading roof bolts onto bolter in panel FM07. He had loaded about 60 bolts on the bolter when he felt pain in his right side, stomach area.

Employee was opening a dump valve and got splashed with hot liquid, causing 2nd degree burn on ee's left arm.

EE walked through hot ash and burned his right foot.

Employee was walking sam 1 beltline, training belt. Ee reached up to loosen a eye nut on a return roller basket standing on his tip toes to reach bracket. He felt a pain in his lower back. Ee did not go off work until 7/19/06.

Employee was lifting and hanging ventilation tubing and felt a pull in his right shoulder/neck area. Nov overtime restrictions.

Employee was hanging a stamler cable and as he pushed the cable up he felt a burning sensation in his right shoulder. Employee went in for shoulder surgery on 2/23/07 making this injury reportable.

Employee was removing an impeller off of a pump and cut his finger on a sharp vain on the impeller.

Employee was helping pull a piece of belt when he felt a pop or pull in his right elbow. Employee did not miss any time from work but was restricted from working overtime for two weeks.

Employee was lifting pipe and felt strain in his abdomen. This case is being disputed due to the fact that employee had a pre-existing hernia. Worker's comp. accepted this hernia as an exascerbation of a pre-existing condition on 5/24/06 determined to be work-related.

EMPLOYEE WAS ATTEMPTING TO MOVE A SIX FOOT STEP LADDER AFTER HE LEFT A HAMMER ON TOP OF THE LADDER. THE HAMMER FELL FROM THE LADDER STRIKING HIM ON THE UPPER LIP CAUSING A LACERATION.

EMPLOYEE WAS TROUBLE SHOOTING HYDRAULIC PROBLEM ON FB-1. HE PINCHED HIS LEFT MIDDLE FINGER. CUT FINGER BETWEEN GUARD & SHAFT.

EMPLOYEE WAS LIFTING 42" BELT TOP ROLLERS FROM BELT STRUCTURE TO PUT ON FORKS OF LO TRAC AND FELT A STRAIN IN HIS LOWER BACK.

Employee was lifting 42" belt top of rollers from belt structure to put on forks of the Lo Trac and felted a strain in lower back. Accident was reported and closed with MSHA. Miner is going to be off of work due to prior injury. March 21, 2017 was last day of work to start pre-op testing and surgery on March 22, 2017.

Employee strained his lower back while pulling bags on Hivac or running 6" hose on Hivac.

Employee picking up shaft and cut finger in key way.

EMPLOYEE WAS REMOVING THE NOZZLE TO THE ALUMINUM PIPING ON THE HI-VAC TRUCK WHEN HAND SLIPPED AND EMPLOYEE STRUCK LEFT THUMB AGAINST EDGE OF PIPE

EE WAS REMOVING A BELT CLAMP OFF BELT TRUCK. EE WAS STRUCK IN THE MOUTH BY ONE END OF THE BELT CLAMP, KNOCKING THE CAP OFF OF HIS CANINE TOOTH. EE DID NOT MISS ANY WORK.

Employee was performing maintenance on filter and filter leaf support bar fell onto ee's neck and shoulder area.

EE ALLEDGED THAT WHILE PULLING MINER CABLE OR UNLOADING PANS OFF ROOF BOLTER, HE FELT A SHARP PAIN IN THE SHOULDER BLADE AREA OF HIS MID BACK. EE DID NOT MISS ANY WORK, BUT WAS PUT ON RESTRICTED DUTY AND NO OVERTIME.
