Showing 50 of 444#3 Hoist kicked out on "Drive converter A" code. An electrician had to be called out of the mine to reset the hoist. #3 hoist was reset and back into service at 11:50 pm.
EE was coming out of the MCC on the 3rd floor of GR3 plant and looking up, when he tripped over a pump/motor that is welded to the floor. He fell on his right side. He was off 3/10,3/11, and 3/12 due to his injury. Returned to work with dr note 3/14.
While attempting to get #3 calciner to feed, employee swung at #3 bin bottom with hammer and felt pain in his back. Employee restricted to no overtime on 3/19/08.
Employee was carrying a 6" hose and slipped on another hose buried in snow. He fell and hurt his shoulder. He restricted to no overtime for 2 weeks on 6-18-08.
The north skip tripped on "Track Limit Switch." The hoist was accidentally tripped by a subcontractor painter.

EE was removing bolts out of the drive end of the #6 dryer feed screw on GR3 EE was using a bar to keep screws from turning, when he hit the bolt the screw moved causing the drive pin to push finger up against screw housing. This act resulted in sutures to the 4th digit on the left hand and a fracture to the left wrist.
An increase in seepage from the west embankment of our D cell trilings pond was observed. Upon further investigation, a void near the toe of the downstream slope on the west embankment was discovered; resutling in corrective action-including pumping, trenching, diking and diverting water from the plant. Cause still under investigation.
#3 hoist kicked on check point fault. When reset, the drive convertors kicked out. A contactor was replaced so the hoist could be reset.
Overtravel switch on #2 hoist stuck. Once the problem was determined, it was found that the hoist was still operable in case of emergency.
EE was installing a screen in the GR2 Deca centerfuge, when he cut his finger on the screen, which required sutures.
EE strained muscle in addomen while trying to get on Cat. Restricted to no over time for 2 weeks on 2/12/08.
Hoist bearing was vibrating. Investigator checked bearing and noticed that the south pinion bearing was bad and needed replacing so the hoist was ordered down until bearing was replaced.
2 mechanics were in the process of repairing floculent scres in GR3 process plant when they ignited guar gum dust. No injuries were sustained by either employee.

While switching the hoist controls from manual to auto the hoist faulted. it faulted due to a malfunction in the hoist computers drive. The hard drive was changed out to the correct the problem.
EE was reading clipboard, when he slipped and fell on the ice. EE struck his right hip on the ground and indicated that his clipboard struck his ribs (left front). EE limited to no overtime for 2 weeks.
Oiling gearbox for cooling tower fan in GR3, he fell from a ladder straining his right arm and shoulder. Restricted duty: no overtime.

While doing a power move, employee was taking down power cable and felt a pain in his right shoulder. Employee has been working his regular job since the injury but was recently diagnosed with a shoulder tear requiring surgery. Surgery scheduled on 11/19/07 making this injury reportable.
Employee was mining and a piece of ventilation tubing fell striking employee on his head and left shoulder.
EE felt a pop in her thumb while turning off water valve. EE worked until 10/10/07, when she went off for medical treatment, thus making the injury reportable.
EE was walking down stairs and twisted his ankle. EE restricted to no overtime on 9/14/07.

Employee was using a metal wedge and a ball pien hammer to obtain a proper gap between two pieces of pipe that were to be welded. When employee was hammering wedge into place with hammer, he hit his thumb and the hand holding the wedge. Fractured thumb on the left hand between the nail and the first knuckle.
EE twisted his upper back while carrying belt roller.
Employee was swinging a sledge hammer and end of hammer hit his shin. No overtime restriction.
Hoist operator attempted to empty the #3 hoist skip into the bin on the surface & some ore spilled over the bin damaging the guide slippers & gate at the collar. Inspection was performed inspecting hoist & ropes. After this shaft hands installed some new tail rope dividers. #3 Hoist was released @4:30pm on 5/3/07.
Employee cut his finger while cutting a paint can lid.
