The subcontractor's employee was removing solar panels from their frame when a the glass from a broken solar panel lacerated their wrist.

The miner was performing routine housekeeping maintenance for a conveyor belt located inside a sloped steel tunnel. The miner lifted a piece of floor grating, tilting it to the side against tunnel wall and then shoveled material that was being accumulated underneath. Employee was washing the Caterpillar 226 skid steer loader. When he opened the engine cover the radiator cap came loose and hot water was spilled from the engine to his abdominal area.

Allegedly, EE was changing a drill rod of the drilling machine and felt pain in his lower back.

EE WAS REPAIRING A PIECE OF MACHINERY AND SOMETHING HIT HIS RIGHT THUMB.

HE WAS GOING TO LIFT A WOOD PALLET AND HE GOT PINCH WITH NAIL IN HIS LEFT HAND.

CHECKING A PORTABLE COMPRESSOR THAT HAD TURNED OFF BECAUSE THE COMPRESSOR NEEDED WATER. WHEN TAKING OFF THE TOP, THE EMPLOYEE THOUGHT IT WAS COLD ALREADY BUT THE WATER WAS STILL HOT AND BURNT HIS LEFT ARM.

EMPLOYEE WAS TRYING TO TAKE OUT A PART FROM THE TRUCK WHICH COULD NOT COME OUT. WHEN HE FINALLY WAS ABLE TO TAKE IT OUT HE PULLED IT AND CUT HIS INDEX FINGER OF HIS RIGHT HAND.

HE WAS WORKING WITH A DUMP TRAILER AND A PIECE OF METAL FELL FROM THE DUMP, HITTING HIS LEFT SHOULDER. PUTTING DUMP TRAILER ON THE HITCH.
