An employee was conducting an inspection of the burner in DR-8, Argo Kiln. The employee was wearing a Tyvek dust suit, N95 dust mask, gloves, a hard hat, and safety glasses. As they were exiting the kiln, a small piece of dried soda ash struck their hard hat, and dust from the soda ash made contact with their left eye.

Employee working overtime was manually carrying a 6-foot-wide bottom belt roller, weighing approximately 70-80 lbs., on EE's shoulder. Upon reaching EE's destination, EE began to set the belt roller down when EE felt lower back muscles tense up and spasm.

Installing 14 roof bolts in the 2W Mains, employee experienced pain in shoulder as they lifted the cable bolt to insert it into the roof bolter chuck. EE is advised to rest it on days off, re-evaluate 6th when they receive a full work release. EE notifies Solvay of an MRI scheduled on 20th, 22nd they are seeking Ortho for acute tearing and surgery is scheduled for June 6th, 2024.

Employee was preparing a maintenance kitting pallet and moving material on top of the bottom pallet to stack another pallet on top. Positioning self in a manner to obtain proper leverage to slide a crusher anvil, then applying force to move the crusher anvil, EE felt pain in the lower back. Seeking medical treatment on March 12, EE was not released to work until March 18.

While two employees were attempting to move an angle iron brace one of the workers had picked it up but dropped because they lost their grip before the second employee could grab it. This caused the angle iron to drop about 16 inches the second EEs left foot.

At approximately 9:25 am on Friday the 29th of September 2023 while conducting media loading activities on the Heat Recovery Chamber-6A, at the Vulcan Project site, the employee forced between the pallet of media and support brace inside the HRC. The employee sustained soft tissue injury and minor fractures to the pelvic area.

Employee secured a section of the miner cable with both hands and proceeded to lift the cable upwards to place on the cable hook, then felt a pop in their right shoulder and sharp pain causing them to drop the cable. Received a full work release from the clinic but opted to have surgery after MRI results which became a lost time injury starting 8/15/23.

Attempting to line up the pan line, an employee stepped up on a relay bar and placed hand on top of the spill plate to get a better view. As EE was looking towards the headgate, a large rock slid down the pan line and slid across EE's left hand, pinching it between the rock and the spill plate causing a laceration.

Employee was working on un-salting a production line. EE was bleeding steam off the line, isolating the steam, taking the hose off to drain and open the line. In doing so, EE turned and hit EE's head, spinning EE around and EE's hand went under the hot condensate.

Employee received surgery 11/10 for torn rotator cuff after a couple months of pain that was not improving. MRI revealed full thickness perforation of supraspinatus tendon and concomital tear; supra- and infra-spinatus tendinopathy, partial and full thickness tears and bursal surface fraying. Degenerative arthritis in AC joint, bursopathy, and mild nonspecific synovitis.

A surface mechanic suffered a laceration to left middle finger, while picking up a volute liner which had a sharp edge, cutting through EE's gloves, requiring three sutures.

Removed a counter weight guide rail on a bucket elevator to work on a bearing behind it and when re-installing it, the lower portion slid off of the base and started to fall downward. As it fell, the injured employee grabbed the rail so it would not fall towards EE. This resulted in EE's left index finger being pinched between a stop block and the counter weight frame.

Employee was adjusting fly ash system when a piece of material blew into EE's eye. EE was checked by a physician and received a prescription antibiotic ointment for precautionary purposes. This injury report was overlooked during furloughs, etc. amidst the COVID crisis.

Employee was installing roof bolts. The steel plugged, the employee struck the steel with the drill wrench. Upon striking the steel, the socket end of the wrench broke causing the socket to ricochet off the mast and struck the employee in the thumb.

Team member was lining a rail switch when team member felt a pop in left chest/rib area. Team member continued to work and finished shift. On 8/5/2019 team member went to a doctor where xrays were performed and a steroid was prescribed to help with swelling. No lost time, no work restrictions were placed.

NAIS crew was hydroblast cleaning the inside of tank. EE wanted to shotgun a different area of the tank but there was a slab of material approx. 30-40 lbs where EE wanted to stand. EE tried to pick up the material to move it out of the way. EE lost control of grip and the material fell on EE's right hand. EE was able to get hand out from underneath the material and exited the tank.

A piece of a cone shaped plate was being lifted into place. The plate started to tip over and the supervisor said to let it fall because it was still attached to the chain fall. While the supervisor was talking, EE reached out to grab the chain and cut left middle finger on top of the plate. EE had not yet put gloves on.

A mine crew was welding sections of Drisco pipe together for use by Mine Tailings. In the process of aligning two sections of pipe in the welding machine assembly, the employee's hand was pinched between the sections of pipe. This resulted in a non-displaced fracture to the first knuckle joint of the right index finger.

Team member received a papercut to their right index finger on 7-5-18. Washed the cut out with soap and water and put latex glove over it. 7-12-18 Team member went to dr for antibiotics for infection to the papercut. 7-17-18 EE was taken to hospital for care and to prevent septic shock.

Employee was hauling a load of roof bolts and resin to a production panel. The bolting resin stack had worked out of position in transit. The employee attempted to manually re-adjust the bolting resin, as ee was pushing the resin back into the proper position, ee felt a "pop" and discomfort in lower right leg. EE was placed on restricted duty beginning 4/11/18.

An employee was setting up the first piece of decking on a new overcast. As the crew set the side walls, the first piece of decking rotated and struck the employee. Their left little finger was pinched between the decking and floor. The employee was treated on site, taken to the local hospital, and the wound required stitches to close. The employee was released back to work.

A Mine Maintenance employee, that was wearing cut resistant gloves, was repositioning a sprocket the employee was welding. In the process of moving the part, finger was pinched between the sprocket and another tool. The employee was treated on site before being taken to the local hospital where the wound was closed with twelve stitches. The employee was released back to work.

Removing a table and vice from a drill platform. Removing the top T bolt and was starting to remove the bottom T bolt, when the vice rotated and pinched the employee's finger between the vice and the bolt. Requiring nine stitches to close the wound to the tip of right middle finger. An x-ray showed no fracture. The employee was released on restricted duty.

Working to connect piping in mine, EE used EE's shoulder to support the pipe while lowering the pipe hanger. Felt a pop in EE's right shoulder. Reported on 9/11/16, taken to be evaluated and given a full release on 9/13/16. 9/15/16, the employee requested an MRI and was given a second full release. 10/6/16 the employee was seen by the company doctor and placed on restricted duty.

Working in an area with accumulations of crystal & hot liquor, felt a sharp pain in left heel, exited the area and found a small blister because of a tear in the back of rubber boots. The burn was minor and EE worked the rest of the shift, next shift and went on scheduled days off. On 9/16 the total area had grown in size, went to the doctor and was placed on restricted duty.

The employee was adding 25' of vent tube to a high velocity fan when dust was dislodged from the tube which was blown into eye from the side around safety glasses. Employee's eye continued to be irritated so employee notified supervisor on 8/25/2016 and was seen by an Optometrist. Employee was diagnosed with an eye infection and given antibiotic drops.

Troubleshooting SC-42 screw, after removing the lid, the employee was lowering it to the deck and caught his finger between the bracket and the lid causing a tuft fracture of the left ring finger.

Employee was stepping off railcar ladder. He missed the bottom step with his right leg his left leg was caught on the side sill grab iron and he hyper extended his knee. On October 20th he received an MRI. On October 29th MRI results showed a complete tearing of his ACL and MCL. Employee is off work until ACL an MCL are repaired.

A mechanic was replacing a "T" section of pipe, which to align was suspended from two points. One of the points was a section of 3" x 3" x 3/8" angle iron supported by structural I-beams. While maneuvering the horizontal pipe, the section of angle iron shifted from between the I-beams allowing the angle iron to fall and strike the mechanic on the hand.

EE and another ee were instructed to clean out EL-11 pit. They cleaned the grate covering the pit, then decided to remove the grating to better access the pit. EE was holding the grating and let it slip out of his left hand resulting in a tendon tear in his right arm at the bicep.

An employee was removing a shuttle car tire to work on the steering. He removed all the lug nuts and separated the wheel from the hub, 4 to 6 inches. Picking up the tire with both hands, the rim and tire slid back toward the hub and his finger was smashed between the tire and a wheel stud.

Employee injured on 8/18 with no reportabilty. On 9/10, after a follow-up medical visit, the employee was directed not to return to work until further healing takes place. Decoupling a hose from a guar line, shut the ball valve, bled the pressure, opened the pump box drain valve, start decoupling hose, was sprayed with residual condensate left in the line in the abdomen.

Opening a condensate flush valve on a tank already in overflow, the hot liquor inundated the overflow of the tank causing it to spill over the top of the tank contacting the employee on his left side from the top of his shoulder down his left arm, chest and back.

The employee stepped on to the operator cab floor of the shuttle car with his right foot, then extended his hand upward to reach for a roof bolt plate and pushed off with his left foot. The employee felt pain in the interior part of his left ankle and foot. When the employee stepped down he could not apply weight to the left foot.

The employee accessed a stationary railcar to release the hand brake. While applying pressure to the hand brake she felt a pain in her right forearm. She was diagnosed with having inflamed tendons in her forearm and released to work. Upon subsequent follow-up with a specialist on January 14, 2013 a cast was placed on the arm which now constitutes a medical treatment case

An employee was un-hooking a ratchet strap from a bundle of mesh which had stored energy. As soon as he pulled the strap the bundle of mesh pinned/bounced him off of the rib. This sudden release of energy struck the employee's right shoulder.

An employee underwent an MRI to determine the nature of the injury associated with his right shoulder. On 2/27 the MRI was reviewed and he was diagnosed with a partial tear to the rotator cuff. He continued to perform his assigned job and is awaiting a 3/14 appointment with a surgeon to determine if surgery is needed repair the damage to his shoulder.

On 11/13 the employee was manually handling belt structure. On 11/14 he stated that his left shoulder was bothering him for some time to the point that he could not raise his arm, perform physical work activities and the pain would reside during days off. An MRI on 11/15 shown an acute tear to the left rotator cuff.
