While EE was walking up 89-817/818 belt line tube dust hit employee's hard hat, and as the employee looked up, dust entered the EE's left eye. Co-workers assisted the EE with eyewash, then the EE was taken to medical treatment.

Showing 50 of 373EE was installing idler. Pushed on idler to put bolt in to hold idler from moving EE felt a pop in their right wrist. Medical treatment sought. Work restrictions issued due to sprain.
EE was moving an idler for conveyor. One end was in the air and fell on EE left foot missing the steel toe of EE's boot by an inch. Idler is 82" long by 6" wide.
EEs were in the process of removing old belting. Roughly 30lbs of belting per piece. EE lifted the old belting up with assistance from another EE onto a cart to take out of tunnel. When they bent over to pick up another piece they felt a sharp pain across their lower back.
While inspecting overhead conduit for shorts and looking up, product fell between an EE's eye protection and hard hat from above and entered their eyes. The EE and supervisor flushed the eyes, but the EE was still feeling discomfort and pain in the right eye and was transported to an eye clinic for treatment.
Installing Brick at Black river in kiln#1 using a pneumatic brick machine. Labor that was injured had improper hand placement on deck of brick machine. Bricklayers move the brick machine and did not communicate on brick machine adjustment.
EE on 12/16/2025 reporting back pain, due to an injury on 12/13/2025. They started changing a return roller and felt pain and reported to supervisor, but thought was going to be alright. On 12/16/2025 employee is requesting medical care and taken to local ER.
A temporary EE stated that sharp pain was felt while working with a second EE to install new bags onto cages for a dust collector. The injury was reported at the end of the shift and the EE declined treatment. The next day, the EE stated that the pain was bad. The EE was seen at a doctor for lumbar pain, which resulted in work restrictions.
ee was removing idlers from conveyor for a belt change. when they went to move the idler to the floor, their finger got pinched/rolled in between the cans causing a laceration / split to right pinky finger.
On October 21, 2025, Schad personnel were performing demolition work on Kiln #4 at Carmeuse Lime. While moving a debris buggy into the kiln, the buggy became lodged on the ramp at the kiln entrance. A Schad employee attempted to free the buggy manually. When the buggy became dislodged, it shifted unexpectedly and struck the employee's right knee, resulting in a contusion.
ee was assisting with a belt pull. When pushing on the belt, felt a pop in EE's knee. was evaluated and was put off work.
ee was climbing up stairs; reached out to grab the handrail and felt pain in middle back area.
Injured was clearing dust chamber with a shovel when hot material entered EE's boot causing a burn to EE's ankle.
While using water hose to clean under belt in stone tunnel, mud splashed onto hand.
ee was pulling the slack out of a wash hose when they felt a pop in their back. Incident occurred on 8/15/2025 - ee was evaluated by medical professional and released to work without restrictions. Pain was not getting better so went back to the doctor and was put off work on 9/8/2025.
ee was throwing scrap steel into the skip when EE reaggravated a strain in EE's shoulder that occurred the day before.
ee was moving a diesel pump in the quarry. ee picked up the suction hose and felt a strain in their back.
EE twisted knee while performing maintenance work on a screen (no report filed at this time. A couple days later, EE felt it "pop" again and was unable to get out of the loader due to the pain. Incident was reported at this time and ee sought medical treatment. Was informed on 7/11 that EE had been put on work restrictions.
EE was trying to lift new bearing housing into place on take up. EE said EE was in an awkward position and said EE hurt EE's left wrist.
Employees had a shaft in the upright position in the elevator. when they arrived their floor, one ee held the top while the other bent down to pick up the bottom end. Upon lifting, the ee felt a pop sensation in the groin area. The shaft weighed appx 100 lbs according to ee's. EE sought medical treatment on 3/6 and was put on work restrictions.
Employee was rolling a piece of pipe on the welding table and it hit a piece of angle that was also on the table causing it to fall off and land on employee's left foot just behind the steel toe leaving it bruised and swollen. Employee did not seek medical attention until a later date- ee was put off work eff 3/6/25.
Injured was moving a 55 gallon drum in the warehouse when the injured felt pain in knee. Incident occurred on 2/16, but not reported until 2/18 when medical attention was requested.
While removing a multi tone screw EE's left leg was stuck when load shifted.
EE reported pain in right shoulder after attempting to move a water line connected to a relief valve. As EE was moving the hose it became charged with water causing EE's right arm to be thrown up and back.
EE was lifting trays of stone for sampling when they felt a strain in their shoulder. Incident was reported on 12/10. EE sought medical treatment on 12/18 and was issued work restrictions and continued medical treatment.
Kiln Assistant was moving an oxygen tank from the storage rack to the CAT 908 loader bucket for transportation. When EE bent over to lift up the bottom end of the tank, EE felt a pull/pop in the outside of EE's right hamstring.
EE was lifting a piece of plate steel into place with left hand - felt pop in bicep upon lifting. Suspected rupture of tendon; currently under medical evaluation.
4/8/2025 management was notified that employee was reporting an incident/injury that they state occurred on 11/13/2024. Employee states injury occurred when pulling a fire hose over a hand railing located at the lower deck of the fines dock. Employee states injury is herniated disks in back.
Ee was moving/loading reducers for transport when one shifted causing hand to get pinched between the two items resulting in laceration/stitches.
Employee was assisting in removal of kiln 1 man door. Employee raised employee's glasses to wipe away sweat when something entered employee's eye. Employee flushed the eye and continued working. On 9/25/2024, employee sought medical treatment when the condition didn't improve.
EE was assisting in moving an electric motor when EE felt pain in EE's back. Three mechanics were dragging an electric 30 hp motor over a steel graded floor when one of the employees complained of back pain.
EE was heading to the top of the mill in the elevator to check an alarm. Upon reaching the 12th floor EE got out of the elevator and went to close the elevator door. EE was pulling the Strap on the outside of the door and EE's hand got crushed between the doors. Work restrictions issued after medical evaluation.
The EE was loading wooden cribbing blocks weighing approximately 20 pounds when they reported pain in their right shoulder. It was determined the blocks were being lifted in the bed of the service truck without lowering the tailgate.
Injured was shoveling material out of a hopper when EE felt pain in their wrist.
EE bent down to remove toilet & felt a strain in back - reported at time of incident, no treatment sought at the time. EE was treated on 8/12/24 in ER due to ongoing/increased pain. Was put off work - became reportable at that time.
Injured reports that EE was dragging a 4" corrugated vacuum hose when EE felt pain in EE's back. Injured did not report injury until 7/10/2024 when EE requested medical attention. Injured was given work restrictions on 7/10 which made the injury reportable.
EE was rolling fuel hose back into hose reel when end kicked up and hit EE in the mouth resulting in chipped tooth. Treatment sought on 8/6/24. Confirmed type of treatment that was provided with dentist on 8/21.
EE picked up a large stone to remove it from the roadway. As EE picked it up, it broke into pieces. A sharp edge of the stone cut the ee's finger. Laceration requires stitches. No further restrictions or treatment administered.
Employee was straightening a piece of pipe by hand. The pipe kicked back striking right thumb. Went to Dr. 6/12 and was released to full duty. Went back to Dr. on 6/20 and was released to work with restrictions and a brace with possible torn ligament in right thumb.
Employee was attempting to remove an oil drain plug, and when it broke free, employee smashed pinky finger on a pipe
Employee statement-Checking Dust collector at the re-crush and got a piece of lime into left eye. Attempted to flush myself and was unsuccessful. Got help to flush eye again. Found after going to Urgent Care that I still had a piece of lime in my eye as well as a scratch on my eye.
EE lifted a 2 x 12 plank to move it out of the way - as EE lifted, EE heard a "pop" in left bicep followed by shooting pain down to fingers on left hand. EE did a report only at the time, but sought treatment a few days later and has been put on work restrictions with further medical evaluation.
Injured was attempting to close the rear engine door on skid steer when they smashed a finger in the door, causing a fracture to the tip of the finger.
Injured was assisting another employee clean hydrate slurry from a 3' deep bucket elevator pit. One employee left the area for a few minutes. When they returned, injured was standing in the slurry in the pit wearing jeans and standard work boots. Later that evening, they began having leg irritation and sought medical treatment the following day.
EE reported discomfort in ankle - no specific event or incident contributed to the onset of pain. Following day, EE was experiencing increased swelling/pain and wasn't feeling well. Went to ER - admitted for severe bacterial infection in ankle assumed to be attributed to something in the water at work. EE was working in wet area at time of report. Still under investigation.
Installing conveyor. Pinched right ring finger between conveyor and another metal piece. Laceration of tip of right ring finger.
Employees finger was pinched between screen deck media & frame resulting in laceration and fractured finger tip.
While closing a manual slide gate beneath a hopper, EE smashed right-hand pinky finger between the slide gate and the hopper, resulting in a distal fracture.
On 3/26/2024 EE reported that EE had an accident on 2/13/2024. EE states EE was struck by a piece of chain in the front of EE's mouth causing tooth damage.
The EE was installing a drive line on a Caterpillar 980M front end loader in the shop area when left middle finger became pinched. Seven stitches were required to close the open wound.
