While EE was walking up 89-817/818 belt line tube dust hit employee's hard hat, and as the employee looked up, dust entered the EE's left eye. Co-workers assisted the EE with eyewash, then the EE was taken to medical treatment.

EE was exiting skid steer inside the maintenance garage. EE tripped and fell forward onto their hands and knees. The EE's knee hit the concrete floor first and felt a twist in their knee. EE had pain in that knee and was taken to urgent care. EE was given a work restriction of no kneeling or squatting for 5 days.

Injured was poking the built-up coal out of the chute to the transfer screw using a 1/4" piece of round stock steel. The poker made contact with the screw conveyor below, causing it to pinch their finger and cause a laceration on the ring finger of their left hand.

On 04/15/2026 management was notified by EE that they had struck right knee while exiting a Toyota Forklift. They further stated that this incident happened on 03/31/2026 at approximately 11:00am. EE has now been seen to have their knee drained due to swelling on 4/16/2026 and 4/20/2026. No lost time or restrictions at this time.

At approximately 6:30am on 03/30/2026 EE informed incoming production supervisor that at approximately 10:00pm the night prior (03/29/2026) they had felt a "pop" in their left wrist while turning a wrench. EE denied medical care when asked and offered a ride to the hospital by the supervisor three times.

EE was grabbing a fuel can in the garage when they were walking, they stepped over the hammer bit, lost footing and fell. EE has a laceration on left hand and right knee. Employee is complaining about left arm pain. Taken to medical facility for evaluation.

While scaling in 33 West, a rock fell from the back and struck the scaler boom jolting the cab causing the operator to hit the windshield with EE's forehead and right shoulder. The operator was wearing a seat belt. EE as a mild concussion and a strained A/C shoulder joint. Scaler equipped with a lap belt style seat belt.

EE was moving an idler for conveyor. One end was in the air and fell on EE left foot missing the steel toe of EE's boot by an inch. Idler is 82" long by 6" wide.

EE was cleaning on top of the kiln silos. As they went to straighten up to lift the wheelbarrow they heard and felt a pop in their back and was in a lot of pain. Dr. released them with no restrictions, went to Ortho on 4/13/26 due to continued pain. Dr. gave restriction and EE is now off work.

Employee was descending a 6' ladder. When EE stepped on the ground, EE's knee gave out and EE fell to the ground. Incident occurred on 3/17 but did not become reportable until 4/21 when employee was given a brace and PT was ordered.

On 3/10/2026 6:30 p.m. management was made aware that the surface trash compactor storage container had material inside on fire. This metal container measures eight feet by 22 feet and is used to transport trash from the mine property to an offsite dump. MSHA was notified and the fire was extinguished. There was no damage to property or injury due to this incident.

Employee was coming in to begin the shift when they slipped (no fall) in water on the men's locker room floor. They reported the incident to their supervisor but did not seek medical attention until 4/6/2026 and was given work restrictions.

On 3/6/2026 3:20 a.m. management was made aware power had been lost to the slope hoist. This being the secondary escapeway, all mine personnel were evacuated using the primary escapeway. MSHA was notified when power restored at 4:02 a.m. No miners were injured due to this incident.

EEs were in the process of removing old belting. Roughly 30lbs of belting per piece. EE lifted the old belting up with assistance from another EE onto a cart to take out of tunnel. When they bent over to pick up another piece they felt a sharp pain across their lower back.

Employee was following wires in the engine compartment for a light that was not working on a piece of equipment when their fingers contacted the fan blade of the engine cutting 2 fingers. They received 7 stiches in one finger and was put on restricted duty for 10 days.

EE fell 10ft 10in while leaning out a 11-1/2 in opening between a screen and adjacent handrail trying to look at the screen springs.

The EE walked to a roadside berm that had 2 feet wide opening to allow water to exit the roadway into a ditch. While standing at this opening, the ice underneath EE broke, causing EE to lose EE's balance. EE then slid down the ditch wall 4 feet before EE's left foot contacted a tree branch. This contact resulted in a dislocated and fractured ankle.

On 2/4/2026 an employee was using a spud bar to clean material from a clogged chute at the surface crusher when EE injured EE's right pinky finger. EE refused treatment at time of incident, but after seeing a doctor on 2/26 & 2/27 it was determined EE's finger had been fractured.

EE reported to management at 6:50am on the morning of 02/05/2026 that they had been experiencing lower back pain since 01/31/2026 and that they had received medical treatment and been released without restriction on 02/03/2026 and called off of work on 02/04/2026. EE was seen at a clinic at 11:30am on 02/05/2026 and was instructed to not return to work until 02/09/2026.

While inspecting overhead conduit for shorts and looking up, product fell between an EE's eye protection and hard hat from above and entered their eyes. The EE and supervisor flushed the eyes, but the EE was still feeling discomfort and pain in the right eye and was transported to an eye clinic for treatment.

EE was hauling tailing pond waste in a Volvo A60. When the EE attempted to dump the material about 30 tons remained frozen in the box and when putting the box back down it slammed hard, shaking the cab and operator. EE did not seek medical attention at the time of the incident. Work restrictions issued on 2/5 following ongoing discomfort and medical treatment.

A contract laborer was positioned on the downhill side of a brick rig used to install kiln brick when they sustained an injury. They had placed their right hand on the brick rig's platform and when the rig was moved a brick fell striking their hand. This resulting in 3 broken fingers.

Installing Brick at Black river in kiln#1 using a pneumatic brick machine. Labor that was injured had improper hand placement on deck of brick machine. Bricklayers move the brick machine and did not communicate on brick machine adjustment.

EE was clearing material from a blocked chute. After removing wet material, the dry material behind it broke loose and slid down the chute causing some hydrated lime to pass underneath EE's safety glasses and into EE's left eye. After rinsing with eye wash and going to ER, EE was prescribed eye drops for scratch on left eye.

EE states that while pulling water hose to back to hose reel on water truck that the EE's foot hooked a frozen clump of material, causing a fall to the ground on EE's left side. EE was seen at urgent care and was issued restrictions due to limited mobility and pain in left shoulder and left thigh.

On 12/18/2025 the vertical shaft hoist (primary escapeway) was taken out of service due to not being able to close the surface landing door. No miners were in the mine when this issue was found. It was determined a bolt on the landing door track needed replaced.

EE on 12/16/2025 reporting back pain, due to an injury on 12/13/2025. They started changing a return roller and felt pain and reported to supervisor, but thought was going to be alright. On 12/16/2025 employee is requesting medical care and taken to local ER.

On 12/1/2025 MSHA was notified that the vertical shaft hoist was being taken out of service due to intermittent issues. The brake alarm was resolved after adjustment per manufacturer specification. There was also a problem with a control wire, which was changed out with a spare. No miners were injured due to this incident.

A temporary EE stated that sharp pain was felt while working with a second EE to install new bags onto cages for a dust collector. The injury was reported at the end of the shift and the EE declined treatment. The next day, the EE stated that the pain was bad. The EE was seen at a doctor for lumbar pain, which resulted in work restrictions.

On October 21, 2025, Schad personnel were performing demolition work on Kiln #4 at Carmeuse Lime. While moving a debris buggy into the kiln, the buggy became lodged on the ramp at the kiln entrance. A Schad employee attempted to free the buggy manually. When the buggy became dislodged, it shifted unexpectedly and struck the employee's right knee, resulting in a contusion.

Injured was reinstalling Raymond mill panels using a 2-ton overhead chain hoist. The chain hoist trolley over-traveled the end of the beam and fell, striking injured on side of head and shoulder, causing EE to strike EE's face on the mill structure.

Intern Process Eng. was providing help with racking of material. While doing so intern pinched EE's index finger on EE's right hand between the rake EE was using and metal door. This resulted in a cut to the back of EE's index finger which required 4 stitches.

On 9/2/2025 two employees were unable to lower the rental JLG manlift basket they were working from due to a blown circuit power fuse. Another manlift on site was used to get the two employees safely to the ground.

after drawing out feeder chute and running it dry, ee locked out belt and feeder and crawled up into chute to investigate a potential obstruction. After poking around with a pry bar, material from the overhead stockpile sluffed off and engulfed ee in the chute. EE was extracted and is recovering at home (lacerations and contusions)
